[Choanal atresia--experience with early surgery].

INTRODUCTION: Prior to 1982, patients in our department with choanalatresia were operated by the transpalatinal approach. To eliminate the risk of insufficient transpalatine growth, only older children were operated. MATERIAL AND METHODS: From 1982 we have chosen to operate as soon as symptoms occurred, usually at an earlier age. Therefore, we chose the transnasal approach in 32 patients. RESULTS: Of 14 patients with bilateral choanalatresia, nine were operated before two months of age. In nine patients we took care to remove some of the posterior part of vomer. Four of them have been symptom free for more than a year, an additional two symptom free but with an observation period of less than a year, and three have renewed symptoms. Of 18 patients with unilateral atresia, 11 were operated before seven years of age, seven of them successfully. Of the seven operated after the age of seven, six are symptom free. INTERPRETATIONS: We believe that these results justify continuing with transnasal operations as soon as symptoms are distressing, regardless of age.

The effect of cetirizine on symptoms and signs of nasal polyposis.

Forty-five patients with residual or recurrent nasal polyposis after ethmoidectomy were treated with either cetirizine at twice the daily recommended (20 mg) dose or placebo for three months. The number and size of polyps remained unchanged during the study period. Cetirizine was found to reduce nasal sneezing and rhinorrhoea effectively. The drug also had a beneficial effect on nasal obstruction in the latter part of the study. The side effects of 20 mg (double the recommended daily adult dose) of cetirizine were few and comparable to placebo.

Phenotypic distribution of T cells in human nasal mucosa differs from that in the gut.

Phenotypic and functional studies are required to understand the immunoregulatory role of mucosal T cells. Information about T cells in the human upper respiratory tract is limited and conflicting. Therefore, we phenotyped T cells in nasal mucosa by means of multicolor in situ immunofluorescence. In normal mucosa, most CD3+ intraepithelial lymphocytes (IELs) and lamina propria lymphocytes (LPLs) (> 90%) expressed T-cell receptor (TCR)alpha/beta, and only approximately 5% expressed TCRgamma/delta. Although most IELs in the surface epithelium were CD8+ (64%), many expressed CD4 (30%) and the CD4 phenotype dominated (55%) only slightly in the lamina propria. This result was strikingly different from that obtained for comparable compartments in histologically normal jejunal mucosa, where IELs consisted of 83% CD8+ and LPLs of 73% CD4(+) T cells. Nasal CD3+ IELs and LPLs were mainly CD45RO+CD45RA- and usually expressed CD7. The integrin alphaEbeta7 was, as expected, more common on IELs than on LPLs (78 versus 20%). In conclusion, nasal T cells show several similarities to those of the normal jejunum but some notable differences exist, especially a relative increase in CD4+ T cells in the epithelium and a decrease in the lamina propria. It should be explored whether this disparity, together with an increased expression of epithelial adhesion molecules, might contribute to local immunological overstimulation and partly explain the relatively high frequency of airway allergy.

Eosinophil infiltration is related to increased expression of vascular cell adhesion molecule-1 in nasal polyps.

Endothelial adhesion molecules are important in the recruitment of leukocytes to inflammatory sites. Nasal polyps characteristically contain a leukocyte infiltrate in which eosinophils often are remarkably prominent. We have studied whether this feature is related to a particular profile of adhesion molecules on the local microvascular endothelium. Nasal polyps were obtained from 15 patients. Mucosal biopsy specimens of the lower and the middle turbinate from the same patients as well as from three control subjects served as reference tissue. Expression of endothelial adhesion molecules and the relative numbers of eosinophils and neutrophils were examined by two- and three-color immunofluorescence staining. Both the number of eosinophils and the proportion of vessels positive for vascular cell adhesion molecule-1 (VCAM-1) were significantly increased in nasal polyps compared with the turbinate mucosa of the same patients (P = 0.008 and P = 0.001, respectively). By contrast, the number of neutrophils and the relative expression of E-selectin and intercellular adhesion molecule-1 were similar at both tissue sites. Furthermore, the relative number of eosinophils in nasal polyps was well correlated (rs = 0.73, P = 0.006) with the percentage of vessels positive for VCAM-1, but this was not true for neutrophils. Taken together, this direct in situ observation strongly supports the crucial role suggested for VCAM-1 in human eosinophil extravasation at inflammatory sites.

[Dystrophy caused by upper airway obstruction].

We describe three infants whose main symptoms were poor weight gain, delayed motor development and recurrent respiratory infections. Obstructive sleep apnea was diagnosed, and all three improved after adenotomy or adenotonsillectomy. Obstructive sleep apnea is an important differential diagnosis in infants showing retarded development and failure to thrive.

Treatment of T3 laryngeal carcinoma. Clinical evaluation after 50 Gy, is it of any value?

29 patients with T3 carcinoma of the larynx were evaluated after irradiation with 50 Gy with special emphasis on the mobility of the vocal cord. 10 of the patients with fixation of the vocal cord were assumed to have unsatisfactory response to treatment and were therefore advised laryngectomy. In 4 of the patients the fixation was due to fibrosis. One patient also had a fixed vocal cord, but was discarded from the investigation because of inoperable regional metastases. 18 patients with improved mobility of the vocal cord continued the irradiation to 70 Gy. 6 of these patients later developed local recurrences. It is concluded that the mobility of the vocal cord after 50 Gy is of dubious prognostic value.

Irradiation-induced tumours of the head and neck.

Though irradiation-induced tumours are uncommon they represent a well defined entity. At this Hospital, 14 "irradiation-induced" head and neck tumours were encountered in 11 patients over a 10-year period. The irradiation had been given for tuberculous lymph-adenitis in 6 of the patients, for lupus vulgaris in one, and thyrotoxicosis in another; the other 3 patients had received radiotherapy for malignant tumours. The interval between the treatment and the diagnosis of the tumour disease ranged from 9 to 48 years (mean 32). The induced tumours included 10 squamous cell carcinomas of the hypopharynx (4 tumours), the buccal mucosa (3), the skin (2), and the larynx (1), one poorly differented carcinoma of the parotid gland, 2 thyroid carcinomas and 1 fibrosarcoma of the stenocleidomastoid muscle. Three of the patients had multiple tumours. In view of the risk of cancer--albeit a small one--associated with radiological diagnosis and radiotherapy, these should be performed only on strict indications, especially in young patients.
