Inhibition by nociceptin of neurogenic inflammation and the release of SP and CGRP from sensory nerve terminals.

Pretreatment with the novel neuropeptide nociceptin (20 micrograms kg-1, i.p.) caused an inhibition of plasma extravasation evoked by antidromic stimulation of the saphenous nerve or by topical application of 1% mustard oil on the skin of the acutely denervated hindleg of the rat. In contrast, it did not affect non-neurogenic inflammation evoked by s.c. injection of bradykinin after chronic denervation. Release of substance P (SP) and calcitonin gene-related peptide (CGRP) from rat isolated tracheae in response to electrical field stimulation was diminished by nociceptin (100 nM). It is concluded that nociceptin inhibits the release of sensory neuropeptides from terminals of nociceptive neurones.

Myocardial and gastrointestinal release of vasoactive intestinal peptide during experimental acute myocardial infarction.

BACKGROUND: Vasoactive intestinal peptide (VIP) acts as a vasodilator on coronary and gastrointestinal arteries. During coronary occlusion, the locally released VIP may exert a protective effect on the heart, but it may aggravate the shock state through its vasodilatory effect in the gastrointestinal tract. METHODS: After left thoracotomy, the left circumflex coronary artery (LCx) was prepared, and a pneumatic occluder was introduced around it. After 60 min of coronary occlusion, the LCx was reperfused in six dogs (reperfusion group), while in another six the occlusion was maintained for 6 h (occlusion group). Five dogs served as sham-operated controls. The plasma concentration of VIP was determined at baseline, after the 60 min occlusion and 10 min, 3 h and 6 h after reperfusion, or 3 h and 6 h after continuous occlusion in the coronary sinus and in the femoral and portal veins. RESULTS: The plasma VIP concentrations in all three vessels were increased after 60 min of LCx occlusion. During the 6 h constant coronary occlusion, concentrations remained increased in both the coronary sinus and the portal vein, but not in the femoral vein. In the reperfusion group, 10 min after reperfusion, the plasma concentrations of VIP in all three vessels had decreased. CONCLUSIONS: Coronary artery occlusion causes a long-term increase in plasma VIP concentrations that decreases after reperfusion, when measured in the portal vein and coronary sinus, but not in the femoral veins.

Experiences with the rectal use of trimethoprim.

The possibility of rectal use of trimethoprim was studied. The in-vitro liberation of the drug from 24 different suppository bases was examined and the results used to select bases for in-vivo examination. The in-vitro liberation from the suppositories containing 50-200 mg trimethoprim was studied by the method of dynamic diffusion, and the released drug content was measured spectrophotometrically. The in-vivo examinations were performed in anaesthetized rats. The concentration of trimethoprim in blood was determined by bioassay. The absorption of the drug in the form of oral suspension, rectal solution and suppository was also studied. The pharmacokinetic parameters obtained after blood-level curve fitting were compared by use of the MedUSA 1.6 program. The best in-vivo results were achieved with the lipohydrophilic Witepsol W 35 vehicle containing 10% polysorbate 20 and 10% polysorbate 61 (bioavailability = 63.8%) and with Witepsol W 35 containing 10% polysorbate 60 (bioavailability = 63.8%). The results for hydrophilic Macrogol 1540 vehicle containing 5% of Macrogol 400 were only slightly worse (bioavailability = 52.9%). In the case of the lipohydrophilic Witepsol W 35 vehicle with 10% polysorbate 20 and 10% polysorbate 61 content a significant negative exponential relationship was found between the administered doses and their respective bioavailability values; this tendency was also observed during in-vitro examinations. When incorporated in the appropriate vehicle trimethoprim was absorbed well. With three vehicles the extent of absorption exceeded that for oral administration on the same model (bioavailability = 38.8%). Trimethoprim rectal suppositories, which are formulated with the vehicles having the best in-vitro and in-vivo results, are suitable for clinical pharmacological investigation.

Elevated levels of plasma vasoactive intestinal peptide in human acute myocardial infarction.

The plasma levels of vasoactive intestinal peptide in peripheral vein were measured in human acute myocardial infarction. The plasma vasoactive intestinal peptide level was increased within 1 h after the onset of the symptoms of acute myocardial infarction (Group 1, n = 9), compared with normal values (6.3 +/- 0.7 vs. 2.8 +/- 0.9 pg/ml, P < 0.05). Two or more hours after the onset of acute myocardial infarction there was no subsequent increase in peripheral plasma vasoactive intestinal peptide levels (n = 26). Ten days after the onset of acute myocardial infarction, the elevated plasma vasoactive intestinal peptide levels in Group 1 had normalized (3.5 +/- 0.5 pg/ml).

[A case of detection of pulmonary sequestration during the early stage of pregnancy and subsequent successful therapy].

As result of routine ultrasound screening during the pregnancy, the number of congenital abnormalities causes severe perinatal respiratory or circulatory failure detected antenatally is rising. The severity of the extralobar pulmonary sequestration is variable. The intrauterine diagnosis and the proper prognosis is difficult because of the rarity of this abnormality. The Color-Doppler ultrasound method has a great importance in the diagnosis. With our case report we would like to help in the better understanding of this rare abnormality.

The photorefractive excimer laser is not only able to alter the refractive power of the corneas but also suitable to treat superficial corneal pathologies. Authors performed phototherapeutic keratectomies (PTK) in 80 eyes of 77 patients. The age was on average (+/- S.D.) 49.4 +/- 22.03 years. The follow-up time varied between 3-24 months. All treatments were performed with the Aesculap Meditec MEL 60 ArF excimer laser, working in the 193 nm extreme ultraviolet-C range. Authors conclude that the best postoperative results can be achieved with PTK in cases of recurrent corneal erosions and superficial corneal opacities. Favourable outcome could be expected in treatment of infectious corneal ulcers. In cases of bullous keratopathy the subjective complaints decreased and it was successful to postpone the time of perforating keratoplasty. The PTK is not recommended in case of corneal Bowen disease and recurrent pterygia. A hyperopic shifts was observed in 2 eyes. Besides, the traditional surgical methods the phototherapeutic keratectomy is an important progress in treatment of superficial corneal pathologies. Applying the method, the recurrent erosions healed quicker, in most of the cases the transparency of the cornea could be reestablished in the pupillary area and the need for perforating keratoplasty could be postponed or avoided.

[Examination of subepithelial scar formation after photorefractive keratectomy with the ultrasound biomicroscope].

BACKGROUND: The subepithelial haze and the concomittant refractive regression are the most important complications of photorefractive keratectomies (PRK). The authors examined the subepithelial haze characteristics by ultrasound biomicroscopy (UBM). MATERIALS AND METHODS: Twenty previously photokeratectomized myopic eyes, which showed a certain level of subepithelial scarring have been examined (age: 34.22 +/- 3.57 years, 12 women, 8 men). The range of PRK treatments varied between -4.0 D and -12.0 D. The subepithelial haze graded according to Hanna et al, between 1.0 and 4.0. The UBM examinations were carried out between 1-3 months following PRK with the 50-80 MHz transducer of a Model 840 Zeiss-Humphrey Ultrasound Biomicroscope. RESULTS: The severity of subepithelial haze correlated with the previous photoablation depth. Up till haze graded 2.0 according to Hanna et al. UBM showed loss of Bowman membrane and a slight thinning of the central cornea. Between grade 2.0 and 3.0 the reflectivity of anterior stroma increased, above grade 3.0 a hyperreflective 1/3 anterior stroma with irregular borders were observed. CONCLUSION: Haze phenomen graded below 2.0 is not observable with UBM. Haze, graded more than 2.0 caused an increase of anterior stormal reflectivity. In more serious cases the 1/3 of the anterior stroma might be affected. The UBM-method with a 50-80 MHz probe is suitable to document and follow the serious haze phenomen after photoablations.

Development of somatostatin radioimmunoassay for the measurement of plasma and tissue contents of hormone.

A specific and sensitive radioimmunoassay was developed in our laboratory for measuring plasma and tissue somatostatin levels. The hormone content of arterial blood and skin samples of untreated and mustard oil (a specific agent causing neurogenic inflammation) treated animals was detected by this method. The somatostatin level of the inflamed tissue was significantly higher, but no difference was found between the plasma concentrations.

[Experience with excimer laser photorefractive keratectomy].

UNLABELLED: The correction of refractive errors based on superficial ablation of the cornea has a couple of years long history. Authors performed photorefractive laser surgery with the Aesculap Meditec MEL 60 ArF excimer laser in 221 eyes of 147 patients. The follow-up time varied between 2-24 months, average 8 months. They have treated the following groups: 1. group: 0-6.0 D; 2. group: -6.5-9.0 D; 3. group: eyes with refractive error above -9.5 D; 4. group: hypermetropic eyes; 5. group: eyes with astigmatism. RESULTS: In the 1. group the average -4.58 +/- 1.25 D refractive error decreased to -0.19 +/- 0.53 D in the follow-up time; 2. group: from -7.46 +/- 1.22 D to -0.45 +/- 0.83 D; 3. group: -13.76 +/- 5.46 D to -1.52 +/- 1.74; 4. group: +428 +/- 1.6 D to +0.91 +/- 2.41 D; 5. group; from -6.74 +/- 2.81 D sph -2.3 +/- 1.54 cyl to -0.51 +/- 1.3 D sph -0.54 +/- 1.06 D cyl. According to their experiences they conclude: the best results have been achieved in the first two groups and in eyes with astigmatism (1., 2., 5. groups). In severe myopia cases postoperative correction is usually needed, which is significantly smaller than the preoperative spherical correction. The method is able to change the refractive power of the cornea and provides an efficient alternative to the spectacles and contact lenses.

[Diagnosis of Verner-Morrison syndrome (VDHA) and its treatment with sandostatin an streptozocitin].

A case report is presented of a man with Verner-Morrison syndrome of extreme severity, caused by an unresectable pancreatic VIPoma. The pathological role of vasoactive intestinal polypeptide (VIP) is discussed in the pathogenesis of Watery Diarrhoea, Hypokalaemia, Achlorhydria (WDHA) syndrome. The authors describe the typical symptoms of the syndrome and provide a diagnostic and therapeutic strategy. Plasma level of VIP was determined by the authors' own VIP RIA method. Administration of a long acting somatostatin analogue, octreotide (Sandostatin, Sandoz) at a dose of 100 micrograms daily, decreased the plasma level of VIP from about 55 to 38 fmol/ml, which was associated with complete regression of the diarrhoea. Due to the 'escape phenomenon' the dose of Sandostatin was gradually increased and finally completed with streptozotocin (Zanosar, Upjohn) administration, which was repeated every 8 weeks. The combination of Sandostatin and streptozotocin resulted in complete regression of diarrhoea and substantial diminution of the tumour mass. The patient displayed a weight gain and returned to normal life.

Pancreatic trophism in experimental liver cirrhosis.

Pancreatic trophism and pancreatic enzyme composition, and plasma levels of cholecystokinin, insulin, glucagon, and glucose in liver cirrhosis induced by chronic thioacetamide administration (0.02% in the drinking water for 12 mo) were studied in rats. Advanced liver cirrhosis was evident in all thioacetamide-treated rats. The weight of the pancreas and its contents of DNA, protein, trypsinogen, chymotrypsinogen, proelastase, secretory trypsin inhibitor, and amylase were significantly increased as compared to the controls. The pancreatic secretory enzyme content changes showed a nonparallelism, characteristic of a cholecystokinin effect. Light and electron microscopy revealed a normal pancreatic architecture. Bioassayed plasma cholecystokinin levels in both fed and 24-h-fasted cirrhotic rats were significantly higher than in the corresponding controls. The plasma glucose, insulin, and glucagon levels demonstrated hypoglycemic tendencies with a glucagon predominance. These findings indicate that advanced liver cirrhosis in the rat is accompanied by pancreatic hypertrophy and hyperplasia, which might be attributed, at least in part, to elevated circulating cholecystokinin levels.

[Examination of the orbit using a non-ophthalmologic ultrasound equipment].

Ophthalmic examinations were performed for determining the suitability of seven different types of non-ophthalmic ultrasound equipments for ophthalmological purposes. According to the results, the instruments with probes of 5 MHz or higher frequency are appropriate for certain screening ophthalmological purposes in hospitals where ophthalmic ultrasound instrument is still missing. The possible indications are: approximate estimation of axial eye length for intraocular lens calculation, and visualization of gross intraocular and orbital diseases, such as retinal detachment, tumours, foreign bodies etc. In case of pathological result or in case of axial eye length different from the average value, the high resolution ophthalmological ultrasound examination is mandatory. The modern colour-coded Doppler ultrasound equipments were found suitable for the examination of the circulation of the eye and orbit.

[Determination of hepatitis C antibodies, using the Abbott and the Ortho anti-HCV EIA kits, in chronic liver diseases and patients under hemodialysis for chronic renal failure].

The diagnostic usefulness of anti HCV EIA test of Abbott and Ortho companies were compared. The anti HCV levels determined in the sera of 173 patients with chronic liver diseases and of 17 haemodialysed kidney patients. 109 of 190 (57%) sera were found to be negative and 81 (43%) positive determined by Abbott kit, while 127 (67%) were negative and 63 (30%) were positive by the Ortho kit. Positive results in patients with chronic liver disease were found in 66 patients by Abbott and 58 patients with Ortho Kit. Distribution of positive cases according to the diagnosis is as follows: 29 (57%) out of 51 chronic hepatitis, 17 (61%) out of 28 cirrhosis. Conflicting results were obtained in 20 cases of 190 (11%) when 17 sera were positive by Abbott and negative by ORTHO, and 3 sera were negative by Abbott, and positive by Ortho. The samples close to the cut off and with low positivity with conflicting results were checked again by the neutralization HCV EIA Abbott assay. We found the Abbott HCV EIA test more sensitive in our excellent for screening of large numbers of samples, we recommend to confirm the positive results by a neutralization type test.

[Somatostatin in the prevention of postoperative increase of pancreatic enzyme after pancreatic surgery].

The prophylactic effect of perioperative use of somatostatin on postoperative increase of pancreatic digestive enzymes was investigated in this double blind, randomized study. Thirty three patients undergoing pancreatic surgery because of chronic pancreatitis were divided randomly into two groups. Fifteen patients received somatostatin- (dose 125 micrograms/hour), 18 placebo-infusion, pre- and postoperatively for a total time of 48 hours. The level of serum amylase, lipase, gammaGT, calcium, creatinine and blood glucose was determined every 12 hours. In the placebo group the serum lipase and amylase increased significantly (p less than 0.001), while the calcium decreased. In the somatostatin treated patients only the lipase level increased significantly (p less than 0.01), while the amylase and calcium showed no significant changes compared to their initial values. The postoperative increase in serum enzyme levels is interpreted as being an indicator of pancreatic injury. These results suggest that the perioperative use of somatostatin has beneficial effect for the prevention of pancreatic enzymes increases, associated with pancreatic surgery or its complications in patients with chronic pancreatitis.

Changes in the ocular dimensions after trabeculectomy.

Ocular wall dimensions (axial eye length, anterior chamber depth, lens thickness, vitreous length, thickness and volume of the ocular wall) were determined by echobiometry on 10 eyes of 10 patients with glaucoma one day before and four days after trabeculectomy. The results showed significant differences: decreased axial eye length, and increased thickness and volume of the ocular wall were found after trabeculectomy. The observed changes in the ocular dimensions might be due to ocular hypotony and postoperative inflammation. Echographic follow-up of the changes in the ocular wall is a useful monitor to study alteration in eye dimensions after glaucoma surgery.
