Duraplasty with pretreated freeze-dried sterilized human dura mater.

Different collagenic tissues, such as allogeneic fascia lata, dura mater and xenogeneic pericardium preserved by deep freezing or freeze-drying and packed in National Blood Service jars have been used with good clinical results in duraplasties performed at the Department of Neurosurgery, University Hospital at Hradec Králové since 1956. In 1986 a new technology of preservation of the human dura mater was elaborated by the Tissue Bank at Hradec Králové. The implant is pretreated, freeze-dried, wrapped in two plastic bags or Steriking see-through peel-packs (Wipak Medical, Finnland) and sterilized by gamma radiation or ethylene oxide gas. Its advantages are in simple storage in operating rooms and in opportunity to choose an ideal graft with regard to its shape, size and plasticity. No rehydration is needed before the clinical use. The total of 30 grafts in 30 patients (15 children and 15 adults) have been implanted successfully since 1987. Most duraplasties were performed in the posterior cranial fossa and on the brain convexity. In these situations an intradurally burying of the graft edges and atraumatic continuous suture is recommended.

[Computer tomography of the brain in children. I. 1. Perinatal encephalopathy of vascular origin. 2. Congenital brain malformations].

CT findings are evaluated in 103 children with vascular lesions (aged up to 15 years), and in 59 children with congenital cerebral and cerebellar malformations. Their maximum has been stated to occur perinatally due to hypoxic and ischemic brain affections. Therefore most of diagnostical problems were related to the differentiation of hypodense immature brain from ischemic changes. Mentioned findings were always correlated with clinical course. Similar confrontations along with earlier CT control examination may be helpful in distinguishing milder transitory postischemic oedema from serious necrotic and malatic changes. In more aged children, vascular lesions are of rare occurrence being different in etiology. More unambiguous CT patterns are obtained in vascular changes with hemorrhagies. Of special importance in their onset is a hypoxic attack with subsequent venous bleeding mainly into the germinative matrix. While well tolerated in premature newborns, the extensive hemorrhagies in on-term ones have worser prognosis, resulting in significant changes on control examination, predominantly hydrocephalus and porencephalia. Atrophies of various extent were the most common consequence of all encephalopathies of vascular origine. In almost a half of congenital brain and cerebellar anomalies, unsignificant findings of smaller middle line cavities were obtained predominantly in pellucide septum. From the serious findings, most frequent were different varieties of dysgenesis, hypogenesis up to agenesis of corpus callosum combined sometimes with either the lipomas or cysts. Another findings consisted in Dandy-Walker's cerebellar malformation, holoprosencephalia and only two patients manifested hydrencephalia and basal meningoencephalocele. Most of diagnostical effort has been made in mainly anomalies associated with ventriculomegalia and/or cystic formations in order to elucidate the eventual communication between the structures mentioned. In addition, CT ventriculo-cysto-orcisternography has also been performed using smaller amount of nonionic contrast. At present, an important portion of CT examinations may be replaced with sonography which is more simple as used in both newborns and toddlers.

Autoradiographic localization of receptor sites of 3H-muscimol in the CNS of rats.

Radiolabelled muscimol (3-hydroxy-5-aminomethylisoxazole) a potent gamma-amino-butyric acid (GABA) agonist was continuously perfused throughout intraventriculocisternal system of the rat brain. Then the brain tissue was prepared for autoradiography. GABA receptor sites as indicated by 3H-muscimol binding and autoradiographic silver grains display different patterns of distribution over numerous cerebral and spinal cord structures. The precise cellular localization of 3H-muscimol in rat neostriatum was described and compared with other labelled regions of rat brain. The new subpopulations of local neostriatal medium size neurons labelled by 3H-muscimol was identified. The morphology and distribution of the specific neuron subpopulation is discussed. Autoradiographic approach using synthetic analogues structurally related to GABA represent a new tool for morphological demonstration of GABA receptor sites.

Autoradiographic localization of receptor sites of 3H-spiroperidol in the CNS of rat.

Our study presents macroautoradiographic maps of the distribution of 3H-spiroperidol in the CNS of rat. The radioactively labeled butyrophenone neurolepticum enables in vivo after brain intraventriculo-cisternal perfusion an exacter autoradiographic localization of dopamine receptors in various regions of the brain. For the first time 3H-spiroperidol has been administered by intraventriculo-cisternal perfusion. Also original are the finding of receptor labelling by means of 3H-spiroperidol on astroglia and in the spinal grey matter.

Effect of palmitoyl-DL-carnitine on the 3H-dihydroergotoxine intestinal absorption in rat.

Using the methods of rat intestine perfusion in situ and kinetics examination in vivo, absorption of 3H-dihydroergotoxine (3HDHE) from the gastrointestinal tract into systemic blood was investigated. The aim of the study was to increase absorption of the ergot alkaloid with palmitoyl-DL-carnitine (5 mg/rat). Low absorption of 3HDHE was demonstrated in contrast to fast and almost complete absorption of the model drug theophylline (5 mg kg-1). In the experiment this was evidenced by in situ continuous measurement of the administered activity (125 micrograms kg-1) into a reservoir. At the end of the experiment (120 min) the plasma activity reached 0.034 +/- 0.014%, retention in the intestine achieved more than 60%, uptake in the brain 0.044 +/- 0.015%, cumulative excretion in bile 1.50 +/- 0.31% of administered activity. Palmitoyl-DL-carnitine did not influenced the percentage of activity in the plasma and did not affect bile excretion, retention in the intestine and uptake in the brain. In the vivo experiment oral administration of 3HDHE into the stomach (222 micrograms kg-1) increased activity in plasma (0.069 +/- 0.020% within 24 h), cumulative excretion in urine was 6.1 +/- 3.6%, retention of the drug in the stomach and intestine 46 +/- 12%, activity of the brain 0.11 +/- 0.02%, in the kidney 0.39 +/- 0.17%, and in the liver 0.80 +/- 0.30%. After palmitoyl-DL-carnitine administration the activity of plasma reached 0.084 +/- 0.022% (NS), retention in gastrointestinal tract 39 +/- 9% (NS), activity in the liver 0.71 +/- 0.17% (NS), and activity in the kidney 0.42 +/- 0.13% (NS).(ABSTRACT TRUNCATED AT 250 WORDS)

[Repair of the dura mater in children using xenogenic pericardium].

The authors report their five-year experience with the use of xenogenic calf pericardium for plastic operations of the cerebral and spinal dura mater which they introduced into neurosurgical practice in 1983. Of 165 grafts they selected 50 which they used in children for the plastic operation of the dura mater above the posterior cranial fossa, above the convexity of the brain and at the skull base. The xenogenic pericardium proved successful and resolved also the shortage of allogenic collagenous tissues (fascia lata, dura mater) used for plastic operations of the dura mater. Moreover, due to its resistance it achieved priority in some indications and localizations. When used, certain conditions must be met to ensure satisfactory incorporation (suitable size, intradural submersion, careful suture).

Congenital dermoid cysts over the anterior fontanel. Report on 13 cases in Czechoslovak children.

The authors report on 13 cases of dermoid cysts over the anterior fontanelle in Czechoslovak children. These children were 2-19 months of age; with a male-to-female ratio of 2:1. The cyst sizes ranged from 10 to 30 mm. Hence, the number of cases published in the world so far in children has increased to 174. Simultaneously, the number of European cases has increased to 30 (17.2%). Nevertheless, the dominance of the reported cases is still 74 in America (42.5%) and 45 in Africa (25.9%).

Bobble-head doll syndrome associated with the III ventricular cyst. Three cases in children 7 years after CVP or CVA shunting.

Thirty-nine cases of the bobble-head doll syndrome have been operated upon and are described in the literature. The authors re-evaluate three cases of III ventricle cysts treated with cystoventriculoperitoneal (CVP) or cystoventriculoatrial (CVA) shunting 7 years ago. In the first case (a 4.5-year-old girl), the CVP shunt was removed after 16 months due to suppuration around the device after trauma of the scalp. Nevertheless, the communication between the ventricle and cyst continued and the bobbing never returned. In the second case (a 10-year-old girl), after 4.5 years a pseudocyst at the end of the peritoneal catheter was accidentally found. It was evacuated and, as in the third case (a 17-year-old boy) follow-up for 7 years showed excellent results from the clinical point of view and CT scans.

Xenogeneic pericardium as a dural substitute in reconstruction of suboccipital dura mater in children.

A 5-year experience with the glutaraldehyde-stabilized freeze-dried radiation-sterilized calf pericardium used as a dural substitute is reported. The structure of pericardium xenograft is compared with other collagenous materials used for duraplasty (allogeneic fascia lata and dura mater) by light and electron microscopy. The special neurosurgical techniques involved in using pericardium xenografts in the reconstruction of suboccipital dura mater in children are presented in detail.

[Developmental cysts in the area of the quadrigeminal plate in children].

The authors describe three cases of operated developmental cysts in the area of the laminae quadrigeminae in children. The performed multiple fenestrations or excision of the wall of the cyst and effective communication with the subarachnoid or ventricular system, as well as a plastic operation of the dura mater with collagen tissue from a Tissue bank. By means of light microscopy the walls of two cysts were examined (one arachnoid and one glioependymal), one cyst was examined by electron microscopy (glioependymal). The authors supplemented by their own cases the number of hitherto described cysts in the collicular region to 56.

Immunological aspects of diagnosis of celiac sprue in children.

Serum anti-gluten (AGA) and anti-reticulin (ARA) antibodies were examined in children suffering from celiac sprue (CS); cellular hypersensitivity to gluten was tested and secretion of immunoglobulins and anti-gluten antibodies into the culture medium after 24-hour in vitro cultivation of jejunal mucosal biopsies was investigated with the aim to assess significance of these methods for CS diagnosis. Indirect immunofluorescence was used in ARA determination, ELISA method for AGA determination, cellular hypersensitivity was examined using the test of leucocyte migration inhibition (LMIT) with gluten. ARA were detected in 69% of children with untreated CS and in 28% of CS children who were on a gluten-free diet. ARA specificity was 100%. Statistically significant higher titres of IgG AGA and IgA AGA were proved in children with untreated CS as compared with the control group. IgA AGA were detected significantly more frequently than IgG AGA. No relationship between positive AGA and the degree of alteration of the jejunal mucosa was found. IgG AGA sensitivity in CS children with pathological findings on the jejunal mucosa was 52%, specificity being 95%. IgA AGA sensitivity was 82% with specificity 90%. After a parallel application of IgA AGA and ARA, sensitivity of the tests rose up to 95.5%, specificity being 90%. Examinations of ARA and AGA have a significant importance for laboratory tests used for screening children with pathological findings on the jejunal mucosa and for indication to jejunal biopsies. The above tests do not replace jejunal biopsy in CS diagnosis. They can be applied in monitoring children with CS during gluten challenge and in checking how the gluten-free diet is observed. Significantly higher stimulation of leucocyte migration in gluten environment was proved in children suffering from CS as compared with the control group. Stimulation of migration is supposed to indicate cellular hypersensitivity to the antigen used in CS children. After a 24-hour culturing of jejunal mucosal biopsies, significantly elevated concentrations of IgA immunoglobulin and IgG, IgA and IgM AGA were found in the culture medium as compared with those obtained from cultured jejunal mucosal biopsies of control group children. The test of leucocyte migration inhibition and in vitro culturing of jejunal mucosa are quite complex and exacting methods when used in routine practice. Their significance lies in the fact they enable us to study in vitro immunological reactions in children suffering from celiac sprue.

[Regeneration of auto- and isografts of submandibular glands in the brain of laboratory mice. I. Morphologic evaluation of the healed graft using light microscopy].

The mouse granular convoluted tubules of submandibular gland of laboratory mouse are releasing a range of biologically active peptides (renin, neural growth factor and others) into both the saliva and blood circulation, the males producing it in a larger extent than females. Recently, several from respective peptides were identified to be endogenous ones and brain-related. The present work was aimed to utilise submandibular gland/SMG auto- and isotransplants regenerating in murine brain as a possibly local source of peptides. Experimentally, the newborn and juvenile matured white A breeded mice of both sexes were used. Glandular grafts were grafted into brain parenchyma or CSF spaces. Laboratory animals have then been perished during the first 6 weeks after transplantation, and the transplants so acquired evaluated as serial frontal sections embedded in paraffin and H.E. stained by light microscopy. Also cryocate sections were incubated in order to detect the presence of alkaline phosphatase (AP) and succinic dehydrogenase/(SDH). It was stated experimentally that both mentioned SMG grafts underwent the survival and development intracerebrally. Some first regressive changes were gradually replaced by glandular proliferation and lobular neomorphogenesis having been more pronounced in osotransplants. The proliferative period was characterized by cellular mitoses, multiplication of duct-like and terminal tubulous structures of newly formed glandular lobules. Partially, the isotransplants display the transformation of proliferation stage into that of cellular cytodifferentiation followed by gradual appearance of striated ducts, acini and even granular convoluted tubules on the 5th week after transplantation. Also the reoccurrence of enzyme activities in the transplant parenchyma after their initially total disappearance is testifying of both proliferation and cytodifferentiation developed gradually. During the first days of implantation, the revascularization of grafts occurs, those being high in AP endothelial activity of vessels newly formed. This is to conclude that higher proliferative intensity of isotransplants and their exclusive cytodifferentiation demonstrate that an undifferentiated murine SMG which can develop itself ontogenetically is more effective graft than a SMG differentiated fully. On the next stage, the development of glandular grafts will be studied with more delay after transplantations. Also the enzyme implementation of new parenchymatous components is to be elucidated. Further experimentation is planified as to influencing intracerebral SMG graft development with administration of hormones and isoproterenol to laboratory animals.

[Scanning electron microscopy of corrosion preparations of the vascular beds of organs in laboratory rats].

A corrosion casting method of the vascular bed of rat's organs using injection of methyl metacrylate resin was described. The casts were photographed in scanning electron microscope Tesla BS 300. Microphotographs of vascular bed casts of selected organs demonstrate the contributions and possibilities of scanning electron microscopy for the investigation of three-dimensional architecture and topographical relations of blood microcirculation in organs.

[Ileus in an infant caused by a ventriculoperitoneal shunt].

The authors describe a case of adhesive ileus caused by the intraperitoneal part of a ventriculoperitoneal shunt which was performed on account of congenital hydrocephalus. Preserving the patency of the ventriculoperitoneal drainage, the ileus was treated according to principles of abdominal surgery. The functional catheter was left in situ. Complete intestinal occlusion is one of the very rare intraabdominal complications of ventriculoperitoneal shunt. In the discussion the authors analyze available data from the literature on intestinal occlusion with the above aetiology.
