[Adjuvant chemotherapy (MVP-CAB; methotrexate, vincristine, cisplatinum, cyclophosphamide, adriamycin, and bleomycin) for bladder cancer].

Twenty-three patients with bladder cancer (TCC; 18 patients, SCC; 5 patients) were treated with adjuvant chemotherapy (day 1: methotrexate 20 mg/m2, vincristine 0.6 mg/m2, cyclophosphamide 500 mg/m2, adriamycin 20 mg/m2, bleomycin 30 mg, day 2: cisplatinum 50 mg/m2; MVP-CAB). A total of 3 cycles of MVP-CAB were given as preoperative or postoperative therapy. The following results were obtained. Group 1 (purpose to preserve the bladder, preoperative MVP-CAB): Four of 7 patients achieved a partial response. It was possible to perform bladder preservation surgery in 3 of these 4 patients. All 3 patients had pedunculated, solitary tumors, and there was no carcinoma in situ. Group 2 (purpose to improve the prognosis; preoperative MVP-CAB): Hydronephrosis did not resolve in the 4 patients with this complication. They received total cystectomy, and 2 patients died of cancer 23 months later. Group 3 (purpose to improve the prognosis; postoperative MVP-CAB): Ten of 12 patients (total cystectomy; 10 patients, partial cystectomy; 2 patients) survived disease-free for an average of 17 months (5-44 months), 1 patient developed recurrence 12 months later, and 1 patient died of cancer 6 months later. The 1-year survival rate in Group 3 was 86% for TCC, 100% for SCC, 80% for grade 3 TCC, and 89% for pT2 or more advanced cancer.

Effect of dietary sorbitol on the performance of broilers.

1. The effect of dietary sorbitol on the performance of broilers was investigated. Male and female broilers were given a control diet or a sorbitol diet (100 g/kg diet) ad libitum from 29 to 57 d of age. 2. Body weight gain, food intake, food efficiency and the ME values of the diets were not significantly different. Absolute and relative weights of abdominal fat were significantly reduced in birds given the sorbitol diet. 3. Compared to the control diet, the diet containing sorbitol lowered the serum glucose, total cholesterol and very low density lipoprotein concentrations.

Effect of dietary sorbose on lipid metabolism in male and female broilers.

Male and female broilers were given diets (6 males and 6 females per diet) containing varying percentages of sorbose (0, 3, 6, and 9%) and fed for ad libitum access from 28 to 56 days of age. Body weight gain and feed intake were decreased with increasing dietary sorbose, particularly in male birds fed diets containing 9% sorbose, although feed efficiency and N retention rate were not influenced by dietary treatments. Absolute and relative abdominal fat weights were higher in females than in males and decreased with the increasing levels of dietary sorbose in both sexes. Fat content in the pectoral muscle also decreased as dietary sorbose increased. Dietary sorbose did not have significant effects on serum glucose, triglyceride, total cholesterol, low density lipoprotein, very low density lipoprotein, and chylomicron levels in either male or female birds. The ME values of diets decreased as dietary sorbose increased. Palmitic acid content of abdominal fat was significantly lower in birds fed the 9% sorbose diet than in birds fed the control diet. The reverse was true for linoleic acid content. It was concluded that dietary sorbose can be used as a potential regulator of lipid deposition in broilers.

In vitro disassembly of the nuclear lamina and M phase-specific phosphorylation of lamins by cdc2 kinase.

The nuclear lamina is an intermediate filament-type network underlying the inner nuclear membrane. Phosphorylation of lamin proteins is believed to cause lamina disassembly during meiotic and mitotic M phase, but the M phase-specific lamin kinase has not been identified. Here we show that the cdc2 kinase, a major element implicated in controlling the eukaryotic cell cycle, phosphorylates chicken B-type lamins in vitro on sites that are specifically phosphorylated during M phase in vivo. Concomitantly, cdc2 kinase is capable of inducing lamina depolymerization upon incubation with isolated nuclei. One of the target sites of cdc2 kinase is identified as a motif (SPTR) conserved in the N-terminal domain of all lamin proteins. These results lead us to propose that mitotic disassembly of the nuclear lamina results from direct phosphorylation of lamins by cdc2 kinase.

Identification of major nucleolar proteins as candidate mitotic substrates of cdc2 kinase.

Following the identification of the cdc2 kinase as a major element controlling entry of cells into mitosis, it is important to define the physiological target range of this enzyme. Here, we demonstrate that two major nucleolar proteins, nucleolin and NO38, are highly phosphorylated during mitosis. Importantly, the two nucleolar proteins are also phosphorylated by highly purified starfish cdc2 kinase in vitro, on sites that correspond to those observed specifically during mitosis in vivo. A repeated motif (TPXKK) is identified as the likely mitotic phosphoacceptor site in nucleolin, in that a synthetic peptide mimicking this site functions as both a substrate and a competitive inhibitor of cdc2 kinase. These results identify two novel candidate substrates for cdc2 kinase, and they implicate protein phosphorylation in controlling mitotic changes in nucleolar structure and activity.

Regulation of lipid metabolism by dietary sorbose in laying hens.

Single Comb White Leghorn laying hens received ad libitum diets (10 birds per diet) containing varying concentrations of sorbose (0, 100, and 200 g/kg diet) for 4 wk. Body weight gain and feed intake decreased with increasing dietary sorbose. Serum triglyceride, cholesterol, low density lipoprotein, very low density lipoprotein, and chylomicron levels were significantly reduced, in dose-dependent fashions, as dietary sorbose increased, whereas serum glucose level remained unchanged by the dietary regimens. Absolute and relative weights of abdominal fat were also decreased by increasing dietary sorbose. Liver color improved; it became less white and less yellow with the supplement of dietary sorbose. Hen-day egg production rate was not affected with dietary sorbose, and hence, it has been shown that dietary sorbose can be used as a potential regulator of lipid metabolism in the laying hen.

[A case of mediastinal schwannoma originating from the thoracic vagal nerve complicated by early stomach cancer].

Most of primary tumors of the nerves arising in the mediastinum originate from the sympathetic or intercostal nerves. Schwannoma developing from the vagal nerve is very rare. The patient was a 73-year-old male referred to us with abnormal chest X-rays and stomach cancer. One-staged operation was performed by thoracic and upper abdominal median incision. The tumor was removed, which measured 6 X 5 X 4 cm and involved the origin of the left recurrent nerve, by neurectomy. To our knowledge, this was the 18th reported case of schwannoma of the intrathoracic vagal nerve in Japan. A similar schwannoma was observed also in the celiac plexus, and this case was also a rare instance of multiple schwannoma.

[The surgical treatment of ruptured thoracic aortic aneurysm with use of permanent bypass].

Two surgical cases with ruptured thoracic aortic aneurysm were reviewed. Case 1 was a 74-year-old female, and Case 2 was a 77-year-old female. In both cases, chest X-ray examination revealed a mass like shadow in the left superior mediastinum, and aortography demonstrated a ruptured saccular aneurysm distal to the left subclavian artery. Midsternotomy and an incision over the third left intercostal space were made to approach the aneurysm. Since inflammatory adhesion was marked in the perianeurysmal area, a permanent bypass was placed between the ascending and descending aorta followed by exclusion of the aneurysm. One of the two cases was alive, but the other case was died due to bleeding from the area of permanent aortic clamp.

[Correlation between the function and structure in papain-induced emphysema in dogs].

To examine the relationship between morphologic changes and the alteration of pulmonary function, papain solution was administered to 11 dogs by inhalation and to 5 dogs by a single intratracheal injection. Pulmonary function tests, including determination of lung volume subdivision, diffusion capacity (DLCO), pressure-volume relationship of the lung and arterial blood gas were performed before and after papain treatment. The dogs were then killed and the lungs processed for pathologic studies. The severity of emphysema was graded as percent of emphysema according to Dunill's method and we compared this to the antemortem pulmonary function data. Exponential constant K was determined by fitting lung pressure-volume data to the equation V = Vo (1-e-KP). After papain administration, DLCO decreased, K of the pressure-volume curve increased, and arterial blood oxygen pressure reduced significantly. The changes of residual volume, the ratio of residual volume to total lung capacity, DLCO per unit lung volume, and the values of the exponential functions of pressure-volume data were correlated with the severity of emphysema with statistical significance, but none of them showed excellent order correlation. Multiple regression equation using 2 to 4 of the parameters indicated that the severity of emphysema can be predicted more accurately. We conclude that recognition of the severity of emphysema requires a multivariate approach including the determination of lung volume, DLCO, and pressure volume relationship of the lung, because the relationship between the severity of emphysema and the values obtained with these measurements are nonlinear.
