In patients operated for cerebral aneurysm or angioma, the same basic method of anesthesia has been used. Premedication consisted of Thalamonal or diazepam. After induction with thiopentone, curarisation with pancuronium and tracheal intubation, anesthesia was maintained with N2O 70%, O2 30% and enflurane 1%. Small doses of fentanyl or Thalamonal were given at the beginning of anesthesia, but no more within 30 minutes before starting controlled hypotension. Adjuvant drugs and methods to reduce intracranial pressure were also used, such as dexamethasone, mannitol and cerebro-spinal fluid subtraction. The approach and dissection of the vascular lesion was done under controlled hypotension with sodium nitroprusside 0.01% solution. The mean dose of sodium nitroprusside to maintain a mean blood pressure at about 50 Torr was 1.37 mcg/kg/min.
Solitary vertebral osteochondromas with spinal cord compression are very rare. Here we report a case of an osteochondroma of the 5th cervical vertebra protruding into the spinal canal. The therapeutic approach is discussed in relation to the natural evolution and the favourable prognosis of these lesions. The usefullness of the C.T. scan is shown both in diagnosis and post-operative follow-up.
The CT features of cerebellar lesions with von Hippel-Lindau's disease are analyzed. The use of CT scanning for associated spinal hemangioblastomas is discussed in the light of a case with a low cervical lesion. The role of computed tomography in the neuroradiological investigations is discussed.

The authors present a serie of 20 cases of craniofacial injuries with or without CSF leakage. They propose a simple classification in 2 groups following the impact: braniofacial injury or orbitofacial injury. For their, craniofacial or orbitofacial injuries are to be operated in one stage by an neurosurgical and maxillofacial team. The best time for operation is approximately around the 10th day following trauma.

A case of multiple tuberculomas of the brain in a pregnant patient is reported. The symptoms and signs suggested a lesion of the cerebellum which was found to be only congested and oedematous at craniotomy. The correct diagnosis was made at autopsy.

Value of brain scanning in pediatric subdural collections. Acta Paediatr Scand, 64:2, 1965. Eighteen children with subdural collections were submitted to brain scintigraphy. By this method, idopathic and post-traumatic hematomas were detected in 40% of the cases, and subdural effusions in 70% of the cases. No false-negative results were noted in the 3 cases of empyema. Several false-positive images were recorded, most of them following purulent meningitis, without any satisfactory explanation. Neither the technique of scintigraphy used in the department, the dimensions of the skull, the age of hematoma, nor the presence of membranes seemed to affect the accuracy of the method. Compared with the other easily performed examinations (eye fundus, EEG, Echo), scintigraphy still remains important in the diagnosis of subdural collections in children.
Endotracheal administration of gentamicin has been compared to the endotracheal administration of aminosidin plus polymyxin B as a preventive measure against tracheobronchial infections in 25 and 22 tracheotomized patients respectively who had been admitted to a neurosurgical unit. Both series were comparable as far as underlying disease, duration of hospitalization, surgical therapy. Both regimens were similarly effective from the bacteriologic and clinical points of view. Both regimens were similarly effective in preventing colonization of bronchial secretions by potential pathogens and were associated with a similar frequency of infectious episodes (eight in each group). The use of aminosidin-polymyxin B combination was associated with a lower incidence of emergence of gentamicin resistant strains, but the endotracheal administration of gentamicin was better tolerated than that of the combination. It is concluded that the combination of aminosidin-polymyxin is a useful alternative to gentamicin for the prevention of bronchopulmonary infections in unconscious tracheotomized patients.

Presentation of two cases of open orbital roof fracture with destruction of all the orbital roof extending from the fronto-maxillo region to the spina orbitalis. After a first emergency neurosurgical treatment, a cast of the excised soiled bone fragment was taken and identical replica was made from methylmethacrylate. In a second surgical procedure, the resine mould was set in place by endocranicl approach, and was fastened firmly to the surrounding bone with wire sutures. In such types of defects cranioplasty and orbitoplasty are mandatory.
