Functional activity of a thyroid nodule under the influence of stable iodide administration in a hyperthyroid patient.

In a thyrotoxic patient with nodular goiter treated with stable iodide and Carbimazole, a series of 131I scans was performed at different intervals for proposed 131I therapy. Originally, total uptake was suppressed and the node was evaluated as active. The same type of scan was seen repeatedly with uptake increasing for about 6 months following stable iodide withdrawal. Only 9 months later, the effect of 127I disappeared and uptake rose considerably with a reversal of the scan to the picture of a hypofunctioning node. The same distribution was found after therapeutic 131I administration. 6 months following treatment, without any thyrostatic drugs, the patient was euthyroid and on the scan most of tracer was found in the node. It is believed that this change is best explained by the different sensitivity of the nodule and the paranodular tissue to the effect of stable iodide. It is believed that the hyperthyroidism originated in the paranodular tissue highly sensitive on the 127I, while the nodule (presumably an adenoma) was less sensitive and showed uptake only when the paranodular tissue was depressed by 127I or, later, injured by the effect of therapeutic 131I.

[Voice - speech - life satisfaction. Existential analysis in view of a medical discipline].

The author begins with a characterization of one of the fundamental problems of phenomenological existential analysis: how can results which have been arrived at by means of special scientific methods be applied to an understanding of the world we live in? Referring to the work of L. Binswanger, M. Boss, A. Jores and others, he demonstrates the possibility of a new understanding of the meaning of some voice and speech disorders, particularly hysterical aphonia, spastic dysphonia, stuttering and so-called functional development symptoms (which are found in the results of organic examinations in connection with speech disorders). He bases an adequate psychotherapy mainly on the interpretation of dreaming, of resistance and of the meaning of pathological symptoms in general.

[Anaplastic carcinoma of the thyroid gland with chondroid, osteoid, and marocellular metaplasia of the stroma, associated with Hashimoto's lymphocytic thyroiditis].

Anaplastic, mostly spindle-cellular, carcinoma of the thyroid gland was diagnosed bioptically in a 65-year-old woman. Necroptically it was found that the carcinoma was associated with Hashimoto's lymphocytic thyroiditis. In the hitherto-differentiated carcinomatous parts the papillary component prevailed and the original lymphocytic stromal infiltration was in evidence here. In the anaplastic parts isolated macrocellular elements occurred and, in some places, also chondroid and osteoid structures, especially the structures of macrocellular bone tumor character. These structures were entirely missing in the distant metastases and were, therefore, evaluated as a stromal nontumorous metaplastic process.

The value of plasma TSH estimation and TRH stimulation test in the course of treatment of differentiated thyroid cancers.

Plasma TSH levels were measured on 114 occasions in 96 patients treated for differentiated thyroid cancer. Prior to thyroid surgery, plasma TSH levels were within the range of normal. Plasma TSH levels increased slightly following partial thyroid resection and definitely after total thyroid ablation. In patients where the removal of normal thyroid induced hormonogenesis in thyroid tumours, plasma TSH levels were dependent on the hormonal secretion of the tumour as shown by inverse relationship between TSH and both PBI and 131I uptake. The increase of radioiodide uptake following stimulation by exogenous bovine TSH was inversely related to the plasma thyrotropin levels. The suppressibility of enhanced thyrotropin levels was complete with individually adjusted doses of synthetic thyroid hormones. With the exception of patients on suppressive treatment, TRH administration induced increase in plasma TSH levels. The findings are discussed with regard to the role played by TSH in the induction of hormonogenesis in thyroid tumours. The practical values of TSH estimation and TRH stimulation seem to be low; the measurements of thyrotropin levels may be important for the estimation of the suppressive effect in the course of and following withdrawal of treatment with thyroid hormones.

[Thyroid neoplasms and HL-A system].

In 40 patients with carcinoma of the thyroid gland and 301 healthy test persons who are no relatives 23 HLA-antigens of the locus A and B are typed. There was not found a statistically significant difference concerning the frequency of HLA-antigens between the two groups. The cause of the negative results of these examinations with regard to the relation between tumour and HLA is discussed.

Differentiated thyroid cancer following radioiodide 131I therapy of hyperthyroidism--a case report.

Differentiated (papillary) thyroid cancer was detected 17 years following radioiodide 131I treatment for toxic multinodular goiter. 21 cases of thyroid cancers with previous 131 I therapy for hyperthyroidism were summarised. This combination is rare compared to the incidence of thyroid cancers following external irradiation. This may be due to higher absorbed dose to thyroid in 131I treatment.

Anaplastic transformation of medullary thyroid cancer.

The paper presents a detailed microscopic study of a case of medullary thyroid carcinoma with the loss of amyloid production and of argyrophilic cellular granules combined with the prevalence of giant multinucleated cells in a part of the primary tumor and namely in metastatic deposits. These changes are believed to give evidence of anaplastic dedifferentiation. Similar cases to that reported are reviewed with the conclusion that not only differentiated thyroid cancers but medullary thyroid cancers as well are capable of anaplastic transformation. However rare the medullary thyroid cancers are, they should be diagnosed and radically treated so as to prevent their fatal anaplastic transformation.

Lasting disappearance of autonomously functioning thyroid nodule (AFTN) following the TSH stimulation test.

Two AFTNs in one thyroid lobe were detected in a eumetabolic woman, with complete suppression of uptake in the paranodular tissue. After TSH stimulation, performed 2 months later, uptake was found in paranodular tissue only, both nodules being cold on the 131I scan. The same distribution of 131I was found on the next scan after a further 2 months without stimulation. Cystic degeneration was found by sonography and confirmed by aspiration biopsy. The destruction of one of the AFTNs was incomplete, as witnessed by a reappearance of function after 16 months. No differences in the thyroid handling of pertechnetate and iodide were detected. It is believed that the TSH stimulation might have hastened the otherwise spontaneous degenerative changes in the AFTNs.

[Collision carcinoma of the thyroid gland in Hashimoto's thyroiditis. Spinocellular carcinoma, arising from a cyst of the thyroglossal duct and a follicular carcinoma with an anaplastic turn].

A collision carcinoma of the thyroid was revealed in a surgically removed thyroid affected with Hashimoto's thyroiditis. A follicular carcinoma composed of rather uniform and spherical cells. The anaplastic carcinoma appeared to collide with structures of a noncornifying squamous cell carcinoma supposed to originate in the isthmic area from a ductus thyreoglossus cyst.

Comparison of diagnostic accuracy of different peripheric parameters of thyroid function.

Diagnostic value of a scope of peripheric parameters of thyroid function was assessed in an unselected group of untreated patients with suspected thyroid disorder, further in untraeted selected patients (without cardiac involvement) and in treated patients. Comparison of relative values of individual tests was performed, based on relation to plasma thyroid hormone level represented by PBI. It was found that: 1. The diagnostic value of heart rate, plasma cholesterol level, B. M. R. and Hegglin's sign (T-2s interval) is of a very limited degree. 2. The diagnostic accuracy of AJT, Q-Kd interval and PEP was found to be of considerable interest even in unselected patients. Values of IRVD and D indices found for these tests are comparable and allow the immediate estimation of thyroid function in bedside diagnosis. 3. The diagnostic value of PEP could be enhanced by exclusion of patients with suspected or proved cardiac disorder or myocardial failure; this may be useful for physiologic studies. 4. While heart rate is profoundly and inconsistently influenced by beta-blockade, AJT is influenced to a minimal degree only and Q-Kd and PEP are uniformly shifted to higher values, allowing thus diagnostic evaluation during this form of treatment also. 5. As Q-Kd is considerably age-and height-dependent, AJT and PEP are believed to be the most suitable test for immediated clinical diagnosis.
