Pregnancy complicated by heparin associated thrombocytopenia: management by a prospectively in vitro selected heparinoid (Org 10172).

A pregnant woman treated with unfractionated heparin for pelvic vein thrombosis in the 26th week of her first pregnancy developed heparin-associated thrombocytopenia. Diagnosis was verified by the heparin induced platelet activation (HIPA) assay, which revealed cross reactivity to various LMW heparins, but not to the LMW heparinoid Org 10172. Upon intravenous (i.v.) anticoagulation with the heparinoid Org 10172 the platelet count returned to normal within 6 days and remained stable throughout the entire treatment period. After 3 weeks i.v. treatment with Org 10172, administration was changed to the subcutaneous route. At term a healthy boy was delivered spontaneously. No Org 10172 was detected in the cord blood, while therapeutic levels were measured in the maternal blood. The infant's platelet count was normal, but serum from the cord blood induced platelet activation in the presence of heparin, indicating the transplacental passage of heparin-dependent maternal antibodies.

Native fluorescence of the cervix uteri as a marker for dysplasia and invasive carcinoma.

A non-invasive and non-destructive fluorescence technique developed recently for an in situ detection of melanomas has been applied for determining in vitro dysplasia and invasive carcinomas in the cervix uteri. The cervices uteri exhibit a fluorescence band with a peak at about 475 nm if excited with 365 nm. The fluorescence intensity increases concomitantly with the degree of dysplasia, ranging from 30 counts/100 ms (healthy) to approximately 200 counts per 100 ms (CIN 3). At the rim of a malignancy, the intensity is 250 counts/100 ms and higher and decreases towards the healthy region. In the tumor region, the intensity is about zero or very small, at the most. The naturally occurring chromophore being responsible for the fluorescence observed seems to be NADH.

[Acute dermal gangrene: a postoperative complication. With an example from obstetrics].

In the anglo-american literature the acute dermal gangrene is characterized as a cutaneous syndrome of infectious etiology which is usually leading--postoperatively or after an injury--to necrotizing destructions of the cutis, subcutis, subcutaneous fat, fascia or muscle. Depending on the localization of the involved tissue a superficial progressive bacterial gangrene is distinguished from a more deeply localized necrotizing fasciitis. Diagnostic and therapeutic considerations of these extremely rare syndromes are described in connection with an obstetric case which has been seen by us recently.

[Morphologic comparison of stroma rich and stroma poor tumors in the agar colony-forming test].

In the study reported here the morphologic relationship between the original tumor and a colony cultured in vitro was investigated in stroma-rich and stroma-poor tumors. The colonies, cultured in a double-layer agar system were marked with graphite and bedded in paraffin after centrifuging. Subsequent preparation of the microtome sections was in accordance with customary histologic methods. Taking an ovarian carcinoma as an example, far-reaching agreement between the original histologic findings and the section through the colony in the light-microscopic image was demonstrated. In contrast, the tumor cell colonies in mammary carcinomas, which are typically representative of stroma-rich tumors, generally showed less similarity to the histologic structure of the mother tumor, since the formation of connective tissue cannot be induced due to the lack of mesenchymal elements in the agar culture. However, under comparable in vivo and in vitro conditions, e.g., tumor manifestation in performed cavities, there was, also with mammary carcinomas, a similar morphologic analogy between the original tumor and the clone, as in cell-rich neoplasms. The possibility of identifying malignancy directly in colonies and knowledge of their morphologic relationship to the primary tumor increase the relevance of chemosensitivity testing in colonization tests.

Mechanically induced tube damage in the artificial hydrosalpinx.

One-sided artificial hydrosalpinx was caused in mature New Zealand rabbits by proximal and distal ligature. After 2 weeks the average secretion accumulation was 2.1 ml, after 8 weeks 6.3 ml. A fold relief could no longer be detected after 8 weeks. The light-microscopical study of the epithelium showed signs of cell degeneration with cell dedifferentiation, pyknosis of the nucleus, and perinuclear aerolae. The ciliation of 66% ciliated cells for the normal oviduct (pars ampullaris) was reduced to 15% after 8 weeks of artificial hydrosalpinx. Based on the hypertrophy of the muscularis the percentage loss of E2 receptors within 4 weeks was smaller in comparison with the percentage reduction of the ciliation during the same time. Finally, after 8 weeks an amount of 15% for the estrogen receptors as well as for the ciliation was achieved--both compared to the untreated oviduct. It is possible that the oviduct damage, caused only by the mechanical influence of the secretion congestion, is the reason for the unfavorable pregnancy rate after salpingoneostomy of a chronic atrophied hydrosalpinx.
