Involution of seminiferous tubules in aged hamsters: an ultrastructural, immunohistochemical and quantitative morphological study.

In this study, we examined the age-related changes on morphometric parameters and ultrastructure of seminiferous tubules, and on the expression of extracellular matrix proteins in lamina propria of Syrian hamsters. A significant decrease in the percentage of normal tubules and an increase in the percentage of hypospermatogenic and arrested maturation tubules was observed with aging. Aged animals showed a decrease in tubular diameter, tubular lumen, seminiferous epithelium volume and total tubular volume. However, the total length of seminiferous tubules was significantly increased with aging. The most important ultrastructural changes with aging were the thickening of the lamina propria, the presence of diverse abnormalities in the spermiogenesis process, degeneration of germ cells, and vacuolization and flattening of Sertoli cells showing abundant lipofucsin droplets and residual bodies. Laminin immunoreactivity was found along the lamina propria of seminiferous tubules both in young and aged animals. Fibronectin immunoreactivity was found along the lamina propria and blood vessels. Both laminin and fibronectin total volume of immunostaining per testis was increased in aged hamsters. In conclusion, the age-related changes in seminiferous tubules of hamster include: a decrease in tubular width and an increase in tubular length; widening of the lamina propria caused by a more extensive connective matrix between the peritubular cells and the basal membrane; and a strong disarrangement of the seminiferous epithelium, including germ cell degeneration and important alterations in both spermiogenesis and Sertoli cell structure.

[Infection by cytomegalovirus in patients with ulcerative colitis requiring colonic resection].

BACKGROUND: The objective was to determine the rate of colonic infection by cytomegalovirus (CMV) in patients with ulcerative colitis (UC) who have required colonic resection because of lack of response to medical treatment or complications. PATIENTS AND METHODS: Thirty nine patients (26 males, 13 females) colectomized because of refractory or complicated UC were included. Clinical and pharmacological characteristics were analyzed. Specific immunohistochemistry for CMV was performed in biopsic samples of all specimens. RESULTS: Immunohistochemistry was positive for CMV in 7 out of the 39 patients. Since 1985 to 1994 there was only one case of CMV that contrasts with the 6 cases diagnosed from 1995 to 1998 (p < 0.05). All patients with CMV infection have received systemic corticosteroids before surgery, and additionally cyclosporine in 5 of them. Twenty two out of the 32 patients without CMV infection have also received corticosteroids, cyclosporine, azatioprine or methotrexate before surgery. The rate of immunosuppression was not different between the patients with or without CMV infection. Length of treatment before surgery was also similar for both groups of patients: (17 months (18-89) in non-infected group and 30 months (13-49) in CMV infected group; p = NS). Four patients infected by CMV died in the postoperatory. CONCLUSIONS: CMV infection in patients with refractory or complicated UC is a serious complication due to its frequency and prognosis. A significant increase in the prevalence of CMV infection, not related with differences in therapeutic immunosupression, has been observed in recent years.

Efficacy and tolerance of oral dipyrone versus oral morphine for cancer pain.

In a double-blind, randomised and parallel clinical trial, two oral doses of dipyrone (1 and 2 g) administered every 8 h were compared with 10 mg of oral morphine given every 4 h for the relief of chronic cancer pain. A total of 121 patients with cancer pain without gastric involvement participated in a 7-day treatment course and were allocated to receive either dipyrone 1 g (n = 41), dipyrone 2 g (n = 38) or morphine (n = 42). Drug efficacy was analysed according to the degree of pain relief using a 100-mm visual analogue scale, and the number of patients who decided to increase the dose of the analgesic drug on day 4. The analgesic effect of dipyrone, 2 g every 8 h, was similar to that of morphine. The efficacy of both schedules was significantly greater than that of dipyrone, 1 g every 8 h. Dipyrone at either 1 or 2 g doses tended to be better tolerated than morphine, although the differences were not statistically significant.

[Study of diffuse chronic hepatopathy in the outpatient context].

OBJECTIVE: Description of the general characteristics of patients with Diffuse Chronic Hepatopathy (DCH) in an ABS. DESIGN: Empirical study of a prospective and retrospective type. Site. Primary Care. ABS La Mina. Sant Adrià de Besòs. PATIENTS AND OTHER PARTICIPANTS: All patients diagnosed as suffering DCH since 1984, in line with histological or clinical--morphological criteria accepted in the literature. MAIN MEASUREMENTS AND RESULTS: Using a data base of the centre's morbidity and mortality register, 287 reports of patients over 14 years old defined as having DCH were examined. 222 cases met the established criteria. Most of these were males (68%), and to an even higher proportion in the group with alcoholic etiology (84%), which was the most common cause of DCH (64.4%). There was a lower number of complications and deaths attributable to DCH than in published hospital series. CONCLUSIONS: Among the characteristics of DCH in a natural population using the health services, it is important to note the predominance of compensated forms having a rate of complications and deaths attributable to DCH significantly lower than what was observed in hospital series. This information may be of use in planning activities at the ABS level.

[The evaluation of different methods for rapid diagnosis in the detection of asymptomatic bacteriuria in pregnant women].

We compared the effectiveness of different methods of rapid diagnosis in the detection of asymptomatic bacteriuria in 74 females during the first trimester of pregnancy. The sensitivity, specificity and negative predictive value for the visual reading of nitrites, leukocyte sterase, bacteriuria and leukocyturia in the sediment and bacteria in Gram stain were 77%, 22%, 33%, 22% and 88%, 98%, 85%, 82%, 91% and 86%, and 97%, 89%, 90% and 90%, with and accuracy of 96%, 77%, 76%, 82% and 86%, respectively. The combination of visual readings of nitrites and Gram stain showed a sensitivity and negative predictive value of 100%. We conclude that if the reading of the reactive strip is negative, the likelihood of a positive culture is very small, and it is minimal when combined with Gram stain.
