Results of treatment of uncomplicated urogenital gonorrhoea with enoxacin compared with ceftriaxone.

The increasing incidence of penicillinase producing Neissreria gonorrhoeae (PPNG) and chromosomally-mediated resistant N. gonorrhoeae (CMRNG) has generated a need for therapy other than penicillin. PPNG constitutes about 30% of the circulating strains of gonococci in the high risk population in Mexico City. The object of the present study was to compare the safety and efficacy of single-dose treatment with either oral enoxacin (400 mg tablet) or intramuscular ceftriaxone (250 mg injection), for the treatment of acute uncomplicated gonococcal urethritis in men, caused by N. gonorrhoeae, including both PPNG and non-PPNG strains. Of the 93 men with uncomplicated gonorrhoea entered into a study, randomly assigned to receive ceftriaxone or enoxacin, 80 completed it. Infections were initially diagnosed by Gram-stained smears of urethral exudate and subsequently confirmed for isolation of N. gonorrhoeae. Chlamydia trachomatis was confirmed by direct immunofluorescence. About 30% of the strains of gonococci were PPNG and 1% were CMRNG. C. trachomatis was coexisting with N. gonorrhoeae in about 25% of the patients and remain positive after treatment. In the study presented, a single dose of either enoxacin or ceftriaxone was highly active against both PPNG or CMRNG. One single dose was enough to achieve a 100% cure rate. There was no difference between the two treatment groups. Thus either regimen has proved to be a useful alternative to the present-day treatment of uncomplicated gonococcal infections.(ABSTRACT TRUNCATED AT 250 WORDS)

The structure of rat 28S ribosomal ribonucleic acid inferred from the sequence of nucleotides in a gene.

The nucleotide sequence of a rat 28S rRNA gene was determined. The 28S rRNA encoded in the gene contains 4718 nucleotides and the molecular weight estimated from the sequence is 1.53 x 10(6). The guanine and cytosine content is 67%. The sequence of rat 28S rRNA diverges appreciably from that of Saccharomyces carlsbergensis 26S rRNA (about 50% identity), but more closely approximates that of Xenopus laevis 28S rRNA (about 75% identity). Rat 28S rRNA is larger than the analogous nucleic acids from yeast (3393 nucleotides) and X, laevis (4110 nucleotides) ribosomes. The additional bases are inserted in specific regions and tend to be rich in guanine and cytosine. 5.8S rRNA can interact with 28S rRNA by extensive hydrogen bonding at two sites near the 5' end of the latter.

Immunology of histoplasmosis: humoral and cellular activity from a polysaccharide-protein complex and its deproteinized fraction in experimentally immunized mice.

In a previous publication it was reported that a polysaccharide-protein complex (PPC), sensitive to beta-glucosidase, was isolated from Histoplasma capsulatum. This complex was strongly reactive in an agar gel diffusion assay with sera from patients with histoplasmosis, but was unreactive with sera from patients with coccidioidomycosis. Here, the studies with human sera have been expanded and attempts were made to determine the response of mice immunized with nonviable H. capsulatum or Coccidioides immitis to PPC or its deproteinized fraction (D-PPC) using more sensitive tests for antibody and including also test for cell-mediated immunity. Histoplasmin and coccidioidin were compared with PPC or its deproteinized fraction (D-PPC) in all assays. In a counterimmunoelectrophoresis (CIE) assay, PPC and D-PPC reacted only with sera from patients with histoplasmosis, whereas cross reactions were noted with histoplasmin and coccidioidin using heterologous sera. Cross-reaction were observed with all four antigen preparatins and both types of antisera using a microcomplement fixation assay. The assay for macrophage migration inhibitory factor (MIF) was also relatively nonspecific, in that inhibition occurred with cells from animals sensitized with Histoplasma or Coccidioides using both homologous and heterologous antigens. In the footpad assay, histoplasmin and coccidioidin were highly cross-reactive in animals sensitized with the heterologous fungus, but the PPC and D-PPC from H. capsulatum elicited significant reactions only in animals sensitized with Histoplasma.

Possible relationship between neurocysticercosis and hematological malignancies.

BACKGROUND: Previous studies have shown an increased frequency of chromosomal abnormalities in lymphocytes from animals and humans with cysticercosis. Some reports have suggested an association between cancer and cysticercosis. The aim of this study was to investigate the possible association between neurocysticercosis and cancer. METHODS: We designed a mortality rate study from the autopsy files of the Department of Pathology at the General Hospital of Mexico. A total of 1,271 autopsy files were reviewed. All files in which a malignant neoplasia was found during autopsy were selected as cases. Autopsies in which no malignant disease was found were used as controls. The odds ratio was determined between the frequency of neurocysticercosis in patients with any malignant neoplasia and that of the controls. RESULTS: Neurocysticercosis was more frequent in cases with malignant hematological diseases (MHD) than in controls (p = 0.01). The odds ratio for this association was 3.54, with 95% confidence interval from 1.17-9.79. CONCLUSIONS: Most human cancers arise from the interaction of a multiplicity of factors, including xenobiotics and endogenous constituents. Therefore, while it will be difficult to demonstrate that neurocysticercosis is a causal agent of malignant hematological diseases (MHD), it should be considered as a potential risk factor for cancer induction in countries where cysticercosis remains a public health problem.

[The use of the International Guidelines for the Diagnosis and Treatment of Asthma (GINA) in clinical practice among family physicians].

OBJECTIVE: To evaluate the degree of knowledge of family physicians related to the International Guidelines for the Diagnosis and Treatment (GINA) of Asthmatic Patients. MATERIAL AND METHOD: We invited the family physicians from some clinics of Mexico City. A questionnaire was applied to 50 doctors who accepted to participate in the study; they had a 46 year-old age average and a proportion 1:1 between genders. The analysis was carried out with the test r of Pearson. RESULTS: The correlation between general knowledge of asthma and knowledge of GINA was r = 0.022; between the general knowledge of asthma and the specialty of family medicine was r = -0.117; and that among the knowledge on International Guides of Asthma and the specialty of family medicine was r = 0.33. CONCLUSIONS: A scarce knowledge exists on the guides proposed by GINA.

[Epidemiology of rubella in Mexico].

An analysis of the transmission dynamics of rubella in Mexico based upon notified cases during 1983-1990 is presented. It is observed, for the first time, that the force of infection in the age-group of 15-44 years old and the average age of the infection at the national level show a slight increase simultaneously. The theoretical estimates of the interepidemic period coincide with the observed ones. The annual proportion of susceptible individuals is practically constant during 1983-1990. The intrinsic reproductive rate of rubella at national level and for the different age-groups is assessed. The annual estimates of Ro are small and they are practically unchanged for all age-groups, excepting the group of 15-44 years. The herd immunity of rubella for the different age-groups is evaluated and it is concluded that the best strategy for preventing congenital rubella in Mexico is to continue with the no vaccination policy against rubella. Nonetheless, the continuous monitoring of this policy is necessary.

[Seroepidemiology of rubella in Mexico: data and theory].

The pattern of seropositivity of rubella obtained through the National Seroepidemiological Survey (NSS) of 1987-1988 is compared with the observed annual patterns of morbidity and with theoretical serological profiles based upon the age-distribution of cases. In spite of a lack of direct correspondence among the NSS results and the ones obtained from morbidity data, a relationship among the patterns and trends is observed. The theoretical methods used for inferring the proportion of seropositives as a function of age provide an inexpensive and fast alternative in order to monitor the current policy of no vaccination against rubella in Mexico.

[Regionalization of Mexico based on primary health care indicators proposed by the WHO].

In order to carry out this work, the following questions were made: how has the development of the coverage of Primary Health Care (PHC) for each entity of the Mexican Republic been? has it been homogeneous? If heterogeneous, which states show similar development? and, which are the indicators of PHC that account for these differences? A statistical multimodal analysis known as Cluster analysis, based upon the PHC indicators recommended by the World Health Organization (WHO) in 1985, was carried out in order to answer the questions above made. One of the main results of this work was to identify clearly that there are important inequities among the different federal agencies regarding coverage. These differences indicate that the coverage of PHC in México has been polarized.
