Biomechanically important articular surfaces in the talocrural joint.

The authors investigated the influence of relations between the utile areas of the articular faces in ankle joint in medial twisting of the foot. For their investigation they used the material from the Institute of Anatomy "Drago Perović", Institute of Pathology and Institute for Forensic medicine of the Medical Faculty of Zagreb. Investigations were performed in articular faces of the tibia, fibula and talus on the left and right legs of the adults. Mutual comparing of the joint faces, their areas and altitudes, angles between the medial malleolus and tibia, and the altitudes of the medial and lateral malleolus has shown great individual differences. According to these results the authors concluded that the disproportion of the articular bodies caused habitual medial twisting of the foot. The importance and the role of ligaments, muscles and joint capsule were not the object of this investigation. The obtained results are a contribution to the functional anatomy of the ankle joint, and to the clinical pathology of this region.

[Long-term follow-up of related kidney donors after nephrectomy].

From 1973 to July 1990, 183 kidney transplantations were performed at the Transplantation Unit of the Department of Urology--Faculty of Medicine--University of Zagreb. Out of these, 57 were from the living related donors. The research included 50 (88%) living related donors (28 mothers, 12 fathers, 5 brothers and 5 sisters) followed up from 1 to 15 years after donor nephrectomy. The donors' health conditions were evaluated by clinical-laboratory examinations (biochemical tests, renal function tests, urinoculture, ultrasound, isotopic renal studies, arterial blood pressure). In 48 (96%) neither morphological nor functional changes of the urinary system were found. A certain number of donors requested a transfer to less physically strenuous jobs. Except for the grafts functioning well in the recipients, the results also showed that the quality of donors' lives remained unchanged.

[Auto-reactive anti-B antibodies and hemolysis in patients with type B blood after kidney transplantation from an O blood group donor].

The course and the treatment of the patient M.F., who after kidney transplantation, produced autoreactive antierythrocyte antibodies is described. The patient M.F. was B, Rh positive and the kidney donor was 0, Rh positive. On the 12th posttransplantation day severe hemolysis, anemia and bilirubinemia were observed. Beside anti-A, autoreactive anti-B antibodies were discovered in the B group patient and the direct antiglobulin test (DAT) was positive. The patient was treated with several transfusions of 0 blood group washed red cells, immunosuppressive drugs (Cyclosporin, Methylprednisolone) and with daily plasmaphereses. After the 5th plasmapheresis the serum became clear, autoreactive anti-B antibodies could not be detected in the serum, but DAT was still positive. At that time the function of the graft was adequate. Between 22nd and 30th posttransplantation day DAT became negative. Autoreactive, anti-B antibodies were generated by the donor's lymphocytes, that were transferred with the kidney graft and which continued to synthetize anti-A,B antibodies in the patients' circulation.

[Conservative surgical procedures in transitional cell carcinoma of the kidney and ureter].

Conservative surgical procedure in selected patients with transitional cell carcinoma of upper urinary tract is alternative to radical surgery. Generally, a conservative surgical procedure is indicated in solitary low-stage lesions, particularly impaired renal function and bilaterality. At our Institute, conservative surgical procedure was performed in patients with solitary kidney and significant renal function damage. Survival after conservative and radical surgery was similar, extending the scope of indications for conservative surgical procedure in cases of low-grade low stage tumors.

[Immunologic and clinical definition of the rejection reaction in kidney transplantation].

In the evaluation of the immunological state of a kidney transplant there is no reliable diagnostic test. Therefore, several various tests should always be applied. The aim of this study was to define the type of immunological reaction (cellular or humoral) that causes the rejection of transplanted kidney. A group of 45 patients with a kidney transplant were tested 175 times, in various phases of the kidney transplant, starting on the day of transplantation up to several months posttransplant. Specific antibodies (SA) and specific cytotoxic lymphocytes (SCL) directed against donor cells in lymphocyte-mediated cytolysis tests in the presence of antibodies (ADLMC) or without them (LMC) were determined. LMC was significantly correlated with acute rejection (85%). In the phase of clinical quiscence of the kidney transplant LMC was positive in 4% of the cases. During the period of chronic rejection it was positive in 75% of the cases. ADLMC test is significantly correlated with chronic rejection (88%). In the period of acute rejection it was positive in 27% while in clinical quiscence in 11% of the cases. Twenty-two patients with non-immunological disorders of the kidney transplant were tested 62 times. LMC test was always negative, while ADLMC test was positive in 6.4% of the cases. These tests prove to be good parameters for defining immunological reaction. Thus, they can be of great importance in determining adequate immunosuppressive therapy. Negative results of tests in non-immunological disorders of graft function are highly significant.

[Injuries of the urogenital system].

War injuries of the urogenital system present serious injuries accompanied with significant morbidity and mortality as well as causing high degree of later disablement. The paper presents our experiences and basic directions in treating urogenital injuries especially those penetrating and inflicted by high velocity missiles. Thirteen patients with urogenital injuries have been operated on and that makes 4% of all war injuries treated at KBC-Rebro hospital. Among the total number of abdominal injuries those urological make a considerable share--as high as 21% and that could be the consequence of the triage. In all the patients except one, in whom either kidney, ureter or bladder has been injured there has also been an associated injury of another abdominal organ (mostly colon and small intestine, than liver and spleen). All the operating procedures have been performed together with the general surgery team. X-ray preoperative examinations have been carried out only in two patients while the general state of the rest of the patients has not allowed for such procedures which could delay surgery. Nephrectomy has been performed in only one out of these thirteen patients. Orchidectomy has been carried out in only one patient in whom extensive laceration of the testis has occurred, while in others preserving surgical treatment has been applied.

[Congenital ventral curvature of the penis].

We report two patients with congenital ventral penile curvature. Both patients were admitted to our Department at 18 years of age, because of deformation which was an obstacle to their normal sexual intercourse. Deformity was corrected with the Nesbit procedure using intraoperative artificial erection. During the surgical procedure no chordee or anomalies of the urethra or spongy tissue could be detected, thus changes were classified in a relatively rare group of deformities due to the inelasticity of tunica albuginea. We have also made some observations on the technique applied which could be helpful in practice.

[Unusual localization of an echinococcal cyst].

Hardly ever one can find a hydatid cyst in other parts of the organism except in the liver and lungs. The localization in the retroperitoneum is very rare. We have shown an unusually located hydatid cyst in the retroperitoneum. The cyst was firmly integrated with the structure of m. psoas and the muscle resection was necessary.
