The impact of diabetes mellitus on postoperative morbidity in laryngeal surgery.

The main aim of this study was to analyse risk factors of postoperative morbidity after extended laryngeal surgery comparing patients with diabetes mellitus and non-diabetic patients. In retrospective study 69 patients (63 male and 6 female), who underwent partial laryngo-pharyngectomy and total laryngectomy between 2003 and 2004, were evaluated. 13 % of the total group of examined patients had concurrent diabetes, while 87% were nondiabetic patients. We performed partial laryngopharyngectomy in 39 out of 69 patients (56.5%) and total laryngectomy in 30 patients (43.5%). Secondary wound infections (88.9%) and pharyngocutaneus fistula (44.5%) had a significantly higher rate in diabetic patients (p<0.001). Among diabetics the cases with intraoperative or postoperative blood transfusions were more frequent--44.4%(p<0.001). Diabetic patients with laryngectomy procedures had more frequently prolonged postoperative anemia (55.5%, p<0.001) and electrolitic disbalance (66.6%, p<0.001). Our results have confirmed that diabetes mellitus is an important independent general clinical factor, which increases postoperative morbidity and hospitalization time in laryngeal surgery. Our data indicate the need to make a very serious plan and clinical assesment for laryngeal surgical therapy in diabetic patients.

The disinfection of water by microalloyed aluminium-based composite.

In the submitted paper the water disinfection capacity of the microalloyed aluminium based composite (MABC) was studied. MABC is material in the form of steel wire, plated with microalloyed aluminium. The effects of the composite are based on the very negative stationary potential of microalloyed aluminium, and its spontaneous dissolution in water with generation of AI(III) ions, and reduction of water with the generation of H2 and OH ions. As a final product of these reactions, a voluminous Al(OH)3 precipitate is formed. Having in mind its great efficacy in purification of different waters from many chemical pollutants we made the following hypothesis: reduction characteristics of the MABC surface, presence of Al(III) and OH ions, and coprecipitation on Al(OH)3, can be also toxic and destructive for bacteria in water. The experiments were carried out with the water model solutions (WMS) based on adapted natural surface water (NSW), inoculated with the Escherichia coli. All treatments were performed in the original semi-flow system (SFS), in which convection increases efficacy. The results show that approximately every 10 min the number of viable bacteria was reduced for about one log10 count, with the complete disinfected water phase as the outcome of the treatment. At the end of the treatment, the Al(OH)3 precipitate still contained a low amount of coprecipitated viable bacteria, which died within a relatively short period.

Abscopal effects of local fractionated X-irradiation of face and jaw region.

The abscopal effects of high doses of X-irradiation, 6000 R given in 12 fractions locally on rat face and jaw region, upon the haematopoietic and lymphatic tissues, were studied. Haematological and pathophysiological studies were performed. Peripheral blood samples were taken after the 7th, 17th and 27th day during the course of the irradiation. 24 hours after the last irradiation the smears of femur bone-marrow were done. In the smears, the cells of erythrocyte and leukocyte series, as well as their mitotic index were determined. 24 hours after the irradiation the pieces of lymph nodes and salivary glands were taken for histological analysis. A significant depression of peripheral leukocyte counts was found in all observation intervals as well as the change in mitotic index of granulocyte series in bone marrow. No significant histological changes on the lymph nodes and salivary gland section were found 24 hours after the last irradiation.

[Possibilities in the use of samples in dentistry].

The paper describes a feasibility of a useful application of different types of samples in an investigation of epidemiology of caries, gingivitis, periodontal disease and orthodonic disturbances. Also, an illustration of sampling procedure suitable for investigation of epidemiology of diseases of mouth and teeth is presented.

[Preoperative planning of the position of endosseous implants].

The malplacement of an implant can lead to compromise in aesthetics and occlusion. When the implant is not placed correctly, every effort must be made by the restorative dentist to make compensation of this malplacement. However it is prudent to remove the implant in severe malplacements. The authors report on the method of preoperative determination of the location of endosseous implant and inraoperative control of its position.
