[Ultrasound diagnosis of the lacrimal sack].

A- und B-scan echography is a useful method for the examination of certain parts of the lacrimal drainage system. In most cases it can substitute for dacryocystography and other expensive methods in the diagnosis of lacrimal sac diseases. Older methods of echography for examination of the lacrimal sac are direct contact, water bath, and rarely transocular contact methods. We have developed a new method, in which a rubber tube filled with fluid is placed between the transducer and the skin surface. Comparison of these methods demonstrates that this is the best method for ultrasound diagnostics of the lacrimal sac because it can be carried out as easily as the direct contact method, and the quality of the echograms is as good as in the water bath method. A special advantage is that causing an infection in the patient with the ultrasound probe can be avoided.

[Changes in the posterior eye segment in glaucoma patients].

In 30 eyes of 15 patients with chronic glaucoma (8 with open-angle glaucoma, 6 with chronic angle-closure glaucoma, 1 with pseudoexfoliative glaucoma), the intraocular pressure was measured and ultrasonic biometry and ultrasonographic determination of the thickness of the posterior coats of the eye were performed. Our results indicate that in patients with glaucoma, the thickness of the posterior coats of the eye is considerably influenced by intraocular pressure. If the axial length of the eye is the same, the thickness and volume of the posterior coats of the eye decrease with the increase in intraocular pressure.

[Ergometry-induced silent ischemia--clinical and angiographic features].

This study was planned to clarify the clinical and angiographic attributes of patients with exercise induced silent myocardial ischaemia. The study sample consisted of 102 patients who had ischaemic exercise electrograms (greater than = 1 mm ST segment depression) and significant angiographic stenosis (greater than = 50% diameter stenosis), in at least one of the major coronary arteries. In 61 patients (60%) painful ischaemia was observed during exercise (group A) while in 41 patients (40%) only SD depression (silent ischaemia) occurred (group B). The groups were similar with respect to age, sex, history of diabetes and all angiographic parameters including the Gensini coronary score. A history of prior myocardial infarctions was observed more frequently (p less than 0.01) in patients with silent ischaemia (group B). Prior myocardial infarction may be a factor causing silent myocardial ischaemia.

Painful and silent myocardial ischaemia during exercise testing.

The study was designed to clarify the clinical and angiographic attributes of patients with exercise induced silent myocardial ischaemia. The sample consisted of 102 patients who had ischaemic exercise electrograms (greater than or equal to 1 mm ST segment depression) and had a significant angiographic stenosis (greater than 50% diameter stenosis), in at least one of the major coronary arteries. In 61 patients (60%) painful ischaemia was observed during exercise (group A) while in 41 patients (40%) only ST depression (silent ischaemia) occurred (group B).

[Unilateral infiltration of the optic papilla in chronic lymphatic leukemia].

This paper reports the clinical history of a 72-year-old woman in whom unilateral papilledema was the only organic manifestation of chronic lymphatic leukemia. In the light of the laboratory parameters direct leukemic infiltration of the optic disk was diagnosed by fundus examination and fluorescein angiography. Combined cytostatic therapy bed to a considerable improvement in the patient's clinical condition.

Vitamin E content and lipid peroxidation of blood in some chronic inflammatory diseases.

Patients suffering from rheumatoid arthritis, spondylosis, coxarthrosis, ankylosing spondylitis, chronic active and chronic alcoholic hepatitis were studied. The plasma vitamin E content remained unchanged. The TBA-reactive plasma substances (malondialdehyde) content of plasma increased in all patients except those with ankylosing spondylitis. Catalase activity of plasma increased in patients of both sexes suffering from rheumatoid arthritis and spondylosis and coxarthrosis, but decreased in the two hepatitis groups. The glutathione-peroxidase activity of RBC (1:9 haemolysate) increased in female rheumatoid arthritis patients and decreased in those suffering from chronic alcoholic hepatitis. The results showed that chronic inflammatory processes affect the rate of lipid peroxidation and the activity of the biological antioxidant mechanism.

Comparative investigations on vitamin A level of plasma in some rheumatic diseases.

The vitamin A levels in the plasma of patients suffering from rheumatoid arthritis, ankylosing spondylitis, spondylosis, ankylosing hyperostosis (whether or not connected with diabetes) were investigated. Somatically healthy neurotic patients and patients suffering from diabetes without rheumatological problems served as controls. It was found that the retinol level of plasma decreased in patients of both sexes suffering from rheumatoid arthritis and clinically active ankylosing spondylitis, but increased in female patients suffering from ankylosing hyperostosis connected with diabetes, and also in the diabetes group. The retinyl-esters content of plasma decreased in the rheumatoid arthritis group and increased in female patients suffering from spondylosis and in the clinically inactive ankylosing spondylitis group. The total vitamin A content changed only in the rheumatoid arthritis group where a lower level was found compared to a somatically healthy control group.

Pentagastrin analogues containing alpha-aminooxy acids. VII. Catabolism in vivo and in vitro of a pentagastrin analogue containing N-terminal aminooxyacetyl residue.

In rat, BOC-NHO-1-14C-Ac-Trp-Met-Asp-Phe-NH2 (14C-I) is metabolised more slowly than BOC-1-14C-Gly-Trp-Met-Asp-Phe-NH2 (14C-II). Of the cytosol fractions of the rat organs the lung and pancreas exhibited a lower activity in the catabolism of BOC-NHOAc-Trp-Met-Asp-Phe-NH2 (I) than in that of BOC-Gly-Trp-Met-Asp-Phe-NH2 (II). In contrast, the cytosol fractions of the dog's lung, small intestine and pancreas hydrolysed I at a faster rate than they hydrolysed II.
