[Comparative estimation of monocompartmental methods of analyzing 99m-Tc-diethylenetriamine pentaacetic acid clearance by declining plasma levels with special reference to long term studies].

Plasma slope clearance estimation with 99m-Tc-DTPA corresponds to total clearance determination. When activity is measured directly in cumulative serum samples, determination of the plasma slope clearance is suitable for practical applications only if simplified analytic procedures are used. Monocompartmental analytic procedure that take the late slope of the disappearance curve into account are particularly useful for GFR estimation. The A3k3-slope-clearance analysis method is a monocompartmental procedure for determining slope clearance that is based on activity measurements between the 100th (120th) and 240th minute after injection. Compared with other monocompartmental procedures, the A3k3-clearance method shows a highly significant hyperbolic correlation with serum creatinine, a highly significant linear correlation with inulin total clearance, is age dependent and yields reproducible results. The sensitivity (frequency of correct function results in the case of disturbed glomerular function) of the A3k3-slope-clearance method with 99m-Tc-DTPA is particularly good. The practically important predictability of pathological A3k3-clearance results (frequency of correct results when the kidney function is truly reduced) is on the average 85%. The mean efficiency of the method is 88% (65-97% at the 95% confidence-level).

[Antidepressive therapy with carbamazepine (Finlepsin)].

For evaluation of the possible effect of Carbamazepine (CBZ) in the treatment of endogenous affective disorders 15 patients with manic-depressive illness including 2 cases in the manic and 13 cases in the depressive phase were studied in an open controlled clinical trial. Blood-levels of CBZ were determined weekly. Clinical response was measured by using different psychopathometric methods (SPES, AMP, HAMD, B-S [v. Zerssen], Beck-Inventory). In the two cases with dominant manic syndrome we observed good regression of psychopathological symptomatology during the first week of the treatment. In the same way good therapeutic results were found in the group of depressive patients. In 11 cases we observed improvement already in the first days of the treatment. There did not exist a defined correlation between blood-levels of CBZ and clinical responses of the patients. Side-effects were rare and disappeared during the further treatment after some days.

[Noma].

A 1 4/12 years old boy after having been ill with measles developed gingivostomatitis ulcero-gangrenosa, a disease known as noma in tropical countries. This rare disease usually occurs during severe depression of the immunologic defence like with measles, malnutrition, malignant neoplasmas or chemotherapy, and is caused by a local infection with aerobic and anaerobic bacteria, which usually are part of the normal oral flora. The course of the disease is characterized by an initially edematous swelling of the face followed by ulcerations of the gingiva, progressing as an ulcerophlegmatous inflammation of the oro-facial tissue and finally causing mutilating destruction. Vigorous general and antibiotic treatment has improved the previously poor prognosis.

[Results of the determination of enteral calcium absorption and calcium kinetics with 47Ca in patients with chronic recurring urolithiasis].

In the context of the classification of hypercalciuria in urolithiasis, the measurement of enteral calcium absorption is very important. A precise method is that of oral load with 47Ca and measurement of its activity by means of a human body counter; 47Ca activity in serum, urine and stools in simultaneously measured. To date 47 patients with chronically recidive calcium lithiasis (with between 3 and over 100 stone episodes) and an average age of 43.2 years have been examined. Calcium absorption lay between 42.7% and 90.0% of the dose, the average was 59.3% with a standard deviation of +/- 12.9. This is higher than in persons with healthy kidneys, but not significantly so. Calcium absorption is significantly lower in patients with renal insufficiency. The studies on calcium kinetics revealed turnover rates and pool sizes within the normal range.

[Aneurysm of the left ventricle in childhood. A contribution on classification and a review of the literature].

Two cases of left ventricular aneurysm in children offer the opportunity of classifying protrusions of the left ventricle into (a) embryonically malformed contractile diverticula and (b) aneurysms, which may be subvalvular, paraventricular, or apical in location. Numerous etiologies have been proposed, but none of these are supported by our examples. Clinical findings on an apical aneurysm of unknown etiology are presented. While in the case of the diverticulum surgical resection is strongly indicated, in the case of aneurysm conservative treatment may often be adequate.

Dissociation of hemoglobin accumulation and commitment during murine erythroleukemia cell differentiation by treatment with imidazole.

The effect of imidazole on DMSO-induced murine erythroleukemia (MEL) cell differentiation has been examined. While imidazole does inhibit heme, globin mRNA, and hemoglobin accumulation in DMSO-induced MEL cells, it does not affect the commitment of MEL cells to the specific limitation of proliferative capacity associated with the in vitro differentiation program. Furthermore, imidazole treatment does not affect DMSO-induced changes in cell volume, in the relative proportion of nuclear protein IP25, and in the specific activity of the enzyme cytidine deaminase. A clonal analysis in the presence of imidazole indicated that the drug prevents heme accumulation even in MEL cells already committed to terminal differentiation. These observations suggest that imidazole effectively dissociates two aspects of the erythroid differentiation program of MEL cells: globin gene expression and commitment to loss of proliferative capacity.

Inhibition of oxygen evolution in chloroplasts by ferricyanide.

Preincubation of chloroplasts from pea leaves (Pisum sativum L. cv. Kelvedon) with 0.5 millimolar ferricyanide in the dark, caused a parallel inhibition of the rate of rise of the variable fluorescence and the rate of electron transport. Both reactions were inhibited to a similar extent by varying the time of preincubation, the concentration of ferricyanide during preincubation, and by raising the concentration of salts in the preincubation medium. Ferricyanide treatment of Tris-washed chloroplasts did not inhibit electron transport from the Photosystem II (PSII) electron donor 1,5-diphenylcarbazide to methylviologen. The inhibition of the variable fluorescence rise and of NADP reduction (caused by ferricyanide pretreatment) was bypassed by addition of the PSII electron donor couple hydroquinone/ascorbate. It was concluded that preincubation of chloroplasts with ferricyanide in the dark inhibited electron transport between water and PSII.

[Compartment analysis of potassium metabolism--mathematical modelling of 42-K tracer kinetics].

The possibility of application of the theoretical concept of the compartment analysis in the modelling of the potassium balance was demonstrated. A use of the compartment analysis of the potassium balance in the clinical routine diagnostics in the sense of a highly specialised functional diagnostics seems according to recent experiences reasonable only in closest cooperation with clinicians. The diagnostics and the control of therapy of the chronic syndrome of potassium deficit as well as metabolically conditioned disturbances of the distribution of potassium were to be seen as possible indication. At the same time the establishment of reference values for the demarcation of relative disturbances of distribution as well as for the establishment of the normal region of the turnover rates were necessary.
