Personal development: a lifelong journey.

This article explores critical aspects of career and personal development, in particular the importance of a lifelong commitment to reflective practice. It focuses on the key questions: Where have I been? Where am I now? and Where do I want to be? Specific approaches discussed include journal writing and collegial small group work.

Investigation of a listeriosis epizootic in sheep in New York state.

OBJECTIVE: To investigate potential sources of an epizootic of listerial encephalitis, using molecular diagnostic and typing methods. SAMPLE POPULATION: A flock of about 655 sheep. PROCEDURE: An epizootiologic investigation was performed. Clinical, feed, and environmental samples were tested for Listeria monocytogenes, using polymerase chain reaction and culture methods; recovered isolates were "fingerprinted," using an automated ribotyping system. RESULTS: Listeria monocytogenes was recovered from brain specimens of 7 sheep with clinical signs of listerial encephalitis. All clinical isolates had fingerprints identical to those of isolates from farm equipment used to transport silage. Corn silage, which was not fed to the sheep, also contained L monocytogenes of the same pattern type as defined by ribotyping. Listeria monocytogenes was not isolated from the stored haylage designated for feeding the sheep (the cut-off point for isolation being < 10(2) colony-forming units/g). CONCLUSIONS: Corn silage was implicated as the source of a listeriosis epizootic. It appears to have cross-contaminated the haylage destined for the sheep during handling with a front-end loader. Suspension of silage feeding coincided with cessation of listeriosis cases. CLINICAL RELEVANCE: Use of advanced molecular techniques can help to identify the sources and restrict the scope of an epizootic. In epizootics, a single L monocytogenes strain can lead to infection of multiple animals, with rapid progression of the disease.

[Sigmund Freud and Hans Blüher in up to now unpublished letters].

In the years 1912 and 1913 there occured an epistolary encounter between the founder-father of psychoanalysis and the young Hans Bluher who had been active in the Wandervogel movement, a German youth movement with strong traditionalist and nationalist leanings. Their correspondence centered around the evaluation of male homosexuality, a point on which Freud and Bluher were not in entire agreement. In his introduction Neubauer outlines the intellectual nub of this debate and sketches Bluher's later career and his gradual transformation into a biologistically motivated anti-Semite completely and utterly disowned by Freud.

The rapidly changing workplace demands that people who manage their careers explore the dangers and the opportunities for themselves personally. This includes accepting the reality of the current environment that indicates 50 percent of the eight jobs an average American will hold will terminate involuntarily. Grieving about previous life and job expectations may be necessary to take advantage of the extensive opportunities available. Developing, implementing, and evaluating a life and career plan are essential to thriving in this chaos.

The learning network. Leadership development for the next millennium.

The complex process of creating a learning organization demands new methods for development and learning. Individuals need to find new ways of working together by increasing self-knowledge and changing their behavior. Learning networks, a simple but profound approach to learning, offer participants an opportunity to exchange experiences in a safe and supportive environment. Participants review their most important and difficult work and develop their interpersonal skills and self-awareness to new heights. In the spirit of empowerment and creating a learning culture, colleagues learn to listen and observe as well as ask helpful questions and offer alternative perspectives and practical ideas. They also give and receive feedback. The author explores the concepts, experiences, structures, and results of learning networks that allow readers the opportunity to contemplate a new form of development for themselves and their staffs.

Contribution of postprandial amino acid levels to stimulation of insulin, glucagon, and pancreatic polypeptide in humans.

The present study was designed to examine the contribution of the postprandial increase of plasma amino acids after ingestion of a protein-rich meal to the rise of the three pancreatic hormones insulin, glucagon, and pancreatic polypeptide (PP). A mixed amino acid solution was designed, which permitted a fairly close imitation of the arterial plasma pattern of the 21 amino acids that rise after ingestion of a 200-g porcine steak meal. In 10 healthy subjects the intravenous infusion of this mixed amino acid solution at a rate of 10 g/h elicited a rise of the 21 amino acids examined that correlated well with the postprandial increase (r = 0.89, p < 0.001). The maximal rise of plasma insulin (64 +/- 5 pmol/L) and glucagon (630 +/- 21 ng/L) was not significantly different from the postprandial increase of these two hormones (49 +/- 4 pmol/L and 780 +/- 28 ng/L, respectively). PP levels rose by 316 +/- 33 ng/L postprandially, which was clearly above the increase of 112 +/- 13 ng/L during intravenous amino acids (p < 0.01). In conclusion, the present data demonstrate that the postprandial rise of amino acid levels in arterialized venous plasma can account for most if not all of the postprandial increase of insulin and glucagon during the ingestion of a protein-rich meal. In contrast, only 35% of postprandial PP levels can be ascribed to the rise of plasma amino acids. In contrast to the effect of carbohydrate-rich meals, an enteric augmentation of insulin release seems to be of minor and possibly of no importance during ingestion of protein-rich meals.

A new device for interstitial 125Iodine seed implantation.

A new device for interstitial implantation of I125 seeds is presented. The technical details and function of the system are described and compared with well-known commercial systems. Its unique design allows for simple, fast, and safe treatment of most tumor sites including stereotactic implantation of brain. Radiation measurements indicate low exposure to both patients and personnel during implantation.
