Endoscopic diagnosis and treatment of Mirizzi's syndrome.

Mirizzi's syndrome is the name given to common bile duct obstruction secondary to a stone in the cystic duct. The cause of the biliary obstruction is often difficult to establish before operation. We report two cases of Mirizzi's syndrome, diagnosed endoscopically and treated without surgery. One of the patients was treated by drainage of both the common bile duct and the gallbladder associated with monooctanoin dissolution of the gallstone. The other was treated by common bile duct stenting.

Serum trypsin-like activity in chronic alcoholized men: possible relationship with lipids, apoA-1 and apoB lipoproteins.

Chronic alcoholization is known to increase plasma trypsin levels. One-hundred and forty-six male chronic alcohol users were tested for serum trypsin-like activity (STA), total cholesterol (TC), LDL-cholesterol (LDL-C), HDL-/cholesterol (HDL-C), triglycerides (TG), apoA-1 and apoB lipoproteins. STA was positively correlated to LDL-C, TG and apoB rates and the CT/HDL-C index and negatively correlated to HDL-C and apoA-1 rates and the apoA-1/apoB index. Eighty-four patients with high STA (group B) compared to 62 patients with normal STA (group A) showed significantly higher LDL-C, TG, apoB rates and TC/HDL-C index contrasting with significantly lower HDL-C and apoA-1 rates and the apoA-1/apoB index. The two groups were matched for age, overweight, cigarette smoking and glycemia. Hepatic dysfunction does not explain the differences in the lipoproteic parameters. Such results would suggest that there may be a tryptic alteration of apoproteins in vivo as already demonstrated in vitro and experimentally suspected in vivo in some other studies. Competition by the trypsin-activated alpha 2 macroglobulin for the chylomicron-remnant LDL receptor-related protein may be evoked.

Alcoholism

[Hepatoid adenocarcinoma of the stomach].

One year after gastric resection for cancer, a 67 year old patient was hospitalized because of a large hepatic tumor with extremely high serum alpha-foetoprotein levels (13,245 ng/ml). Histological and immunohistochemical studies of the gastric tumor revealed hepatoid foci with alpha-foetoprotein and protease inhibitor-producing cells. Histopathological and immunohistochemical features of this and the 8 previously reported cases of hepatoid adenocarcinoma of the stomach are analyzed.

Adenocarcinoma

[Increased fasting gallbladder volume: a new ultrasonic sign of cirrhosis?].

Ultrasound was used to calculate fasting gallbladder volume in three groups of patients: 90 with cirrhosis (alcoholic in 75 cases), 41 with non cirrhotic liver disease (alcoholic in 14 cases), and 38 controls. Gallbladder volume was evaluated according to sex, age, alcoholism, presence of gallstones, time of diagnosis, and biological tests of hepatocellular function. Mean fasting gallbladder volume was significantly higher in cirrhotic patients (45.89 +/- 32.65 ml, m +/- 1 SD) than in patients with non cirrhotic liver disease (25.31 +/- 14.08 ml) and in control subjects (21.28 +/- 10.30 ml) (P less than 0.001), but there was a great overlap between individual results in each group. No relationship was found between gallbladder volume and all clinical and biological tested parameters, except for decreased prothrombin time (P less than 0.02). Further studies are necessary to consider this ultrasound sign as an useful diagnostic tool in cirrhosis.

[Nutrition hygiene centers and prevention of alcoholization and alcoholism: growth or decline].

Initially conceived for detection and early treatment (secondary prevention) of alcoholization and alcoholism by means of a clinical method Le Go's scale of biological markers of dangerous alcoholization and of an intervention method "The Alimentary Hygiene Centers" have deviated to a purpose assembling in theory prevention and care resulting in a new denomination "Alimentary Hygiene and Alcohology Center". In fact, this double purpose leads to a decrease in secondary prevention. This is unfortunate since the results are better when detection and treatment are done before alcohol dependence or visceral lesion appear. There is here a true problem of public health.

Chlamydia trachomatis peritonitis: another cause of protein-rich lymphocytic ascites.

Ascites as a main feature of Chlamydia trachomatis infection in two young women was successfully treated with doxycycline. In both patients, ascitic fluid protein levels and lymphocyte counts were very high. This report, as well as six previously published cases, suggest that ascitic fluid culture for C. trachomatis and blood antibody determinations should be performed in all cases of protein-rich lymphocytic ascites in sexually active women.

Splenosis: ultrasound and CT findings in a case complicated by an intraperitoneal implant traumatic hematoma.

A case of intraperitoneal splenosis, found incidentally at ultrasound examination in a 35-year-old man, is reported. The diagnosis, suspected on the basis of computed tomographic (CT) and radionuclide studies, was established by surgery. Three years later, after direct trauma, ultrasound and CT studies revealed a surgically proven splenosis implant hematoma. The contribution of radiologic examinations in the diagnosis of splenosis is emphasized.

Determination of ascorbic acid in beers by high-performance liquid chromatography with electrochemical detection.

A reversed-phase high-performance liquid chromatographic method with electrochemical detection was developed for the determination of ascorbic acid in beers at the nanogram level. The column was packed with C18 bonded silica, commercially available or prepared in the laboratory. The eluent was a citrate buffer solution with an ion-pairing reagent. Several fatty amines were tested as such reagents. Good recoveries indicated that no interfering substances were eluted together with ascorbic acid. The main advantages are the rapid and simple preparation of the samples and the specificity and sensitivity of the detector, which are superior to those of any other detection systems proposed for the determination of ascorbic acid in beers.

Pseudomonas aeruginosa liver abscesses following endoscopic retrograde cholangiography. Report of a case without biliary tract disease.

We report a case of Pseudomonas aeruginosa liver abscesses following endoscopic retrograde cholangiopancreatography (ERCP) in a patient without evidence of biliary tract disease and of any known cause of hepatic infection. Computer tomography (CT) scan was the best method of diagnosis, allowing, through guided percutaneous puncture of the abscesses, isolation of the organism, which was sensitive to carbenicillin. One month of antibiotherapy with repeated aspirations of the largest abscesses was successful. This report suggests that ERCP may induce cholangitic sepsis by inoculating pathogens in the biliary tree even in the absence of extrahepatic obstruction.

[Immunoblastic lymphoma of the ileocecal region in chronic lymphoid leukemia. Richter's syndrome localized in the intestine and disclosed by ascites].

The authors report the case of a patient with chronic lymphocytic leukemia well controlled during five years, and then complicated with immunoblastic lymphoma of the ileo-caecal area. The tumor was revealed by a protein-rich peritoneal effusion containing numerous large undifferentiated lymphoid cells. Only five cases of Richter's syndrome, apparently localized to the digestive tract, have been published to date.

Ascites
