Effect of disease stage on clinical outcome after syngeneic bone marrow transplantation for relapsing experimental autoimmune encephalomyelitis.

Relapsing experimental autoimmune encephalomyelitis (R-EAE) is an immune-mediated demyelinating central nervous system (CNS) disease. Myeloablation and syngeneic bone marrow transplantation (SBMT), when performed at the peak of acute disease (day 14), prevented glial scarring and ameliorated the disease severity. In contrast, when syngeneic BMT was performed late in chronic phase (day 78), significant glial scarring remained and the clinical severity did not differ significantly from that of the controls. After SBMT in either the acute or chronic phase of disease, the posttransplant immune system remained responsive to myelin epitopes as determined by in vitro proliferation and interferon-gamma (IFN-gamma) production. However, in mice undergoing SBMT, in vivo delayed-type hypersensitivity (DTH) responses were significantly decreased while IFN-gamma RNA levels and inflammatory infiltrates within the CNS were slightly improved. We conclude that failure of SBMT to improve the clinical disease when performed in chronic phase may be due to preexisting glial scarring. We also conclude that in the absence of glial scarring and irreversible neuronal injury, in vivo DTH responses and histology are better predictors of clinical improvement than in vitro proliferation or IFN-gamma cytokine production.

Effects of hyperthermia on contraction and dilatation of rabbit femoral arteries.

To analyze the effect of hyperthermia on the vascular response, the isometric response of isolated rabbit femoral artery segments was recorded at 37 degreesC and hyperthermia (41 and 44 degreesC). Contraction to potassium (5 x 10(-3)-5 x 10(-2) M) was significantly greater at 41 and 44 than at 37 degreesC and increased by inhibition of nitric oxide (NO) synthesis with Nomega-nitro-L-arginine (L-NNA; 10(-4) M) or endothelium removal at 37 degreesC but not at 41 or 44 degreesC. Norepinephrine (10(-9)-10(-4) M) produced a concentration-dependent contraction greater at 41 or 44 than at 37 degreesC and not modified by endothelium removal or L-NNA at either temperature. Phenylephrine (10(-9)-10(-4) M) produced a contraction increased by warming to 44 degreesC but not to 41 degreesC. The specific alpha2-adrenoceptor agonist BHT-920 produced a weak contraction, reduced by the alpha1-adrenoceptor antagonist prazosin (10(-6) M) and increased at 44 degreesC but not at 41 degreesC. The concentration-dependent contraction to endothelin-1 (ET-1; 10(-11)-10(-7) M) was increased by warming to 41 and 44 degreesC and by endothelium removal or L-NNA at 37 degreesC but not at 41 or 44 degreesC. Response to ET-1 was reduced by endothelin ETA-receptor antagonist BQ-123 (10(-5) M) and ETB-receptor antagonist BQ-788 (10(-5) M). In arteries precontracted with ET-1 (10(-8)-3 x 10(-8) M), relaxation to sodium nitroprusside (10(-8)-10(-4) M) was increased at 41 and 44 degreesC vs. at 37 degreesC, but that of ACh (10(-8)-10(-4) M) or adenosine (10(-8)-10(-4) M) was not different at all temperatures studied. Relaxation to ACh, but not adenosine, was reduced similarly by L-NNA at all temperatures studied. These results suggest hyperthermia in muscular arteries may inhibit production of, and increase dilatation to, NO, resulting in unchanged relaxation to ACh and increased constriction to KCl and ET-1, and may increase constriction to stimulation of alpha1-adrenoceptors by NO-independent mechanisms.

[Reliability of the presurgical anatomopathologic diagnosis in primary bronchogenic carcinoma].

BACKGROUND: The specific cell type in primary lung cancer (LC) has important consequences, both for treatment and prognosis. Our objective has been to evaluate the degree of accuracy of several preoperative techniques in the LC diagnosis. PATIENTS AND METHODS: We have studied 442 diagnostic techniques realized in 360 patients who underwent thoracotomy for LC in our hospital (1988-1994). Twelve sputum cytologies, 93 transthoracic needle biopsies (TNB), 73 bronchial aspirated cytologies, 30 bronchial brushing, 205 bronchial biopsies (BB) and 29 transbronchial biopsies were included. In each case the cellular type of LC, obtained through these techniques, was compared with the result derived from the study of surgical piece (reference diagnosis). To calculate the degree of concordance between both diagnoses we used the kappa coefficient (K). RESULTS: The overall concordance between the cellular type observed in the different preoperative techniques and the definitive result obtained by thoracotomy was 0.61. The worst test was TNB (K = 0.41). Sputum cytology and BB presented a good histopathological precision (K = 0.75 and 0.70 respectively). The rest of techniques showed moderate results. On the different histologies, the best result was obtained in squamous carcinoma (K = 0.68) and the worst in undifferentiated large cell carcinoma (LCC) (K = 0.39). Small-cell lung cancer only showed a moderate concordance (K = 0.58). The coexistence and coincidence of two or more tests with the same cellular type were associated with a higher proportion of accuracy (0.97 vs 0.73; p = 0.00002; odds ratio: 12.02). CONCLUSIONS: The preoperative histopathological diagnoses should be interpreted with caution, especially those obtained by TNB and those where it is implied the LCC. In these two circumstances and in cases in which the knowledge of the cellular type have relevance, we think that the initial diagnosis should be reinforced with a second result. The precision of the preoperative diagnoses in small-cell lung cancer must be reevaluated.

Surgical results and prognostic factors in early non-small cell lung cancer.

BACKGROUND: We attempted to clarify the prognostic value of tumor size (maximum, 3 cm), the evidence of invasion proximal to a lobar bronchus at least 2 cm distal to the carina, and the absence or presence of visceral pleura invasion in patients with completely resected non-small cell lung carcinoma without lymph node invasion or satellite lesions (T1 N0 M0, T2 N0 M0). METHODS: The study included 158 patients. Four patients were excluded due to postoperative mortality (2.5%). The variables selected for the survival study were sex, age, symptoms presence or absence, bronchial invasion level (evidence or not of invasion proximal to a lobar bronchus at least 2 cm distal to the carina), pulmonary location, pneumonectomy or lesser resection, cell type, squamous or nonsquamous, tumor size, invasion or not of the visceral pleura, and T1 or T2 status. RESULTS: The overall survival rate in this series was 74% at 5 years and 60% at 10 years. Only the tumor size had a significant influence on survival (p = 0.0092). Patients with a tumor less than 2 cm in diameter did better (p = 0.0023). CONCLUSIONS: These observations suggest that it will be necessary to further research in clarifying the prognostic value of the bronchial invasion level and of the degree of the visceral pleura invasion and its implications when classifying a tumor as T1 or T2.

Peptidergic modulation of the sympathetic contraction in the rabbit ear artery: effects of temperature.

1. The effects of neuropeptide Y, endothelin-1, arginine-vasopressin and angiotensin II on the vascular contraction to sympathetic nerve stimulation were studied in isolated segments, 2 mm long, from the rabbit central ear artery, a cutaneous vessel, during changes in temperature (24 degrees -41 degrees C). 2. Transmural electrical stimulation (1-8 Hz, at supramaximal voltage) produced frequency-dependent contraction, and this response, partially blocked by tetrodotoxin (1 microM) and phentolamine (1 microM), was reduced by cooling (30 degrees C -24 degrees C) and was not modified by warming (41 degrees C), as compared to that recorded at 37 degrees C. 3. Pretreatment with neuropeptide Y (10, 30 and 100 nM) increased in a concentration-dependent manner the vascular contraction to sympathetic stimulation at every temperature studied, but this potentiation was greater during cooling (34 degrees C -24 degrees C) than at 37 degrees C or warming (41 degrees C). 4. Pretreatment with endothelin-1 (3 and 10 nM) or vasopressin (0.1, 0.3 and 1 nM) increased in a concentration-dependent manner the vascular contraction to sympathetic stimulation during cooling (34 degrees C -24 degrees C), but not at 37 degrees C or warming (41 degrees C). 5. Pretreatment with angiotensin II (0.1, 0.3 and 1 microM) did not modify the contraction to sympathetic stimulation at any temperature studied. 6. These results suggest that neuropeptide Y, endothelin-1 and vasopressin, but not angiotensin II, modulate the cutaneous vasoconstriction to sympathetic nerve stimulation by potentiating this vasoconstriction during cooling.

Role of endothelin receptors, calcium and nitric oxide in the potentiation by endothelin-1 of the sympathetic contraction of rabbit ear artery during cooling.

1. To examine further the potentiation by endothelin-1 on the vascular response to sympathetic stimulation, we studied the isometric response of isolated segments, 2 mm long, from the rabbit central ear artery to electrical field stimulation (1-8 Hz), under different conditions, at 37 degrees C and during cooling (30 degrees C). 2. Electrical stimulation produced frequency-dependent contraction, which was reduced (about 63% for 8 Hz) during cooling. At 30 degrees C, but not at 37 degrees C, endothelin-1 (1, 3 and 10 nM) potentiated the contraction to electrical stimulation in a dose-dependent way (from 43 +/- 7% to 190 +/- 25% for 8 Hz). 3. This potentiation by endothelin-1 was reduced by the antagonist for endothelin ETA receptors BQ-123 (10 microM) but not by the antagonist for endothelin ETB receptors BQ-788 (10 microM). The agonist for endothelin ETB receptors IRL-1620 (0.1 microM) did not modify the contraction to electrical stimulation. 4. The blocker of L-type Ca2+ channels verapamil (10 microM l-1) reduced (about 72% for 8 Hz) and the unspecific blocker of Ca(2+)-channels NiCl2 (1 mM) practically abolished (about 98%), the potentiating effects of endothelin-1 found at 30 degrees C. 5. Inhibition of nitric oxide synthesis with NG-nitro-L-arginine (L-NOARG, 0.1 mM) increased the contraction to electrical stimulation at 30 degrees C more than at 37 degrees C (for 8 Hz, this increment was 297 +/- 118% at 30 degrees C, and 66 +/- 15% at 37 degrees C). Endothelium removal increased the contraction to electrical stimulation at 30 degrees C (about 91% for 8 Hz) but not at 37 degrees C. Both L-NOARG and endothelium removal abolished the potentiating effects of endothelin-1 on the response to electrical stimulation found at 30 degrees C. 6. These results in the rabbit ear artery suggest that during cooling, endothelin-1 potentiates the contraction to sympathetic stimulation, which could be mediated at least in part by increasing Ca2+ entry after activation of endothelin ETA receptors. This potentiating effect of endothelin-1 may require the presence of an inhibitory tone due to endothelial nitric oxide.

Role of the purinergic and noradrenergic components in the potentiation by endothelin-1 of the sympathetic contraction of the rabbit central ear artery during cooling.

1. To examine the role of the purinergic and noradrenergic components in the potentiation of endothelin-1 on the vascular response to sympathetic nerve stimulation, we recorded the isometric response of isolated segments, 2 mm long, from the rabbit central ear artery to electrical field stimulation (1-8 Hz) under different conditions, at 37 degrees C during cooling (30 degrees C). 2. Electrical field stimulation produced frequency-dependent contraction, which was reduced during cooling (about 60% for 8 Hz). Both at 37 degrees C and 30 degrees C, phentolamine (1 microM) or blockade of alpha(1)-adrenoceptors with prazosin (1 microM) reduced, whereas blockade of alpha(2)-adrenoceptors with yohimbine (1 microM) increased, the contraction to electrical field stimulation. This contraction was increased after desensitization of P2-receptors with alpha, beta-methylene adenosine 5'-triphosphate (alpha, beta-meATP, 3 microM) at 37 degrees C but not at 30 degrees C, and was not modified by blockade of P2-receptors with pyridoxalphosphate-6-azophenyl-2,4'-disulphonic acid (PPADS, 30 microM) at either temperature. 3. Endothelin-1 (1, 3 and 10 nM) at 37 degrees C did not affect, but at 30 degrees C it potentiated in a concentration-dependent manner the contraction to electrical field stimulation (from 28 +/- 6 to 134 +/- 22%, for 8 Hz). At 37 degrees C, endothelin-1 in the presence of phentolamine or prazosin, but not in that of yohimbine, alpha, beta-meATP or PPADS, potentiated the contraction to electrical stimulation. At 30 degrees C, phentolamine or yohimbine reduced, prazosin did not modify and alpha, beta-meATP slightly increased the potentiation by endothelin-1 of the response to electrical stimulation. 4. The arterial contraction to ATP (2 mM) and the alpha(2)-adrenoceptor agonist BHT-920 (10 microM), but not that to (-)-noradrenaline (1 microM), was potentiated by endothelin-1 at both 37 degrees C and 30 degrees C. 5. These results in the rabbit central ear artery suggest that the sympathetic response: (a) at 37 degrees C, could be mediated mainly by activation of alpha(1)-adrenoceptors, with low participation of P2-receptors, (b) is diminished during cooling, probably by a reduction in the participation of alpha(1)-adrenoceptors, and in this condition the response could be mediated in part by P2-receptors, and (c) is potentiated by endothelin-1 during cooling, probably by increasing the response of both postjunctional alpha(2)-adrenoceptors and P2-receptors.

Transmission of human herpesvirus 7 through multigenerational families in the same household.

BACKGROUND: Human herpesvirus 7 (HHV-7) closely resembles HHV-6 and to a lesser degree cytomegalovirus. HHV-7 infection is usually acquired during early childhood. Primary infection can cause a roseola-like illness but in most cases it is only mildly symptomatic. The majority of adults are seropositive and in contrast to HHV-6 and cytomegalovirus infection, they continue to secrete the virus in their saliva for many years. The mode of intrafamilial transmission of this virus is not well-understood. METHODS: Saliva samples for virus isolation and DNA restriction analysis were obtained from all 47 members of 6 Japanese families, including 4 families with 3 generations living in the same household. RESULTS: HHV-7 was isolated from 43 of 47 saliva samples collected from children and adult members of the 6 families (91.5%). In one family the restriction patterns of the maternal grandmother, the mother and the children were similar, and the patterns of the paternal grandmother and the father were similar. In another family the patterns of the father and 5 of 6 children were similar, and those of the mother and the other child were similar. Altogether similar HHV-7 restriction profiles with his or her mother were found in 48% of offspring, and similar profiles with his or her father were found in 28% of offspring. CONCLUSIONS: The results strongly suggested horizontal transmission of HHV-7 from grandparents to parents to children through close contact within a household. Either parent could transmit HHV-7 to the children.

In vitro selection of variants of herpes simplex virus type 1 which differ in cytopathic changes.

To analyze the mechanisms for in vitro emergence of the syncytial variants of herpes simplex virus type 1 (HSV-1), several cell lines were infected with a mixture of equal amounts of two HSV-1 variants, one syncytial and the other non-syncytial, and changes in their relative abundance were monitored during passage. With a combination of two variants of the Miyama strain of HSV-1, the syncytial variant became dominant during passage in Vero, RK-13 and FL cells. On the other hand, the ratios of the two variants remained around 1:1 during the passage in HEp-2, MGC and HEL cells. In another set of variants of the SKO strain of HSV-1, the outcomes were different from those of the Miyama strain in the FL, MGC and HEp-2 cells. The ratios of the two variants remained around 1:1 during passage in FL cells, while the non-syncytial variant became dominant during passage in MGC and HEp-2 cells. In addition, we examined the effects of a complement and interferon-beta (IFN-beta) on the outcome of the selection. As a result, the complement slowed the selection of a syncytial variant, whereas IFN-beta facilitated it.

CD40 mediated activation of gingival and periodontal ligament fibroblasts.

CD40 is a 50 kDa transmembrane protein important for regulating B lymphocyte proliferation and differentiation. This novel activation antigen is primarily expressed by hematopoietic cells including B lymphocytes, follicular dendritic cells, and monocytes. Recently, human fibroblasts from a variety of tissues were shown to display CD40; however, its function was unknown. Cellular responses mediated by CD40 are naturally triggered by its counter-receptor, the CD40 ligand, which is displayed on activated T cells, mast cells, eosinophils, basophils, and B lineage cells. This study investigated the functional significance of CD40 expression on periodontal fibroblasts, in the context of periodontal inflammation. The experiments reported herein demonstrate constitutive CD40 expression on cultured periodontal ligament (PDL) and gingival fibroblasts. Interestingly, cells of gingival origin displayed up to 13-fold higher constitutive levels of CD40, versus fibroblasts from PDL. Interferon gamma (IFN gamma) treatment enhanced CD40 expression on PDL and gingival fibroblasts, with up to 61-fold induction of expression. Immunohistochemical staining was used to detect CD40 on fibroblastic cells in both normal and acutely inflamed gingival tissue. Expression of CD40 in inflamed tissue was significantly higher than in uninflamed tissue. Western blot analysis of anti-CD40 triggered cells revealed the induction of tyrosine phosphorylation on a 50 kDa protein in PDL and gingival fibroblasts. These results indicate that CD40 is an active signaling conduit in periodontal fibroblasts. This concept was further substantiated by the fact that CD40 engagement stimulated interleukin 6 (IL-6) production by gingival fibroblasts, but not periodontal ligament fibroblasts. Overall, these results demonstrate that CD40 on periodontal fibroblasts may functionally interact with CD40L-expressing cells. This CD40/CD40L interaction can stimulate fibroblast activation and synthesis of the proinflammatory cytokine IL-6.

Infection of a human retinal pigment epithelial cell line with human herpesvirus 6 variant A.

A retinal pigment epithelial (RPE) cell line (K-1034) was examined for its susceptibility to human herpesvirus 6 variant A (HHV-6A). Exposure of K-1034 cells to HHV-6A induced the formation of multinucleated giant cells, which was suppressed by an inhibitor of viral DNA synthesis. In the giant cells, herpesvirus nucleocapsids were demonstrated by electron microscopy and the viral glycoprotein B was detected by immunofluorescence assay. These results indicate that K-1034 cells are susceptible to HHV-6A and suggest that HHV-6A has an ability to directly destroy epithelial cells.

The effect of diazepan and exercise training on selected biochemical and histochemical properties of rat skeletal muscle.

The effects of chronic diazepam (D) treatment and exercise training on total body mass (TBM), microsomal protein yield (MPY), calcium uptake by fragmented sarcoplasmic reticulum (SR), muscle fibre cross-sectional area, and both PFK and SDH activities were investigated in the tibialis anterior (TA), soleus (Sol), and plantaris (Plt) muscles of 50 male albino Sprague-Dawley rats. Rats were assigned randomly to control (C), sprint-trained (S), or endurance-trained (E) groups. Training was of 12 weeks duration. One-half of each group received daily intraperitoneally D doses of 5 mg kg-1 of TBM. Exercise reduced TBM (p < 0.05); increased the relative BM of the TA (E = 2.02 +/- 0.02, p < 0.01) and Plt (E = 1.15 +/- 0.02, p < 0.01; S = 1.13 +/- 0.03, p < 0.01), as well as the Ca++ uptake of the Sol SR (C = 0.08 +/- 0.02, E = 0.16 +/- 01, p < 0.05). MPY was elevated in S-Sol (C = 1.12 +/- 0.6, S = 1.52 +/- 0.1, p < 0.01). D elevated Sol MPY as well as TA PFK. S-trained animals had lower mean fibre areas than the E-trained (D-treated and untreated) animals. The elevated relative masses of TA and Plt are explained by a decreased TBM with exercise. The increased Ca++ uptake of the Sol indicates that E enhances this function, and the increased MPY probably implies an increased SR. The D could be responsible for the D-elevated Sol MPY as well as the TA PFK. El D did not reduce neuromuscular activity to a level adversely affecting oxidative enzyme activity, but in the case of PFK activity in the TA muscle, such a reduction was evident.

Electron microscopic observations of aberrant capsids of pseudorabies virus.

Interactions between four strains of pseudorabies virus (PrV) and seven kinds of cell lines were examined. Three kinds of cells (SKL, CPK and PK-15) were especially infected with PrV at an MOI of about 10 PFC/cell. At sequential intervals after infection, cells of these types were collected for electron microscopic observations and the infectious doses of culture fluids were assayed. Developmental features of PrV were found to be very similar to those of herpes simplex virus (HSV), with the slight difference that PrV developed a little earlier and more vigorously than did HSV. At the late stage of infection, aberrant capsids of PrV were observed frequently in the nucleus of SKL but rarely in the nuclei of CPK and PK-15. The titer of infectious virus produced by SKL was much larger than that produced by CPK and PK-15. Immuno-electron microscopic examination using a monoclonal antibody against the major capsid protein of PrV clearly demonstrated that the complete and aberrant capsids have a common epitope. The mechanism of the formation of aberrant capsids is discussed.

Epidermal growth factor in human breast cancer, endometrial carcinoma and lung cancer. Its relationship to epidermal growth factor receptor, estradiol receptor and tumor TNM.

Epidermal growth factor (EGF) and its receptor (EGFR) were measured in 60 breast cancers (BC), 6 benign mammary tumors (BM), 8 samples of normal breast (NB), 6 endometrial carcinomas (EC) and 30 lung cancers (LC). EGF was measured in plasma, saliva and urine from 20 patients with BC, before and after tumor excision, and in 8 patients with metastatic disease. The median EGF in BM and BC was significantly higher (P < 0.05) than in NB. No significant correlation between EGF and EGFR was found in BC. Neither tumor excision nor the spreading of the disease significantly modified the EGF concentrations in biological fluids. In LC there was an inverse relationship between EGF and EGFR (rs = -0.36; P = 0.09), which disappeared in normal lung. It is concluded that EGF may play a role in malignant transformation; however, the weak correlation between EGF and EGFR lessens the importance of EGF in either autocrine or paracrine stimulation of tumor growth.

Peripheral nerve regeneration across 14-mm gaps: a comparison of autograft and entubulation repair methods in the rat.

A study was conducted to compare the regeneration across 1.4-cm peroneal nerve gaps in rats, repaired with sutured autografts or with artificial nerve grafts. The artificial models were composed of a biodegradable passive conduit made of glycolide trimethylene carbonate, filled with either phosphate-buffered saline or a collagen extracellular matrix. Functional recovery was evaluated by walking track analysis throughout the experiment. After 9 months, the nerves were analyzed by electrophysiology and by qualitative and quantitative histology. Walking track analysis demonstrated the three repair methods to provide statistically equivalent recovery, except at day 195 post-engraftment, when the collagen-filled conduit was superior to the saline-filled conduit. Electrophysiologically, the autograft was superior to the collagen-filled conduit, while the collagen- and saline-filled conduits were equivalent. Quantitative histology demonstrated that normal intact nerve had larger mean myelinated axonal diameters but an equal number of axons to the three repair methods, and that the repair methods were statistically equivalent. While the repair methods had similar histologic and functional outcomes, combined standardized scoring demonstrated that the autograft was superior to the statistically-equivalent entubulation repairs. A collagen gel may serve as an ideal matrix in which to suspend neurotrop(h)ic factors or cells.

Blunt chest injuries in 1696 patients.

We review 1696 patients with blunt chest trauma. Road traffic accidents were the main cause of injury followed by domestic falls and labour accidents. Outdoor falls and sport accidents accounted for a small number of injuries. For clinical evaluation, Stoddart's score was used. The injuries were considered as minor in 710 patients, intermediate in 740 and severe in 246. Global in-hospital mortality was low (5%) but increased to 37% when only patients with multiple severe injuries were considered. Thoracic wall fractures were present in 1419 patients. Flail chest was diagnosed in 140 patients and pulmonary contusion in 275. Diaphragmatic rupture was present in 40 patients and tracheobronchial injury in 6. Cardiovascular injuries occurred in 55 patients. Associated extrathoracic injuries were seen in 611 patients: 923 patients were clinically observed and/or medically treated. An intercostal tube was inserted in 638 patients. Thoracotomy was undertaken in 105 patients. Surgical fixation for flail chest was carried out in 29 patients. The results were generally good: 9 patients did not need any mechanical ventilation and 11 were ventilated for a short period. No deaths were due to the surgical procedure. The authors maintain that a selective attitude restricting, but not ignoring, surgical stabilization is the best policy.
