Lumbar discitis in a patient with quadriplegia: case report.

The diagnosis of septic discitis, or vertebral osteomyelitis, in able-bodied adults is difficult to make and often delayed. Here, the clinical findings and events leading to diagnosis and complications of septic discitis occurring in a patient with quadriplegia after urinary tract manipulation are presented. The diagnosis was delayed nine weeks from onset of fever (11 weeks after urologic manipulation), despite a variety of radiologic procedures and repeated blood and urine cultures. The patient's symptoms recurred five weeks after IV antibiotics were discontinued and while he was still taking oral cephalexin. He underwent open debridement and further IV and IM tobramycin, recovering without complication. The patient died five months later, reportedly of bowel obstruction and pneumonia. A review of the literature revealed only one other case report, of a patient with paraplegia, who also presented a diagnostic problem and died one year after diagnosis.

Upper limb amputees: a clinic profile.

A clinic population of upper limb amputees was located to document their management and functional restoration. Of the 38 people contacted, 26 agreed to participate. Data were collected by questionnaires, and by physical and prosthetic examination. Prostheses were worn by 81% all or part of the day, 77% using active terminal devices. A change in occupation was noted by nine, and one was unemployed. Comfort and cosmesis were satisfactory. Prostheses were reported least useful for leisure activities. Long-term attention to fit and maintenance, updated training, and recent information might result in improved prosthetic function and satisfaction for upper limb amputees.

Dynamic testing of below-knee prosthesis: assembly and components.

Prosthetic assemblies for lower limb amputees are highly engineered and consist of several components each with its own failure mechanism. This paper describes the dynamic testing of HDPE rotational moulded sockets in a specially designed machine which mimics normal gait. Thus the components are subjected to all main loadings occurring during a stride such as axial loading and A-P bending about the knee and ankle. Machine details as well as the other components of the system are described. SACH feet appear to be vulnerable by rapid wear and structural component failures at less than 100,000 cycles were observed. The sockets are much less vulnerable and stand up to simulated loading of 1350 N for approximately 400,000 cycles. Metal components such as the foot bolt may also fail in fatigue if not properly tightened. References to proposed ISO standards are also included.

Skid loader modification for a man having C5-spared quadriplegia.

A front end skid loader was modified to permit a C5-spared quadripletic man to participate in the activities of a family-owned farm. Two elbow control panels each containing 8 microswitches control the motor on/off and 6 electrical actuators that operate engine speed, brake, forward/reverse drive, bucket tilt and bucket arm raise and lower. A self-contained hydraulic crane and electric winch installed on the roof of the protective cage permits transfer to and from the vehicle by 1 attendant operating a control panel on an umbilical cable while guiding the movement of the sling. A modified parachute harness attached to the vehicle frame provides an adjustable quick release restraint system that holds the driver securely in a bucket seat. After 1 year of use no breakdown of the modifications has been reported. Operating the modified farm vehicle has placed this quadriplegic man on a vocational plane equal to other members of his family and farming community.

Rehabilitation of below-knee amputees: factors influencing outcome and costs in three programmes.

The components of three different rehabilitation programmes were analysed to determine the factors that influence functional outcomes and prosthetic costs. Age and additional health problems showed little relationship to inpatient activity level while the physical health status score had a significant relationship. The level of expectation in the rehabilitation programmes for walking independently with canes or without aids at discharge was shown to have a strong influence on the activity level attained and the length of rehabilitation stay. The style of socket did not appear to affect fitting success but had some effect on the efficiency of the prosthetic fitting process and the length of stay.
