Effect of the immunomodulator AS101 on chemotherapy-induced multilineage myelosuppression, thrombocytopenia, and anemia in mice.

The immunomodulator AS101 has previously been found to induce mouse and human hematopoietic cells to secrete cytokines such as interleukin-1 alpha (IL-1 alpha), IL-2, tumor necrosis factor-alpha (TNF-alpha), and gamma interferon (IFN-gamma). The compound was shown to protect mice from lethal and sublethal effects of chemotherapy and irradiation. AS101 prevented the decrease in the number of bone marrow (BM) and spleen myeloid progenitor cells, and increased the survival of lethally treated mice. In this study, we show a dose-dependent response of AS101 in the induction of high secretion levels of IL-6, IL-3, granulocyte-macrophage colony-stimulating factor (GM-CSF), and stem cell factor (SCF). Since these growth factors are known to induce the proliferation and differentiation of multilineage progenitors, including megakaryocytic and erythroid progenitors, we designed this study to evaluate the role of AS101 in attenuating thrombocytopenia, anemia, and multilineage myelosuppression associated with chemotherapy. We demonstrate that pretreatment of mice with AS101 24 hours before intraperitoneal injection of 250 mg/kg cyclophosphamide (CYP) or intravenous injection of 150 mg/kg 5-fluorouracil (5-FU) significantly increased the number of circulating white blood cells (WBC) and platelets. The numbers of both neutrophils and lymphocytes were significantly increased in AS101-treated mice subjected to chemotherapy. In addition, AS101 attenuated erythropenia caused by 5-FU. It could also increase megakaryocyte and erythroid progenitor cells (CFU-MK and CFU-E) in the BM of treated mice severely affected by chemotherapy. We demonstrate that the protective effect of AS101 could be abrogated by treatment with anti-IL-1R or anti-SCF antibodies. We suggest that the endogenous production of cytokines such as IL-1, IL-6, IL-3, SCF, and GM-CSF in mice treated with AS101 offers protection to circulating blood elements and ameliorates the reconstitution of megakaryocytic and erythroid progenitors. The simultaneous protection by AS101 of multilineage cell compartments is probably due to stimulation by AS101 of a selective subpopulation of primitive stem cells resistant to chemotherapy. On the basis of these studies, phase II clinical trials with patients treated with chemotherapy in combination with AS101 have been initiated.

Use of flumazenil for intraoperative arousal during spine fusion.

The intraoperative arousal test is used to detect potential damage to the spinal cord during spine fusion. We determined whether a continuous midazolam infusion, the effects of which are antagonized by flumazenil when the arousal test is needed, is associated with good control of the timing of arousal and with no adverse reactions. In a randomized study of 20 patients (10 in each group), we compared arousal with flumazenil during midazolam-narcotic anesthesia with an arousal test done during halothane-nitrous oxide anesthesia, both under conditions of controlled hypotension. Motor responses to verbal command were obtained 39.1 +/- 36.0 s (mean +/- SD) after the injection of flumazenil in eight patients. Two patients form this group awoke spontaneously 6 and 8 min after terminating the administration of nitrous oxide and midazolam but remained sedated up to the time of the test itself. In the halothane group, arousal occurred 26 +/- 8.3 min after halothane administration was terminated and 10.1 +/- 3.3 min after nitrous oxide was stopped. Two of the patients in this group moved, owing to early arousal 5 and 8 min before the scheduled time and needed reinstatement of anesthesia. In four patients in the halothane group, we had to wait 3-11 min after the scheduled time before satisfactory arousal was achieved. One of the patients in the halothane group showed a motor deficit of the lower limbs during the wake-up test that disappeared after decreasing the spine correction. Scintigraphic presentation of hip joint synovial chondromatosis.

A case of hip joint synovial chondromatosis with an unusual scintigraphic pattern is described. This pattern was suggestive of a hip joint destructive reactive articular process or late manifestations of avascular necrosis of the femoral head. Concurrent radiographs were normal, as were laboratory investigations. Follow-up radiographs six months later showed radiolucencies and erosive bone changes in the diseased joint. Surgical and histopathological findings revealed well developed hip synovial chondromatosis (HSC) with thickened synovium and large, loose, cartilaginous bodies occupying and widening the tightened joint space, with destructive secondary juxta articular pressure and bone erosions. This and other scintigraphic patterns in HSC, and the differential diagnosis of the findings in patients with painful hip presentations are discussed.

Chondroma

Synovial chondromatosis of the hip joint.

Synovial chondromatosis of the hip joint is uncommon entity. Two cases are presented, and the spectrum of the disease is discussed. Partial synovectomy with loose body removal is recommended as the treatment of choice. Joint replacement is performed only in patients with severe secondary osteoarthritis, and used only in selected cases.

Total femoral replacement.

Between 1973 and 1983, 19 patients with sarcoma of the femur were treated by adjuvant chemotherapy, excision of the entire femur, and replacement by a total femoral prosthesis. Five patients had excellent and nine had good functional results. Twelve patients died an average of 23 months after the procedure and seven are at present disease free. This limb-saving procedure permits rapid rehabilitation, prevents severe psychological problems, and improves the quality of life.

Percutaneous discectomy of the lumbar spine.

Ten patients with clinical and radiological evidence of herniated discs at lower lumbar levels were treated with partial discectomy by a lateral percutaneous approach. Eight patients had complete relief from radicular pain and were discharged within four days. They returned to normal daily activity within one month.

Osteoblastoma of the sternum.

Osteoblastoma has become a well defined bone tumor since its first description by Jaffe and Mayer in 1932. This tumor was considered to occur in almost any bone of the skeleton. Only one previous case of sternal osteoblastoma was described in the literature. Herein we report, as far as we know, the second reported case of osteoblastoma in the sternum, as an isolated tumor.

Fracture in progressive ossifying fibrodysplasia. A case report.

Progressive ossifying fibrodysplasia is a rare genetic disorder of connective tissue. A 6-year-old boy sustained a fracture of the humerus and afterwards of the femur. The former fracture was treated closed and the latter with internal fixation. Both fractures healed with severe restriction of joint motion.

Musculocutaneous nerve injury after coracoid process transfer for clavicle instability. Report of three cases.

During the last 10 years, we have treated 54 cases of fracture or dislocation of the lateral clavicle by transposing the coracoid process onto the clavicle. Recently, a postoperative musculocutaneous nerve injury was observed in 3 cases, all of which recovered spontaneously within 5 months. Our technique seems to entail a certain risk of causing stretch injury to the musculocutaneous nerve. Apart from resting the affected arm for a month or two, the condition does not require specific treatment.

Acromioclavicular Joint

The electrospinal treatment of idiopathic scoliosis in the adolescent.

The transcutaneous electrospinal treatment adds another approach to the conservative treatment of small and moderate idiopathic scoliotic curves. The method presented here is readily accepted by both the adolescent and the parents. It is inexpensive, and allows flexibility as to the location and number of electrodes to be used and the curve treated. The data presented here, collected over a five-year period of clinical monitoring and recording, indicate that results are favorable, therefore, the method is acceptable.

Stimulation of bone formation by electrical current on spinal fusion.

An experimental double-blind study to evaluate the effectiveness of a constant current stimulator was performed on 30 young pigs that underwent a spinal fusion at the level of L5-L6. The group of animals with an active bone-growth stimulator showed a statistically significant increase of osteoblastic activity with bone formation.

Hip arthroplasty in quiescent mycobacterial infection of hip.

Three patients underwent hip arthroplasty--one total hip replacement and two, Thompson's hemiarthroplasty. The diagnosis was degenerative joint disease and subcapital fracture of the femur, respectively. Histologic and bacteriologic examination revealed mycobacterial infection. None of the patients had a past history of mycobacterial infection. In one instance, there was a reactivation of the infection eight months following surgery, which was treated successfully by antituberculosis drugs.
