Hypoglycemic hemiplegia.

The usual signs and symptoms of hypoglycemia include tachycardia and profound diaphoresis. These will progress to include an altered mental status that can advance in severe cases to coma, seizures, and death. Occasionally, hypoglycemia may present with focal neurologic signs that can include hemiparesis or hemiplegia with preservation of mental status. In patients with the latter signs, the possibility of a cerebrovascular accident or other intracranial abnormality must be considered. The authors describe an elderly patient with hypoglycemic hemiplegia secondary to insulin administration. Their report includes observations on clinical presentation, the various mechanisms involved, and guidelines for the management of this syndrome.

Acquired Ondine's curse.

Acquired OC, although rare, can occur as a result of neurologic abnormalities, and its diagnosis is dependent on an understanding of all causes of altered ventilation, including sleep disturbances--central, obstructive, or mixed. A neurologic evaluation, appropriate radiologic studies, and sleep workup will in most cases delineate the problem. Treatment includes a tracheotomy and ventilator support. Phrenic nerve pacing is reserved for patients with long-term disease and viable phrenic nerves.

Hypoglossal nerve palsy after tooth extraction.

A rare complication involving hypoglossal nerve paralysis after extraction of third molars is reported. The possible pathophysiology of the condition is discussed. The paralysis was temporary and resolved within one week of surgery.
