Idiopathic arterial calcification of infancy: a case with prolonged survival.

We describe a patient with idiopathic arterial calcification of infancy and the following unusual features of the disease: (1) prolonged survival until age 11 years, (2) discordance between the extent of vascular calcification and clinical manifestations of arterial luminal occlusion, (3) a large area of myocardial calcification, (4) symptoms of cerebral-vascular insufficiency, and (5) spontaneous resolution of hypertension.

Aphthous ulcers in ischemic gastroenterocolitis: a case report.

Extensive occlusive mesenteric vascular disease is described in a 41-year-old man, resulting in aphthous ulcers of the stomach, small bowel, and colon. Ischemic disease was confirmed angiographically and ultimately by surgical intervention. No evidence of inflammatory bowel disease was detected. Occlusive vascular disease must be considered in the differential diagnosis of aphthous ulcers of the gastrointestinal tract. The risks of angiography may be warranted in the presence of atypical and unresponsive inflammatory bowel disease, especially when it coexists with vascular disease elsewhere.

Isolated gastric varices due to spontaneous splenic vein thrombosis.

Isolated bleeding gastric varices due to a spontaneous idiopathic thrombosis of the splenic vein was found to be the cause of obscure, painless recurrent bleeding in our patient. The diagnosis of spontaneous splenic vein thrombosis should be considered in the presence of gastrointestinal bleeding with unexplained splenomegaly. Prominent gastric folds may be the only barium contrast or endoscopic finding often lacking esophageal varices. Arteriography is essential in demonstrating an obstructed splenic vein. Splenectomy is the treatment of choice of "left-sided" portal hypertension.

Angiographic findings in liver amyloidosis.

Angiographic findings in a case of liver amyloidosis are reported. Focal amyloid deposits in the gastrointestinal tract, with irregularity and narrowing of intrahepatic vessels, may mimic an intrahepatic mass. A correct diagnosis can be obtained if the patient's history and angiographic findings are evaluated carefully.

Isolated intrathoracic injury with air bag use.

The restrained (air bag and seatbelt) driver of a vehicle involved in a high-speed motor-vehicle accident sustained a tear of the thoracic aorta with no signs of external injury. Air bag deployment may mask significant internal injury, and a high index of suspicion is warranted in such situations.
