Brain disconnection, schizophrenia, and paranoia.

A lateral deficit explanation of schizophrenic cognition maintains that a left hemisphere deficit is characteristic of nonparanoid schizophrenia whereas right hemispheric deficits may be common in paranoid schizophrenia. An alternate explanation postulates an interhemispheric deficit in schizophrenic functioning. A major piece of evidence for this position is Beaumont and Dimond's oft-cited experiment matching pairs of stimuli presented to left, right, and both hemispheres (Beaumont, J. G., and Dimond, S. J. Brain disconnection and schizophrenia. Br. J. Psychiatry, 123: 661-662, 1973). We expanded upon this study and found left hemisphere deficits and interhemisphere deficits in schizophrenics. However, the type of deficit seemed directly related to the distinction of a paranoid-nonparanoid schizophrenic diagnosis. Paranoids match stimuli in all hemisphere conditions as well as control groups. Nonparanoid schizophrenics, on the other hand, had their greatest problem on all stimuli in the left hemisphere. Their right hemisphere performance was not different from other groups. The nonparanoid also exhibited problems in interhemispheric matching, but this seemed due to an impaired left hemisphere. Results, therefore, did not support an interhemisphere transfer deficit. A discussion of prior work suggests that the nonparanoid schizophrenic has a problem in serial processing in the left hemisphere.

The dependence on heart rate of the human ventricular action potential duration.

For the human electrocardiogram, the relationship between QT interval and heart rate was examined in conditions where the heart rate was changed at different rates. The QT interval provides a measure of the ventricular action potential duration. For slow changes of heart rate, elicited by mild exercise, the dependence of QT interval on heart rate obtained when the heart rate was rising was the same as that obtained when the heart rate was falling. The QT interval was approximately proportional to 1/(heart rate)1/2. For the rapid transient changes of heart rate associated with respiration, the QT interval was approximately constant, independent of the instantaneous heart rate. For rapid sustained changes of heart rate, elicited by heavy exercise or immersion of the face in cold water, the changes in QT interval lagged behind the heart rate changes. Consequently the dependence of QT interval on heart rate obtained when the heart rate was increasing differed from the dependence obtained during a fall in heart rate. This time dependence of the relationship between QT interval and heart rate is similar to that seen for the relationship between action potential duration and stimulation rate in isolated pieces of mammalian ventricle.

Icon thresholds in paranoid and non-paranoid schizophrenics.

Recent work in information processing points to two processes being active when a stimulus produces a short-term memory trace, the icon. Previous work in schizophrenia seems to have blurred this important distinction and as a result the processing of the icon has been confused with the strength of icon. The present paper reviews current work with this distinction in mind and presents data on the icon strength of acute paranoid and non-paranoid schizophrenics, chronic schizophrenics, and two control groups as determined by the method of stimulus exposure time. Results indicate no difference in temporal recognition thresholds between schizophrenics and controls, strongly suggesting that the schizophrenia deficit resides in the encoding process.

Behavioural change for the prevention of oral disease in children: one-on-one programme.

A description is given of the efforts made in a typical dental practice to influence dental health behaviour in a positive manner. A consistent message has been given over the years involving adequate fluoride supplements, restriction in the frequency of sugar intake and thorough oral hygiene. In addition, emphasis is placed on constant repetition and the use of positive feedback.

Subclavian artery injury and fracture of the scapula.

A fracture of the scapula is frequently associated with other significant injuries. In the case described, a motorcycle accident victim sustained multiple injuries, including a scapular fracture associated with a tear of the subclavian artery, demonstrated by angiography and confirmed at operation. A literature review revealed no similar case.

Quantitative structure-activity relationships in 1-aryl-2-(alkylamino)ethanol antimalarials.

A quantitative structure-activity relationship has been formulated for 646 antimalarials acting against P. berghei in mice. The equation developed has 14 terms, 9 of which are indicator variables. The correlation coefficient for the QSAR is 0.898 and the standard deviation is 0.309. The antimalarials are all arylcarbinols of the type X-ArCHOHCH2NR1R2. Sixty different aryl structures, including a variety of heterocyles, are contained in the study. The most important determinate of activity is found to be the electron-withdrawing ability of the substituents X; the hydrophobic character of X and R plays less important roles. Suggestions for more potent analogues are made and the lack of activity of about 100 additional analogues is also considered.

Conditions for tracheal intubation following fazadinium and pancuronium.

Intubating conditions were studied in two groups of patients who received either fazadinium 1 mg/kg or pancuronium 0.1 mg/kg (group 1), or either fazadinium 0.5 mg/kg or pancuronium 0.08 mg/kg (group 2). In group 1 intubating conditions were studied at 30, 45, 60 and 75 s after injection of the relaxant drug, and in group 2 at 60 s after injection. Fazadinium provided better intubating conditions than pancuronium during the first 60 s after administration in group 1 (P less than 0.01). In group 2 there was no significant difference between the conditions provided by the two drugs.
