[Retrosomatic spondylolysis of the 5th lumbar vertebra with segmentation disorder of the corresponding spinal arch].

Retrosomatic spondylolysis is a very rare type of neural arch defects. In typical cases the defect is a vertical cleft of the pedicle, which is thought to be the result of two ossification centers. The authors present a case of retrosomatic spondylolysis with a segmentation defect of the corresponding vertebral arch structures. The follow-up of this child from age 7 to 12 seems to be proof for this defect to be a fatigue fracture quite similar to those in isthmic spondylolysis.

[Indications and results of shoulder arthrodesis and concomitant myoplastic interventions. Follow-up of 60 patients].

From 1949 until 1980 sixty shoulder arthrodeses were performed at the orthopedic department of Heidelberg university. During this period of time a change concerning indication and operative method took part. The increasing number of severe motor cycle accidents with partial injuries of the brachial plexus during the last years makes additional myoplastic operations necessary. The ideal position of the arthrodesis is demonstrated as well results of long term follow ups are repeated.

[Acetabuloplasty by pedicled transfer of the iliac crest apophysis in the dog].

Appositional incremental acetabuloplasty using a self growing iliac crest graft pedicle on its epipyseal vessels was performed in 54 puppies 3 months old. Remodeling an adaptation to the shape of the acetabulum is excellent in most of the cases, issuing into a spherical composite cavity. Persistance of a hyaline cartilage is noted until the end of the first years, but long term morphological transmission electronmicroscopy studies demonstrate a progressive transformation into fibrocartilage. This experimental study demonstrates the possibility to use remodeling and growth properties of a pedicled iliac crest, transplanted as an iland flap on the extraarticular acetabular roof. This new surgical technique can be proposed for the care of difficult dysplastic hips in children.

[Straightening operations in juvenile kyphosis. Results in 31 cases using Harrington's dorsal compression spondylodesis or its combination with ventral osteodiskectomy and clip insertion].

Results of operative treatment in 31 cases of juvenile kyphosis are presented. In 23 cases a posterior Harrington spondylodesis, in 6 cases a combined staged anterior and posterior spondylodesis, in two cases an anterior spondylodesis alone were performed. The average preoperative angle in the first group was 50 degrees (15 degrees-83 degrees), the average postop, angle was 22 degrees representing an 56% average improvement of the initial angle. At reevaluation, however, an average loss of correction of 13.9 degrees or 49.6% reduction of the initial improvement was registered. The combined procedures started with an average preoperative angle of 60 degrees, the postsurgery value was 22 degrees representing 63% of improvement. In 6 cases the average loss of correction at reevaluation was only 7% or 18.9% of early improvement was lost. The results favor strongly a combined staged anterior and posterior procedure for correction. When the behaviour of the wedge vertebrae is included as well, this study offers clear indications for a differentiated treatment of choice in each case of severe juvenile kyphosis.

Effect of scoliosis surgery on pulmonary function in patients with myelomeningocele.

Thirteen patients with myelomeningocele who required operation for scoliosis were tested for changes in pulmonary function 1 month before and an average of 13 months after spinal stabilization. The average age at the time of operation was 12 years 11 months. Ten patients had restrictive changes in pulmonary function preoperatively. Owing to the severity of scoliosis, an anterior approach to the spine including resection of ribs and division of the diaphragm was necessary for eight children. Postoperatively, despite these procedures, eight patients had an increase in vital capacity and six patients had an increased forced expiratory volume in 1 s.
