Selecting anaesthetists: the use of psychological tests and structured interviews.

To test the predictive validity of a selection system for Senior House Officers (SHOs) and registrars in anaesthetics, 140 doctors short-listed from 635 applications between 1980 and 1987 were assessed by a semi-structured interview assessed and a personality questionnaire (Cattell 16PFQ-form C). The 62 doctors selected were followed up for between 3 and 8 years. Future performance was predicted from the psychological tests and by the interviewers. Academic, clinical, behavioural, and overall performance were used as criteria of outcome. Correlation coefficients between prediction and outcome measures were statistically highly significant (P < 0.01). Using multiple regression, equations could be derived from five of the Cattell personality factors to predict overall performance. Personality measures discriminated significantly between the best and poorest performers. Interview predictions were also statistically significant (P < 0.01). The method provides a blueprint for the effective selection of junior anaesthetists. Wastage in terms of those leaving the specialty was 16%.

Oral bromazepam in premedication. A comparison with diazepam.

Bromazepam, a relatively newly benzodiazepine with marked anxiolytic effects, was compared in a randomised, double-blind manner with diazepam for its effectiveness as an oral premedicant drug. A scoring system was used to assess sedation, relief of anxiety, nausea, and cardiovascular effects in two groups of women having gynaecological operations. No difference was demonstrated between the effectiveness of the two drugs.

Metoprolol in anaesthesia for oral surgery. The effect of pretreatment on the incidence of cardiac dysrhythmias.

Metoprolol, a cardioselective beta-adrenergic blocking drug, was given, with a placebo as an alternative, in a double-blind trial to 60 patients who were undergoing removal of third molar teeth under general anaesthesia. Dyshythmias occurred in significantly fewer patients in the metoprolol group than in the control group, and when ventricular dyshythmias occurred they were abolished in every case by the administration of metoprolol. It is concluded that metoprolol is effective in preventing cardiac dysrhythmias occurring during oral surgery carried out under general anaesthesia.

Nominal group technique. A method of decision-making by committee.

Committee-work which involves decision-making from a range of alternatives is frequently time-consuming, inefficient, and frustrating. Nominal Group Technique is a structured method of group decision-making which allows a rich generation of original ideas, balanced participation of all members of the group, and a rank-ordered set of decisions based on a mathematical voting method. A brief description of a Nominal Group Technique session is given and its application to decision-making by committees of anaesthetists is illustrated by an example.

Leakage of oxygen from blood and water samples stored in plastic and glass syringes.

Theory and experiment showed that samples of blood and water stored in 2-ml and 5-ml syringes made of polypropylene, polystyrene, or S.A.N. co-polymer exchanged oxygen with their surroundings. In the first hour the exchange was due mainly to equilibration with the plastic of the syringe and only in small degree to permeation through the plastic. With high initial tension or with blood of low haemoglobin concentration the exchange can result in errors in Po(2) of up to 6% in two minutes and 16% in 30 to 60 minutes. With all-glass syringes the exchange was much slower but, even so, after 24 hours was important in all but a few of 18 interchangeable glass syringes. Therefore unless analysis can be started immediately all-glass syringes are to be preferred, and for prolonged storage even these should be selected.
