[Treatment of recurrence of pleomorphic adenomas of the parotid gland].

This is a retrospective study of 10 patients who underwent surgery for a first or multiple recurrence of pleomorphic adenoma (P.A.). Recurrence may come more than 10 years after an initial episode. During initial surgery, the risk of recurrence is related to pre-operative dissemination and to failure to identify tumoral prolongations in the parotid gland. Recurrence is generally multifocal. In 2 cases, carcinoma developed in association with P.A. Treatment of benign recurrence is surgical: it consists of totalizing the previous parotidectomy. If the previous parotidectomy has been total, tumorectomy is carried out. In all cases, surgery preserves the facial nerve trunk.

Adult

[Adult type rhabdomyoma of the soft palate: pathology of 2 cases with review of the medical literature].

In this paper, the authors report two cases of adult type rhabdomyomas of the soft palate. This tumor is composed of striated muscle cells containing striations and intracytoplasmic rod-shaped inclusions. In specimens already fixed in aqueous fixators (formalin...), some tumor cells presented a fixation artefact and displayed the morphology of so-called "spiderweb cells". Adult type rhabdomyoma is a real tumor. Total surgical removal is sufficient to cure the patient of this tumor in most cases.

[Therapeutic approach in chronic maxillary sinusitis. Apropos of 469 patients and 640 sinuses].

Between 1982 and 1987 we treated 469 patients who accounted for a total of 640 involved sinuses. These patients suffered from various stages of chronic sinusitis. Endoscopy of the maxillary sinuses enabled us to collect important information regarding the state of the mucosa and secretions, to collect bacteriological samples and to inspect the ostium and take pressure measurements. There was an 80% success rate in cases of sinusitis treated by drainage. In our opinion the indication for inferior meatotomy is as an extraction procedure and is not of value for aeration drainages. Nevertheless, out of a total of 141 inferior meatotomies performed, 80% remained patent after a mean follow up period of 3 years.

Chronic Disease

[Indications, technics and results of middle meatotomy. Apropos of 94 cases].

Middle meatotomies were essentially performed during the second period of our study from 1985 to 1987. The typical indication was for closed sinusitis, in order to allow aeration and satisfactory drainage. This was performed with an optical system at 30 degrees. The technique used was an anterior approach via total unciformectomy or a posterior approach using an Ostrum forceps. Good results, i.e. meatotomies which remained patent, were obtained in 80% of cases. The mean period of follow up was 3 years (6 months to 4 years). There were few complications overall for this form of endonasal surgery, though the possibility of damage to the lacrymal duct does exist. Synechiae were the most important functional complication and need to be prevented by meticulous post-operative care.

Drainage

[On the contribution of magnets in sequelae of facial paralysis. Preliminary clinical study].

This trial was designed to evaluate the efficacy of EPOREC 1 500 magnets as an adjuvant to rehabilitation following peripheral facial paralysis. Magnetotherapy is used in many other specialties, and in particular in rheumatology. The properties of repulsion between identical poles were used to decrease the effect of sequelae in the form of contractures on the facial muscles. There were two groups of 20 patients: one group with physiotherapy only and the other with standard rehabilitation together with the use of magnets. These 40 patients had facial paralysis of various origins (trauma, excision of acoustic neuroma, Bell's palsy etc). Obviously all patients had an intact nerve. It was at the time of the development of contractures that magnets could be used in terms of evaluation of their efficacy of action on syncinesiae, contractures and spasticity. Magnets were worn at night for a mean period of six months and results were assessed in terms of disappearance of eye-mouth syncinesiae, and in terms of normality of facial tone. Improvement and total recovery without sequelae were obtained far more frequently in the group which wore magnets, encouraging us to continue along these lines.

Facial Paralysis

[Usefulness of the laser in post-radiotherapy arytenoid edema].

Laser treatment of post-radiotherapy arytenoid edema was applied to two groups of patients with either laryngeal mobility or immobility. When mobility was normal the technic was simple, since only resection of the arytenoid edema was necessary and results were always excellent. In the second group, resection of arytenoid edema must be combined with partial or total resection of arytenoid, but in this case there is a risk of chondritis. Results in this series were excellent and only one case of post-laser radionecrosis was noted.

[Comparison of 2 molded autograft technics on intact ossicular chains].

Two techniques using molded autografts on intact ossicular chains were compared in 159 patients: some patients received molded autografts alone while in the others it was reinforced by a second fragment of temporal aponeurosis inserted behind the manubrium of the malleus. Fixation of these two grafts prevented ascension of the graft into the canal. Results obtained showed slight superiority for the double graft over the single one.

Adolescent

[Clear cell ductal adenocarcinoma or epithelio-myoepithelial carcinoma of the salivary glands. Apropos of 2 cases with review of the literature].

The authors reported two cases of epithelial myoepithelial carcinoma or clear cell adenocarcinoma of salivary intercalated duct with medical literature review. This rare tumor discovered in adults and old patients is frequently localized in parotid gland. Histologically it is composed of two types of cells: epithelial cylindrical cells bordering ductal lumen, clear myoepithelial cells rich in glycogen. This tumor is particular for its local recurrencies but is capable of metastasizing. Its prognosis is uncertain and its treatment is not yet codified.

Adenocarcinoma

[Bilateral parotid gland tumors. Report on 3 cases (author's transl)].

Three cases of rarely observed bilateral parotid gland tumors are reported. In two cases the tumors were of different histological types (mixed tumor on one side and an adenocarcinoma on the other side) while in the third case they were both of the mixed variety. No clear explanation for the possible bilateral nature of these tumors was found, diagnosis being made either during the initial examination or after an interval of several years.

Adenocarcinoma

[Homografts of the tympano-ossicular systems. Technic of removal and preservation].

The use of homografts appears to be a promising method of repairing the sequelae of chronic otitis. The authors describe their method of removing the temporal, dissecting the tympano-ossicular system and labelling and preserving the grafts. A homograft bank can be set up and this is a very necessary preliminary in the regular application of this type of surgery.
