[Long-term results of radical operations on the ventricular septum with organic pulmonary hypertension].

From a total group of 189 children with a ventricular septal defect operated in 1978-1985 at the Cardiosurgical and Transplantation Department in Brno the authors followed up 60 patients with pulmonary hypertension. In addition to catheterization and echocardiographic examination, they made in 28 patients (46%) a preoperative pulmonary biopsy. In the majority of children before and after operation by catheterization the pulmonary tension Rp/Rs was measured and in some also pulmonary vascular resistance. During the echocardiographic examination attention was focused on the possibility to as the pulmonary tension by pulsed Doppler and continuous technique, on the detection of left-to-right shunt, pulmonary or tricuspid insufficiency. Of 60 patients 5 children (8.3%) died soon after operation, one patient died six years after operation. Long-term postoperative follow up made is possible to define more accurate indication criteria for operation as well as for pulmonary biopsy.

[Ebstein's anomaly of the tricuspid valve with the WPW syndrome].

Case-history of a four-year-old boy with Ebstein's anomaly of the tricuspid valve with WPW syndrome. Severe tricuspid insufficiency and repeatedly occurring tachydysrhythmias soon led to cardiac decompensation and death before the intended cardiac operation. Non-invasive clinical, ECG, X-ray and ECHO examinations correlated with catheterization, electrophysiological and pathological findings. In addition to the dislocated, malformed tricuspid valve the septal cusp of which was fused with the ventricular septum the authors found another additional atrioventricular muscular connection between the lower part of the septum of the right atrium and right ventricle.

Hematological and biochemical studies in calves living over 100 days with the polymethylmethacrylate total artificial heart TNS Brno II.

A polymethylmethacrylate total artificial heart (kinetic components made of polyetherurethane) of TNS Brno II type was implanted into seven calves (2-5 months of age) surviving for the average of 152.4 +/- 19.1 days after the implantation. During the entire post-operative period the animals received oral warfarin-sodium, acetylsalicylic acid, dipyridamole and alpha-tocopherol. Blood was taken for biochemical and hematological examinations twice a week from the jugular vein. During the experiments there were decreases in the number of red blood cells, hematocrit and hemoglobin levels. Plasma free hemoglobin and serum enzymes (alkaline phosphatase, AST, ALT, LDH) increased. Coagulation tests were abnormal because anticoagulation therapy was used. There were minimal changes in the number of white blood cells and platelets, fibrinogen, blood pH, blood glucose, serum electrolytes, bilirubin (total and direct), creatinine, blood urea, and lactate. Possible reasons for observed changes include the gradual rise in the central venous pressure and damaged function of the liver parenchyma. Other factors playing a possible role in inducing changes in laboratory findings are also discussed.

Surgical treatment of atrial septal defects.

An analysis is presented of the results attained in 207 patients operated for isolated atrial septal defect at Centre of Cardiac Surgery in Brno from Jan. 1, 1978 to March 31, 1982. The paper discusses classification of the defects, the surgical procedure, results and complications, as well as indications and counterindications for surgery. Early mortality was zero. Emphasis is placed on the need to operate as soon as possible after detection of a haemodynamically significant shunt, i.e. before developing pulmonary hypertension.

Use of physiological and genetic algal tests for the hygienic evaluation of pollutants in waters. Part I. 226Ra studies.

After exposure for several days of intensively growing algal cultures to a radium concentration of 10(-6) g.1(-1) the physiological and genetic responses of Chlorella kessleri and Scenedesmus obliquus cells could be clearly demonstrated. Of physiological characteristics, we recorded the frequency of cells with a reduced number of autospores, the length of the lag phase and cell survival. Of genetic responses, the frequencies of delayed lethal and sublethal effects, the frequencies of minute colonies, colonies with big cells and scabby colonies, and the frequencies of pigmentation changes were followed. The above concentration produced no toxic effect. No simple and clear-cut correlation was found between the recorded responses and the amount of accumulated radium in algal cells. For an interpretation of the mutagenic effect we had to assume a homogeneous radioactive irradiation of the mixed cell suspension. We had also to take into account the participation of repair systems of algae with regard to their survival or manifestation of mutagenic changes in individuals in which mutagenesis had been induced.
