Structural elucidation of natural 2-hydroxy di- and tricarboxylic acids and esters, phenylpropanoid esters and a flavonoid from Autonoë madeirensis using gas chromatographic/electron ionization, electrospray ionization and tandem mass spectrometric techniques.

The chemical composition of Autonoë madeirensis bulbs was characterized as part of a systematic phytochemical study of this species. The compounds reported were mainly identified on the basis of gas chromatography/electron ionization, electrospray ionization and tandem mass spectrometry. The structures of the pure compounds were also characterized by means of other physical and spectroscopic data (m.p., IR, UV, NMR). The compounds identified were 2-hydroxy di- and tricarboxylic acids and esters (malic acid, citric acid and their methyl and ethyl esters), cis- and trans-hydroxycinnamic esters (methyl and ethyl p-coumarate and methyl ferulate) and a new flavone diglucoside, 7-O-[beta-glucosyl-(1-->2)-O-beta-glucosyl]apigenin, the interglucosidic linkage (1-->2) of which is, to the best of our knowledge, reported for the first time in a diglucoside of apigenin. The results may contribute to the chemotaxonomy of the Autonoë genus and lead to a rapid tool for the systematic characterization of these compounds in plant extracts.

Effects of horminone on liver mixed function mono-oxygenases and glutathione enzyme activities of Wistar rat.

The present study reports on the effects of horminone on serum alanine aminotransferase (ALT) and aspartate aminotransferase (AST) levels, on hepatic cytochrome P450 (P450) and cytochrome b5 (cyt b5) contents and on the activities of NADPH-cytochrome P450 reductase (NR), mixed function mono-oxygenases (MFO), glutathione-S-transferase (GST) and glutathione reductase (GR) of Wistar male rat. Horminone is a diterpenoid quinone (7,12-dihydroxyabiet-8,12-diene-11,14-dione) present in several species of the Labiatae family and used as medicinal plants in folk medicine. In this study, horminone was administered by the intraperitoneal route (i.p.) at a concentration of 1 or 10 mg/kg to each group of six mice, using water as a vehicle. On the one hand, results showed that horminone increased serum ALT and AST levels and cyt b5 content and induced the activities of ethylmorphine N-demethylase (EMD). On the other hand, horminone decreased P450 content and inhibited the activities of 7-ethoxyresorufin O-deethylase (ERD), 7-ethoxycoumarin O-deethylase (ECD), aniline 4-hydroxylase (AH) and NR. Based on these results, the possibility of toxic effects occurring after administration of plant extracts containing horminone must be considered.

A rearranged abietane diterpenoid from Plectranthus hereroensis.

A new abietane diterpenoid has been isolated from the aerial parts of Plectranthus hereroensis, together with two known diterpenes. The structure of the new substance, 3 beta-acetoxy-6 beta,7 alpha,12-trihydroxy-17(15-->16);18(4-->3)-bisabeo-abieta-4(1 9),8,12,16-tetraene- 11,14-dione, was established by spectroscopic means and by comparison with closely related compounds. This diterpene showed moderate antibacterial activity.

[Traumatic aortic valve insufficiency].

The traumatic aortic valvular insufficiency (TAVI), through less frequent after a non-penetrating thoracic traumatism, is a serious entity with a very reserved prognosis. So it must be suspected in every patients with signs or symptoms of de novo heart failure post-traumatism. The transthoracic echocardiography and eventually transesophageal echocardiography have a fundamental role in the confirmation of the diagnosis. The clinical picture of traumatic aortic regurgitation is quickly evolutionary and the non efficacy of medical therapy has placed the valvular substitution surgery as the best succeeded treatment. With the advent of the aortic valve repairing surgery some TAVI cases has been submitted to this procedure. Nevertheless, the development of residual aortic regurgitation in these situations, usually requiring later valvular replacement surgery, make the aortic valvuloplasty a controversial surgical technique. The AA describe a recent clinical case of aortic regurgitation after a non-penetrant thoracic traumatism, discussing the aspects connected with physiopathology, diagnosis and therapy. The singularity of this case was based on the fact that the initial clinical diagnosis had been prejudiced by the context of a polytraumatism and there had been a time free of symptoms between the traumatism and the beginning of the symptomatology of left ventricular failure. Even though the identification of the problem allowed an intensive treatment of this serious situation that ended with the replacement of the aortic valve by mechanical aortic prosthesis, with the patient's total recovery.

Structure and antimicrobial activity of diterpenes from the roots of Plectranthus hereroensis.

Two abietane-type diterpenoids have been isolated from the roots of Plectranthus hereroensis (Labiatae), one being the already known horminone [1] and the other a new substance, 7 alpha,12-dihydroxy-17(15-->16)-abeo-abieta-8,12,16-triene-11 ,14-dione [2], whose structure was established by spectroscopic means. Compounds 1 and 2 showed antimicrobial activity against Staphylococcus aureus, Vibrio cholerae, Candida albicans, and Pseudomonas aeruginosa.

Open mitral commissurotomy: a better procedure?

Percutaneous balloon commissurotomy has emerged as an alternative method for the treatment of mitral stenosis, and several comparative studied have recently claimed superior results as compared with those obtained by surgical commissurotomy. However, valve areas after balloon commissurotomy, calculated by Doppler echocardiography have, in the best cases, averaged 2.0-2.2 cm2 and in some cases much less. Areas of 1.5 cm2 have often been considered as good results. In our opinion, open commissurotomy on pliable valves is capable of achieving areas closer to normal. Furthermore, it remains to be determined whether residual transvalvular gradients resulting from an incomplete balloon commissurotomy will cause increased fibrosis, accelerate restenosis and render repeat commissurotomy impossible. To prove this assumption, we have prospectively studied 100 consecutive patients who underwent open mitral commissurotomy, but whose stenotic mitral valves had been considered pliable enough for balloon commissurotomy. They constitute 41.7% of the 240 open commissurotomies performed during the 3-year study period (1988-1991) which, in turn, represent 75.7% of the 317 stenotic mitral valves submitted to surgery. The mean age of the patients was 47.6 +/- 11.5 years (range 19 to 69 years), the majority (87 patients) were female and 54 were in sinus rhythm. Associated procedures included aorto-coronary bypass in two, aortic valve surgery in nine and tricuspid valve annuloplasty in three patients. Bilateral incision of the mitral valve commissures, was performed in all patients. Additionally, division of fused commissural chordae tendineae and/or papillary muscles was performed in 56 cases and excision of thickened basal chordae of the posterior leaflet in 14 patients.(ABSTRACT TRUNCATED AT 250 WORDS)

[Health promotion and education: a diagnosis of sanitation conditions using participatory research and community education (São João dos Queiróz - Quixadá/Ceará, Brazil)].

This study was conducted in a rural community, São João dos Queiróz, a township in the county of Quixadá, Ceará, Brazil, using a combination of participatory research and community education in compliance with the health promotion reference and principles of the 1986 Ottawa Charter. The project was joined by representatives of several local government institutions and organizations from the grassroots community movement. The theme generating the research, as defined by an assembly meeting of the community association, was a diagnosis of sanitation conditions in the community. The starting point was the assessment of local conditions. Results showed adverse local conditions in sanitation, literacy, income, and employment. Suggestions for solving the problems were organized so as to be included in the planning agenda for local health policies. Evaluation was procedural and enriched with daily research activities. The problem-solving pedagogical approach developed during the educational process contributed to a critical reconstruction, appropriation, and sharing of the resulting knowledge.
