Lipoprotein and apolipoprotein profile in men with ischemic stroke. Role of lipoprotein(a), triglyceride-rich lipoproteins, and apolipoprotein E polymorphism.

BACKGROUND AND PURPOSE: The role of lipoprotein abnormalities in the development of ischemic cerebrovascular disease has not been sufficiently clarified. The aim of this study was to identify the lipoprotein profile in ischemic cerebrovascular disease and the possible role of apolipoprotein E polymorphism. METHODS: The relation between the concentrations of lipoprotein(a), intermediate density lipoproteins, apolipoprotein A-I, apolipoprotein B, apolipoprotein E, and other lipoproteins was studied in 100 men with ischemic cerebrovascular disease (48 atherothrombotic, 28 lacunar, and 24 of unknown type) and in 100 healthy age-matched men as a control group. RESULTS: Patients with ischemic cerebrovascular disease had significantly higher levels of lipoprotein(a), lipids carried by intermediate density lipoproteins, and low density lipoprotein cholesterol and lower levels of high density lipoproteins than control subjects. Patients with atherothrombotic infarction had higher total serum cholesterol and low density lipoprotein cholesterol concentrations than patients with lacunar infarction. To assess lipoprotein abnormalities in normolipidemic subjects, a subgroup of 38 patients with ischemic cerebrovascular disease and 53 control subjects, both with serum cholesterol levels < 5.2 mmol/l (200 mg/dl) and triglycerides < 2.3 mmol/l (200 mg/dl), was analyzed. Serum lipoprotein(a), lipids carried by very low density lipoproteins and intermediate density lipoproteins, and low density lipoprotein triglycerides were significantly higher in normolipidemic patients compared with normolipidemic control subjects, whereas high density lipoprotein cholesterol levels were lower. Apolipoprotein E polymorphism in our ischemic cerebrovascular patients differed from that of the control group, with the epsilon 4 allele being more prevalent. CONCLUSIONS: Increased serum lipoprotein(a) levels and intermediate density lipoprotein abnormalities together with decreased high density lipoprotein levels are major risk factors for ischemic cerebrovascular disease, even in normocholesterolemic and normotriglyceridemic subjects. Finally, the epsilon 4 allele could probably be a predisposing genetic marker for ischemic cerebrovascular disease.

Comparative surface microanalysis of failed Brånemark implants.

The chemical composition and topography of an implant surface determine the human immunologic system response. This study compared the surfaces of 13 Brånemark oral implants, 11 that came from retrieved specimens which failed initially or did not osseointegrate and 2 that were never implanted (controls). The period of implantation in human jaws varied between 3 and 20 months. After cleaning and sterilization, the topography, surface chemical composition, and thickness of the oxide layer were studied. The results obtained with scanning electron microscopy did not show any significant topographic differences among the specimens. X-ray spectrographic microanalysis showed very similar composition (titanium and amounts less than 0.5% of other elements) in the outermost layer of the analyzed specimens. The Auger spectroscope revealed considerable percentage differences in the amount of carbon and silicon in the last monolayers, which could be attributed to handling or to an inadequate cleaning process. This places the retrieved specimens out of the acceptable statistical limits of contamination by introducing a factor of doubt for long-term prognosis in the hypothetical situation of their re-use.

Periotest method as a measure of osseointegrated oral implant stability.

Oral implant stability is essential for optimal function. Results obtained from the Periotest measurement of the stability of 204 commercially pure titanium implants, consecutively placed in 22 maxillae and 24 mandibles according to the Brånemark procedure, are reported. Preliminary results suggest that the Periotest value of an oral implant is an objective and easily applied criterion for stability assessment. Since osseointegration is achieved gradually over time, this test may assist the clinician in deciding whether to extend the healing period before loading fixtures that seem clinically and radiologically integrated but give borderline Periotest values.

Characterization of surface roughness in titanium dental implants measured with scanning tunnelling microscopy at atmospheric pressure.

Characterization of the surface topography of implant materials is important for understanding tissue response. We have measured, for the first time, the topography of titanium surfaces used in osseointegrated dental implants. Scanning tunnelling microscopy (STM) which provides 3D real space images was used. In addition to clinical samples, electropolished and anodically oxidized surfaces were also measured. Clinical samples are rather inhomogeneous in character showing grooves and steps with a maximum depth of 0.11 micron. Micropores with an average diameter of about 30 nm are also present. Electropolished samples are rather homogeneous and very smooth, showing steps of 1 to 5 nm in height. The measurements were performed under atmospheric conditions at a resolution in the subnanometer range.

[Intraoperative endoscopic surveillance in laparoscopic surgical treatment of achalasia].

We study the utility of intraoperative endoscopic control, in laparoscopic surgical treatment of the typical Achalasia of Esophagus. 81 patients were intervened surgically in Centro de Cirugia Endoscopica. Hospital Universitario Calixto Garcia in a five year-old period. All patients received intraoperative endoscopic evaluation to guarantee the effectiveness and integrity of the esophago-cardiomyotomy. The average time of surgical procedure was of 144,7 minutes. The transendoscopic complication in relation with endoscopic procedure was seen in one case but it didn't modify the morbi-mortality of the study. According to Vantrappen classification, 79 patients were considered with a evolution between excellent and good. The use of endoscopic intraoperative control is recommended in the surgical interventions of the patients with achalasia.
