[Changes in the isoniazid inactivation index during antituberculosis treatment in children].

Fluorimetric assays of serum isoniazid levels were performed in children treated either with isoniazid alone, or with isoniazid associated with other antituberculous drugs, in order to adjust the dosage. Study of the inactivation index at the 3rd hour, showed a faster metabolism in the younger children. Assays repeated twice or more in 160 patients, showed important variability in the results. The variation coefficient was 18,6% for the whole group and above 25% in 25% of cases. The younger age and the association of several other drugs (corticosteroids and possibly erythromycine) may be responsible, at least partly, for this variability. However, association with antituberculous drugs (ethambutol, rifampin) does not seem to change significantly isoniazid metabolism. The magnitude of these variations should lead to a greater caution in the interpretation of isolated isoniazid dosages.

[Treatment of iron overload due to repeated transfusions with subcutaneous infusions of desferrioxamine (author's transl)].

The urinary excretion of iron has been studied in 23 cases of iron overload (20 thalassemia major, 2 pure red cell aplasia, 1 congenital sideroblastic anemia) after 12 hour subcutaneous infusions of 0.5 g, 1 g, 2 g and 3 g of desferrioxamine (D). The urinary excretion of iron was correlated with the dose of D and the serum ferritin. Continuous subcutaneous infusion of 0.5 g and 1 g D over 12 hours increased the mean urinary iron excretion from 6.6 to 12 mg/day and 9.9 to 18 mg/day respectively compared with intramuscular injection of a similar dose. The test doses of D have been used to determine the dose of D necessary to balance the iron received in transfusion and the iron excreted in the urine. Five children treated for between 7 and 12 months demonstrated that the treatment is effective. Serum ferritin levels decreased and the children were in negative iron balance. Using a portable constant infusion pump subcutaneous infusions can be given at home.

Distal trisomy 17q.

A 3-year-old, male patient with trisomy 17q231qter due to a paternal t(5;17)(p151;q231) is compared to three other patients reported in the literature who are trisomic for the same segment due to a familial t(17;21)(q23;q22). The features common to the four patients are: profound mental retardation; dwarfism; frontal bossing and temporal retraction; narrow squinty eyes; thin lips with overlapping of the lower lip by the upper lip; very low-set and abnormal ears; cleft palate; and hyperlaxity of the ligaments. It thus seems possible to delineate a new cytogenetic syndrome.
