Management of multiple conceptions after gonadotropin-releasing hormone analog/human menopausal gonadotropin/human chorionic gonadotropin therapy.

The treatment course of a 31-year-old infertility patient due to PCO disease is presented. Because the patient failed to conceive after various treatment cycles with CC, she was subjected to a combined GnRHa/hMG/hCG therapy. After plasma E2 levels had reached 2400 pg/ml, three leading follicles, with diameters of 20 to 24 mm, were detected. Induction of ovulation was achieved by 10,000 IU hCG. The patient conceived and developed ovarian hyperstimulation. At 8 weeks of gestation, seven cystic structures were detected within the uterine cavity, five containing single embryos, and two with twin embryos. All nine embryos were vital, as evidenced by their heart beats. Embryo reduction was achieved by transabdominal puncture on three occasions. The three surviving fetuses were carried to the 34th week of gestation. After delivery by cesarean section, three healthy babies developed normally. This communication illustrates the complications that can be associated with ovulation induction in PCO disease: ovarian hyperstimulation, polyovulation, multiple conceptions, and their clinical management.

[Effect of sex steroids on the lipoprotein profile].

Hyperandrogenemia is associated with relatively high LDL- and low HDL-cholesterol levels, increasing the risk of the development of arteriosclerotic disease. In three women suffering from androgen producing ovarian tumors, this observation could be confirmed. After surgical removal of the ovaries, lipoprotein profiles returned to normal. In contrast, androgens did not affect lipoproteins in two testicular feminized individuals. Despite elevated testosterone levels, the lipoprotein profiles were absolutely normal. This observation indicates that the affects on the lipoprotein profile is receptor mediated.

[Self-inflicted necroses (mastitis factitia) as an unusual course of non-puerperal mastitis].

The present paper reports of a long lasting necrotising mastitis which, regarding the psychiatric aspect of the patient as well, has to be interpreted as artefactual mastitis. The uncommon localisation, the unusual history and histology as well as the hardly accessible personality may be indicative of a self inflicted injury with subsequent inflammation. Additional to the local therapy, these patients need a psychological guidance, avoiding direct confrontation with the suspected pathogenesis.

Effect of progestins, androgens, estrogens and antiestrogens on 3H-thymidine uptake by human endometrial and endosalpinx cells in vitro.

The present study describes the effects of 11 steroid hormones and 3 non-steroidal antiestrogens on the 3H-thymidine uptake by human endometrial and endosalpinx cells in long-term cultures. The compounds were added in various concentrations ranging from 10(-9) to 10(-4) M. Progesterone and 19-nortestosterone caused a dose-dependent reduction of the 3H-thymidine incorporation resulting in a 94-98% inhibition at a concentration of 10(-4) M, gestonoroncaproate and d,1-norgestrel were less effective causing a 49% and 40% decrease. Antiproliferative effects were also noted after the addition of androgens (testosterone, 5 alpha-dihydrotestosterone, 5 beta-dihydrotestosterone and danazol). The inhibitory effect of testosterone was equivalent to progesterone at concentrations of 10(-4) M. The addition of estrogens (estrone, estradiol-17 beta and estriol) and antiestrogens (tamoxifen, N-desmethyl- and 4-OH-tamoxifen) produced a dual response in monolayer cultures as low concentrations (10(-9)-10(-6) M) were associated with a slightly increased 3H-thymidine incorporation while pharmacological concentrations (10(-5)-10(-4) M) were followed by a significant decrease. Cells originating from the endosalpinx did not respond to either estradiol-17 beta or progesterone. These results suggest that in contrast to endometrium, the proliferation of endosalpinx cells is independent of sex steroids.

[Modification of lipid metabolism by hPL in late pregnancy].

In the clinical study reported here, the relationship between the behavior of the serum lipids and the concentration of placental lactogen (HPL) was investigated in 107 gravidae in the last trimester. Among the patients examined there were 99 single pregnancies and eight multiple pregnancies--2x twins, 3x triplets, and 3x quadruplets. In these examinations, a significant positive correlation (p less than 0.01) was found between the HPL concentrations and the triglyceride values. Increases in HPL concentrations were associated with increasing total cholesterol and LDL cholesterol values. There was no relation to HPL values in the HDL and VLDL fraction of the cholesterol. Nor was there any relation between estrogen secretion in urine, which was measured in 42 patients, and plasma lipid concentrations. Extreme HPL values in multiple pregnancies, of more than 9 micrograms/ml, were associated with triglyceride concentrations of 383 +/- 37 mg/dl. Total cholesterol in these patients reached values of 294 +/- 19 mg/dl. These values returned to normal on the third day post partum. The mean HDL: total cholesterol ratio was 0.245 +/- 0.065 and was thus clearly above the pathognomonic limit of 0.150. The findings obtained in this study are indicative of the pronounced lipolytic potency of HPL.

Aromatase activity in monolayer cell cultures of human endometrium.

Monolayer cell cultures of proliferative human endometrial stromal cells were incubated with 100 nCi 3H-androstenedione or 100 nCi 3H-testosterone for 15 hours. After termination of the incubations 16% of the recovered radioactivity was identified as estrone by thin layer chromatography and subsequent recrystallization. 3H-testosterone was almost quantitatively converted to 3H-androstenedione. 3H-dihydrotestosterone remained unmetabolized. These findings indicate a highly active 17 beta-ol-dehydrogenase and a high aromatizing capacity of human endometrial cells.

In vitro responsiveness of ovarian epithelial carcinomas to endocrine therapy.

As previously reported, ovarian epithelial carcinomas may respond to endocrine therapy. We examined the direct effect of progesterone, medroxyprogesteroneacetate, gestoneron, 17-beta-estradiol, tamoxifen, 4-OH-tamoxifen, or N-desmethyltamoxifen on the proliferative capacity of ovarian carcinoma cells by means of the colony assay described by Hamburger and Salmon. The growth rate of 25 tested tumors (ascitic fluid, primary tumor, metastases) was 68%. The plating efficiency was 0.078%. Beside the drug testing estrogen and progesterone receptor levels were determined. The inhibition of colony survival was slightest with 17-beta-estradiol, more pronounced with medroxyprogesteroneacetate, gestoneron, N-desmethyltamoxifen, and progesterone, and greatest with 4-OH-tamoxifen and tamoxifen. Significant and dose-dependent inhibition of greater than 70% was observed with tamoxifen and 4-OH-tamoxifen in 80% of the tested tumors. There was no significant correlation between the in vitro responsiveness and the level of hormonal act not only via an estrogen receptor but also via an antiestrogen-binding site.

Cytosol and nuclear progesterone-receptor concentrations in the rabbit endometrium during early pseudopregnancy under different treatments with estradiol and progesterone.

In an attempt to understand the mechanism of the antiprogestational action of estrogens during early pseudopregnancy we determined the cytosolic and nuclear concentrations of progesterone receptors in the endometrium of rabbits treated with hCG followed by various combinations of estradiol and progesterone. The progestational response of the endometrium was followed by quantitation of the uteroglobin content in the uterine lumen. In rabbits treated with hCG alone there was a clear progestational response (40% relative uteroglobin content), but only 16% of the progesterone receptors were located in the nucleus. After additional treatment with progesterone the progestational response remained high (45% relative uteroglobin content), the total cellular content of progesterone receptor increased, and 5% of the complexes were found in the nucleus. These findings suggest that a consumption of nuclear progesterone receptor is required for progestational action. Treatment of pseudopregnant rabbits with estradiol resulted in a marked increase not only of the total cellular progesterone receptor but also of the percentage of it located in the nucleus (35%). Concomitantly, the progestational response was markedly inhibited (5% relative uteroglobin content). These results confirm the relevance of nuclear consumption of progesterone receptor for progestational action, and suggest that some antiprogestational effects of estrogens may be due to their interference with the mechanism of progesterone receptor processing.

[Concentration of estrogen and progesterone receptors in the lower uterine segment at term labor].

BACKGROUND: To determine cytosol estrogen and progesterone receptor concentrations in the human lower uterine segment in different stages of cervical dilatation during parturition at term. PATIENTS AND METHODS: Biopsy specimens of the lower uterine segment were obtained from 51 women undergoing non-elective cesarean section at term. The stage of cervical dilatation at the time of cesarean section was < 2 cm (N = 14), 2 to < 4 cm (N = 13), 4 to 6 cm (N = 11) or > 6 cm (N = 13). The cytosolic estrogen and progesterone receptor concentrations were determined by enzyme immunoassays. RESULTS: Median estrogen receptor concentrations at < 2 cm and at 2-< 4 cm cervical dilatation were 2.12 fmol/mg protein and 2.03 fmol/mg protein, respectively. After a significant drop at 4-6 cm cervical dilatation (median: 1.08 fmol/mg protein), estrogen receptor concentrations raised again at > 6 cm cervical dilatation (median: 2.00 fmol/mg protein). Median progesterone receptor concentration was 84.7 fmol/mg protein at < 2 cm cervical dilatation, diminished significantly at 2-< 4 cm cervical dilatation (36.6 fmol/mg protein) and increased at further cervical dilatation (4-6 cm: 75.7 fmol/mg protein; > 6 cm: 83.7 fmol/mg protein). CONCLUSIONS: These data suggest that predominantly a decrease in cytosolic concentration of the progesterone receptor at the beginning of the active phase of first stage labor may play a crucial role in the hormonal control of cervical dilatation during parturition at term.
