[Rubella infection during pregnancy. Results of serologic diagnosis 1975-1978].

The sera of 663 pregnant women with unknown immune status were tested for HI rubella antibodies between 1975 and 1978 because of exposure to rubella or presence of clinical symptoms. The majority of sera were from women in the first three months of pregnancy. Comparison of the distribution of antibody titers with those in a control group of 670 nonpregnant women of the same age showed no significant difference. The percentage of seronegative subjects was 8.6 in pregnant and 10.5 in nonpregnant women. Eighteen women (2.7%) became infected with rubella virus during pregnancy. Ten of these were devoid of rubella antibodies, representing 17.5% of the seronegatives. Diagnosis was confirmed by seroconversion and rise in antibody titer. The remaining 8 women had antibodies in the first blood specimen. Diagnosis was established by a fourfold or greater increase in titer or by the demonstration of rubella specific IgM-antibodies in a single specimen. It is of interest that almost all the pregnant women with rubella infection had clinical symptoms such as fever or exanthem. The results of this study show that the aim of complete rubella prophylaxis in women of childbearing age has not yet been achieved. It is therefore necessary to increase the uptake of rubella vaccine by schoolgirls and reduce the percentage of seronegative adult women by single vaccination and vaccination in the postpartum period.

Chlamydia trachomatis as possible cause of peritonitis and perihepatitis in young women.

Of 11 young women with acute peritonitis proved by laparoscopy, seven of whom also had perihepatitis, nine had serological evidence of recent infection with Chlamydia trachomatis; In five of these nine patients high antibody titres to chlamydiae were found without laboratory evidence of gonococcal infection, while the other four had evidence of simultaneous gonococcal infection. C trachomatis may play an important part in peritoneal inflammation previously attributed only to gonococci.

[Serum immunity against influenza viruses A/Hongkong, A/Zürich, A/Victoria and A/Swine in the Swiss population].

The HI antibody against the influenza virus A/Zürich/18/75 was titrated in 2351 sera from 17- to 75-year-old blood donors living in 24 different parts of Switzerland. 518 sera selected from these donors with the same distribution of age and location were tested against the viruses A/Hongkong/1/68, A/Victoria/3/75 and A/Swine/15/31. The percentage frequency of 91.7% and 89.4% positive sera reveal almost complete exposure of the population to A/Hongkong and A/Zürich. But only half of the donors possess antibody against the A/Victoria/3/75 virus. It is therefore reasonable to expect that the A/Victoria virus or a closely, antigenetically related virus will cause influenza cases in the winter of 1976/77. Despite small regional differences the distribution of influenza in Switzerland appears to be uniform, in view of the fact that the geometric mean titre of the donors from different parts of the country showed no significant deviation from the total geometric mean. The largest number of positive sera were in the youngest and oldest age groups and the smallest in the 30- to 50-year age group. Antibody to A/Swine/15/31 virus was found almost exclusively in sera from persons aged 50 or over.

[Electronic data processing system for clinical microbiology].

A computerized clinical microbiology data storage and retrieval system, which was introduced at the Institute of Medical Microbiology 14 month ago, is described. This institute has to perform routine diagnostic microbiology for hospitals in the Kanton of Zuerich including the university hospital. In addition, it serves as a public health laboratory for Zuerich and adjacent districts. Patient and physician data are entered into a data station IBM 3741 and stored on discettes. Each afternoon, these data are printed on special report forms, which then are transferred to the diagnostic laboratories. After completion of the investigation, a copy of this form containing the results is sent to the physician. Every two weeks, the information stored on the discettes are converted onto the magnetic tape "discette". In addition, the original report form, containing the codified results and the fees, are read by an optic reader, which transfers the information onto the tape "report". Both tapes then serve the computer to print the accounts as well as to summarize the results monthly in form of the medical statistics. These provide valuable information to enhance patient care. All data are stored in a cumalative microbiology data bank for later retrieval.

[Use of drugs in the hospital. Retrospective model study].

This study sets out to describe and evaluate antimicrobial therapy in the departments of surgery, medicine, gynecology and obstetrics of a 500-bed general hospital serving a mainly metropolitan population. About one third (605) of 2002 hospitalized patients received chemotherapy, 97 patients being treated for nosocomial infections. In 44% antimicrobials were given prophylactically, chiefly in the surgical department (in 60% of the drug courses) and the departments of gynecology (in 78%) and obstetrics (in 71%). Combinations (excluding sulphamethoxazole/ trimethoprim) were used on the medical wards in 5% and on the surgical wards in 15% of the drug courses. Gentamycin combined with beta-lactam antibiotics was most often used in medicine, while penicillin/streptomycin and ampicillin/cloxacillin in fixed ratios were the most frequent combinations in surgery. Finally, each therapy was evaluated by two experts as either rational, irrational or questionable. 61% of the 681 drug courses were considered rational, 16% questionable and 23% irrational. In the 155 irrational drug courses, one or more of the following criticisms were made: inappropriate chemoprophylaxis (101), wrong drug in chemoprophylaxis (66), inappropriate combination (30), wrong drug in symptomatic chemotherapy (28), no bacteriological diagnosis (26), sensitivity test not considered (17), use of antagonistic combinations (11) and wrong drug in therapeutic chemotherapy (5).

[Study of Brazilian agricultural workers. A medico-socio-psychological study].

UNLABELLED: The general state of health of native Brazilian agricultural workers - a total of 750 people from 3 different plantations in the States of Paraná and Saso Paulo - was examined. The main interest of this study war centred on infectious and parasitic diseases, nutritional conditions as well as social, intellectual factors. OBSERVATIONS: Apart from the high number of cases of helminthiasis, amounting to 80%, the general state of health of the examined subjects was found to be good - indeed better than that of a hypothetical, comparable group of Europeans. Except for the Chagas' disease, by which 5% of the test subjects were afflicted, infectious diseases posed no serious problems. There was no case of malnutrition. The relatively lower intellectual level can not be attributed to any heriditary factor, and could definitely be improved by proper schooling. Corrective measures: Chagas: Since brick houses have replaced the wooden ones for several years, new infections are unlikely. Helminthiasis: In addition to the anthelminthic treatment, sanitary prophylactic measures should be taken. Social-intellectual factors: The following points should be emphasized: elementary schooling on the plantations; teaching at intermediary level; sewing and cooking courses; general hygience.

[Investigations on the influence of Rh immunoglobulin prophylaxis on the immune response to postpartum rubella vaccination (author's transl)].

A preliminary investigation showed that patients with rubella HHT antibody titres of 1:8 or greater did not show a significant rise in the antibody titre following rubella vaccination. The rubella antibody titre was determined in 651 obstetric patients. Of these, 43 (6.6%) had no significant antibodies to rubella (HHT less than 1:8) and were included in the present investigation. Patients in Group A received 0.5 ml. of the rubella vaccine Meruvax on the fifth postpartum day. Patients of Group B(Rh negative and Rh positive) received 250 mug anti-D in a 16% gammaglobulin solution intra-muscularly 48 hours postpartum and the rubella vaccination 3 days later. Three weeks following the rubella vaccination the mean geometric rubella antibody titre had risen +/- 1 Standard deviation to 19.6 +/- 7.7 in Group A (17 patients) and to 18.0 +/- 6.3 in Group B (12 patients). Six weeks following the rubella vaccinations Group A (19 patients) showed titres of 61.7 +/- 2.9 and Group B (14 patients) showed titres of 70.0 +/- 2.6. There was no statistically significant difference (greater than 0.5). The conversion rate in both groups was 100%. Patients can therefore be vaccinated against rubella in the postpartum period even though they will receive a concomitant prophylaxis with Rh immunoglobulin.
