Due to controversial evaluation of the contribution of clinical signs for the diagnosing of non-specific inflammatory bowel disease, as well as due to the lack of similar data, we tried to find our own answer to the question as to whether the clinical signs of ulcerative colitis (UC) and Crohn's disease (MC) are helpful, unhelpful, or even confusing for the diagnosis. A group of patients with MC and UC was analyzed from this aspect. Individuals in their twenties and thirties prevailed in the sample, mostly intellectual workers, the number of males and females was equal. Our attempt to analyze all the available diagnostic methods originated in an observation that a long period of health problems precedes the diagnosis of MC, namely 1.5 y in males and as long as 4 y in females. Other striking information was that surgery represented the initial treatment in 66% of cases and the correct diagnosis was made peroperatively only in 56% of cases. We compared our results with those of the OMGE study, one of the largest projects which evaluated positively the contribution of clinical signs to the diagnosing of MC and UC. We found that the main signs of CU have not changed in the last century, and some additional signs occur rather due to complications than due to the disease per se. Frequency of pain increased by 25% in our patients, and approximately 1/3 of it represented intermittent pain caused by tenesms. Pain in MC must be properly analyzed in order to discriminate acute appendicitis. Other indicators did not differ from the OMGE study. In accordance with its results, we confirmed the importance of correct evaluation of clinical signs for the diagnosis and differential diagnosis of UC and MC. The number of diagnostic methods still increases. Their validity must be continuously re-evaluated, however the clinical examination in the dynamic process stays to be of crucial value.
Postoperative analgesia inhibits the stress cascade with negative effects on whole organism. Therefore the spectrum of drugs used for soothing postoperative pain quickly widens. The epidural route appears as being logical, since due to the direct effect in the transmission and processing of pain it suffices with a lower dosage. The authors refer to a group of 30 patients postoperatively treated by a combination of tramadol and Clonidine administrated by means of an epidural catheter. 26 patients evaluated the induced analgesia as excellent or sufficient. (Tab. 3, Ref. 4).
The study compares two groups, both in number of 100 patients subdued to laparoscopic cholecystectomy. In each group a different surgical technique was performed. The comparison study regarded duration of surgical performance, incidence of postoperative complications, surgical morbidity and mortality, as well as duration of hospitalization and return to full activity. The surgical interventions were performed at two distinct workplaces. Neither of the mentioned parameters yielded significant differences in dependence on the utilized operation technique. The entire group of 200 cholecystectomic performances was thereafter compared with a comparable group of classical cholecystectomies which had been performed at the identical hospitals. The mentioned parameters yield statistically significant differences in postoperative morbidity, hospitalization duration and return to full professional activity. The results speak in favour of laparoscopic cholecystectomy. The duration of surgical performance, number of postoperative complications requiring reoperation, and mortality do not yield significant differences. The comparison of the observed parameters in relation to age categories presents laparoscopic cholecystectomy as the method suitable also for older patients above 65 years of age. (Tab. 2, Fig 2, Ref. 6).
The authors compare the results of surgical treatment for colorectal carcinoma in 150 patients aged over 69 years, seen over the years 1979-1988, who had developed local complications requiring urgent surgery. Of gerontologic patients with colorectal carcinoma these represented 24.6%. The cause of the complications was late diagnosis mostly due to the patient's neglect who failed to appreciate an even evident symptomatology. These serious complications of colorectal carcinoma did not present an inoperable condition and in 34% the tumor could be radically removed. The advanced age of the patients along with the necessity of urgent surgery are the major risk factors responsible for the high mortality rate compared to patients who underwent elective surgery.
Splenectomy was performed in five patients with hairy cell leukemia at the Surgical Department of the Hospital with Policlinic, Bezrucova, Bratislava, over the years 1982-1987. One of the patients died 3 years after operation due to progression of the disease, the other four patients have been surviving for 2-7 years following surgery without relapse of the disease. The presented results, compared with the world literature, justify splenectomy to be advocated as the primary therapeutic procedure in hairy cell leukemia.
Authors point out possibilities of preservation of the spleen or its parts in injuries and some diseases, especially non-parasitic cysts. Review is completed by a case report of successful management of a splenic pseudocyst by partial resection.
