[Utero-trophoblastic interface: transitory stage in human placentation].

In the past, the intervillous space of the human placenta has been studied on samples provided by spontaneous abortions. Using the endocavitary ultrasonography and the chorionoscopy in normal pregnancies, the intervillous space during the first trimester has been observed as filled by a plasmatic fraction cell free of the maternal blood. During the first trimester, the human placenta doesn't have its theoretical functional specificities.

Monosomy 11q: report of two familial cases and review of the literature.

We present four children from two families with the typical 11q- phenotype resulting from an unbalanced segregation of a parental translocation. In the first family, the father had a 46,XY,t(5;11)(q24;q23.3) constitution. The father of the three other children had a 46,XY,t(11;17)(q23;p13) translocation. Despite associated partial deletion, three of the children had a typical 11q- phenotype. The fourth one, whose pregnancy was terminated in the second trimester, had a hypoplastic left heart but no other considered gross anomalies. A review of 36 previous cases, including 5 due to translocations (4 familial rearrangements, and 1 of unknown origin) is given with emphasis on the relationships between break-points and phenotype. Undescribed manifestations in our patients include agenesis of corpus callosum adactyly and malrotation of the gut.

Purification, expansion, and multiple fluorochrome labeling of cord blood hemopoietic precursors: preliminary results.

CD34-positive cells were isolated from a total of 23 cords using CellPro Ceprate columns. AIS MicroCellector flasks, and panning. The cells were (1) expanded in serum-free culture supplemented with a variety of combinations of cytokines and (2) immunophenotyped using multiple fluorochrome labeling. The results indicated that the avidin column produced the highest purity of CD34-positive cells, and that immature blast cells could be expanded in serum-free culture. Preliminary results suggested that the four fluorochrome labeling technique may provide useful information on the lineage commitment of cord blood precursor and blast cells.

Protein and steroid levels in embryonic cavities in early human pregnancy.

Biochemical analysis including concentrations of urea, creatinine, human chorionic gonadotrophin (HCG), oestradiol, progesterone, and alpha-fetoprotein (AFP), two-dimensional gel electrophoresis, and the affinity of AFP for Concanavalin A (Con A)-Sepharose were performed on samples of exocoelomic and amniotic fluids retrieved by transvaginal puncture and maternal serum from 25 normal pregnancies between 5 and 12 weeks of gestation. Biochemical assays showed that during this period of gestation no differences in urea concentration were found between fluids from the three compartments, whereas creatinine concentration decreased significantly (P < 0.001) from maternal serum to amniotic fluid. The exocoelomic fluid contained significantly (P < 0.001) higher concentrations of oestradiol, progesterone and HCG than both maternal serum and amniotic fluid. AFP concentration was similar in amniotic and exocoelomic fluids and significantly (P < 0.001) lower in maternal serum. Between the second and the third months of gestation, urea concentration decreased significantly (P < 0.05) and oestradiol, HCG and AFP increased significantly in maternal serum (P < 0.05, P < 0.05, P < 0.001, respectively). During the same period of gestation, exocoelomic fluid concentrations of urea and HCG decreased significantly (P < 0.005, P < 0.001, respectively). Comparison of the two-dimensional gel patterns obtained from maternal serum with those from exocoelomic amniotic fluids revealed no significant qualitative differences, except for several small proteins. These results suggest that protein pathways across materno-embryonic membranes are not simply passive transfers.(ABSTRACT TRUNCATED AT 250 WORDS)

The human placenta becomes haemochorial at the 13th week of pregnancy.

Histological specimens of recent implantation sites are the basis of our current concept on human embryo implantation and placental development. In the Carnegie Collection maternal red blood cells were detected early in the primitive intervillous space (10th-12th day after conception). These cells were localized to the trophoblastic lacunae and originated from distended peripheral maternal sinusoids (Kaufmann, 1981). The classical theory states that progressively more and more maternal vessels are tapped. A true maternal blood flow is established around the 29th day. Dynamic investigations of human placental development in vivo are scarce and hampered by ethical considerations and the absolute requirement to refrain from using non aggressive and potentially harmful techniques. Despite these limitations such studies provide new insights that surprisingly contradict our previously and seemingly definitely established knowledge of the early phases of placental vascularization, and lead us to conclude that there is an absence of maternal blood circulation in the intervillous placental space (IVS) during the 12 first weeks of human pregnancy.

Prenatal diagnosis of cystic hygroma and chorioangioma in the Wolf-Hirschhorn syndrome.

We report the prenatal diagnosis of Wolf-Hirschhorn syndrome (4p-) in a 24-week-old fetus. Echographic features included cystic hygroma, a complex heart defect with right ventricular hypoplasia, and a large placental chorioangioma. We suggest that chorioangioma may be associated with chromosomal imbalance and that systematic careful morphologic examination of the fetus and karyotyping of any pregnancy in which large chorioangioma is detected is advisable. Jugular lymphatic obstruction sequence has not been reported so far in association with 4p-syndrome.

Mosaicism of 46,XX/47,XX,+9/47,XX,+?mar in the same amniotic fluid with apparent loss of one cell line after delivery.

A 46,XX;47,XX,+9;47,XX,+?mar karyotype was detected in an amniotic fluid cell culture and confirmed in a subsequent fetal blood sample from a 40-year-old woman. After termination of the pregnancy, none of the 186 mitoses obtained from a second blood sample was trisomic for chromosome 9 (p less than 0.001). Selection against cells containing trisomy 9 is postulated to explain the disappearance of the lymphocyte clone.

Pulsed Doppler on a vaginal probe. Necessity, convenience, or luxury?

One hundred twenty-seven pregnancies were studied between 5 and 12 weeks of amenorrhea with Doppler ultrasound coupled with a vaginal probe. These biologically proven pregnancies had no clinical sign of abnormality; however, 29 were detected not to be evolving. Doppler signals were obtained from the embryonic heart, umbilical cord, trophoblastic crown, peritrophoblastic area, and from the uterine and ovarian arteries. In normally evolving pregnancies, no intratrophoblastic flow was detected, whereas it was present in all missed abortions. The peritrophoblastic area always generates a Doppler signal with an important diastolic component. The resistance index (RI) of uterine arteries was constant during the first trimester of pregnancy (RI = .77 +/- .15) and a protodiastolic notch was found in 58% of the cases. The combined use of these two modalities of examination was demonstrated. Although the clinical efficacy of the method has not yet been demonstrated, this combination of techniques opens new perspectives to echography and adds a dynamic dimension to morphologic studies.

Echographic [corrected] and anatomic studies of the maternotrophoblastic border during the first trimester of pregnancy.

The initiation of uteroplacental circulation was investigated in early pregnancy with the following material: contact ultrasonography, 250 cases; direct-vision chorionic biopsy, 10 cases; products from voluntary interruption of pregnancy, 60 cases; and hysterectomy with the pregnancy in situ, three cases. Using all of these techniques, we were unable to demonstrate a true intervillous blood flow during the first 12 weeks. We suggest that during this period the intervillous space is bathed by an acellular fluid that could be plasma filtered by the trophoblastic shell and its endovascular cone. If this is the case, the physiology of early pregnancy must be reconsidered.

[Ovarian function under mini-dose oral contraception. Echographic morphology of the ovaries and hormonology].

The present study correlates ultrasound observations of ovaries with gonadotrophin and ovarian steroid levels in plasma in 18 women using oral contraception through 28 cycles. Follicles could often be made out with ultrasound. They were associated with measurable gonadotrophin values, but the resulting ovarian steroid production often seemed to be very low. However, there are exceptions as shown in one case described here in which ultrasound, gonadotrophin and oestrogen levels together show up coherently. Yet no ovulation was demonstrated. This preliminary study should encourage investigators to carry out further series.
