[Peroperative autotransfusion in aortic surgery].

Intraoperative autotransfusion of blood by means of a Solcotrans retransfusion bag was used during abdominal aortic surgery in a series of 25 patients. The need for transfusion of blood products, erythrocytes and plasma, decreased from 8.5 units to 4.5, as compared with 26 patients operated during the same period the preceding year (p less than 0.01). Hemoglobin was not lower at any point per- or postoperatively when autotransfusion was used. In contrast to one out of 26 patients operated without autotransfusion (p less than 0.05), seven of 25 patients whose operations involved autotransfusion did not need any donor transfusions. Exposure to potentially contagious donor products may be further decreased by using polygeline (Haemmaccel), and in some cases heat-treated albumin, in stead of plasma. While the mean number of donor transfusions remained relatively high with autotransfusion, the median number of erythrocyte donor transfusions was 0 units preoperatively and one unit postoperatively. An average of 684 ml of blood was retransfused. This corresponds to less than two units of blood. The main decrease in the number of transfusions was due to more careful surgical technique, the use of impregnated vascular grafts, and a changed attitude towards transfusions.

Assessment of probability of survival in penetrating injuries using the TRISS methodology.

By the TRISS methodology, probability of survival in injury can be estimated. It is based on a statistical analysis of outcome which is influenced by the severity of the injuries as expressed in the Injury Severity Score (ISS), the physiological function as expressed in the Trauma Score (TS) and the patient's age. We have used the TRISS formula in 206 patients with penetrating injury. Of these patients, 149 sustained stab wounds, 32 gunshot wounds and 25 others. ISS ranged from 2 to 38, the mean ISS being 9. The function was good (TS greater than 14) in 85 per cent. Estimated probability of survival ranged from 1.00 to 0.42. Three patients (1.5 per cent) died. The probability of their survival was 0.92, 0.96 and 0.98, respectively. All the fatal cases had serious predisposing conditions: chronic pulmonary disease, alcoholism, and psychiatric illness. In penetrating injury, the patient's functional status at the start of treatment is of greater importance for the outcome than the anatomical severity. The concept of the methodology of TRISS for assessment of probability of survival seems useful for review and comparison in injury care.

Human immunodeficiency virus and hepatitis B virus in injured patients and victims of violence.

Of a total number of 643 injured patients admitted during 1986 and 1987, 113 were tested for antibodies against the human immunodeficiency virus (HIV) and hepatitis B surface antigen HBsAg and the concentration of hepatitis B surface antibodies (HBsAb) was determined. Nine patients were HIV positive while HBsAg was detected in two patients and increased levels of HBsAb were found in 19 patients. HIV antibodies were found in 6 (4.8 per cent) of 124 victims of violence, in 3.3 per cent of the patients with penetrating injuries and 1.1 per cent of the patients with multiple, closed injuries. The estimated prevalence for HIV infection in the Norwegian population is 0.08 per cent, thus indicating an over-representation of infected patients among injured patients and victims of violence.

Plasma proteolysis and circulating cells in relation to varying endotoxin concentrations in porcine endotoxemia.

Ten juvenile pigs receiving a continuous infusion of 0.01 mg/kg of endotoxin over 3 hr and seven animals infused with sterile saline (serving as controls) were studied for 5 hr. Endotoxin concentrations in plasma as determined with a chromogenic Limulus amoebocyte lysate (LAL) test reached a steady state of about 1,000 ng/liter after 1 hr and declined rapidly as the infusion was discontinued. Preinfusion values were reached at the end of the observation period. Endotoxin concentrations found during the infusion period were comparable with those seen in humans with septicemia. The endotoxin infusion was followed by hemoconcentration, leukocytopenia, and thrombocytopenia. Using chromogenic peptide substrate assays, activation of the plasma kallikrein-kinin, fibrinolytic, and coagulation systems was detected. Although the endotoxin concentrations reached preinfusion values within the last 2 hr of the observation period, changes found in circulating cells and components of the plasma cascade systems did not normalize, and the hemodynamic situation did not change.

Early use of corticosteroids in severe closed chest injuries: a 10-year experience.

Because of the controversy about the treatment of injured patients with steroids, each doctor treating closed chest injuries at Ullevål Hospital, Oslo, has been free to decide whether to use steroids. However, if steroids were to be used, early administration was recommended. Thus, on admission methylprednisolone 30 mg/kg body weight was given to 107 patients having at least four rib fractures or a flail chest. The dose was repeated after 8 and 16 hours. The patients treated with steroids were compared with 159 patients not receiving steroids, but who otherwise were treated identically. Three-quarters of the patients had multiple injuries and 219 patients (82 per cent) had intrathoracic injuries such as pneumothorax (39 per cent), haemothorax (37 per cent) or contusion of the lung (59 per cent). Forty-six patients (17 per cent) were in shock on admission. Most patients could be managed with intravenous infusion, oxygen, relief of pain and chest drains. Early thoracotomy was performed in 10 patients and 91 patients needed artificial ventilation. Analysis of the two groups of patients revealed a significantly lower hospital mortality of 11.2 per cent for those treated with steroids as against 23.3 per cent for those without. Comparison of the two groups demonstrated no differences which could explain the difference in mortality. The mean Injury Severity Score (ISS) was 24.0 for the steroid treated group and 21.4 for the control group. The steroid treatment was not associated with any increase in the incidence of infection. The present analysis indicates that steroids, when given early, may improve the clinical course of patients sustaining severe closed injuries of the chest.(ABSTRACT TRUNCATED AT 250 WORDS)

Rhabdomyoma of the heart with intrapericardial expansion.

A rare case of a rhabdomyoma originating from the surface of the right atrium in a 20-year-old female is reported. The tumor showed electrical activity and caused both outflow obstruction and tamponade symptoms. It was successfully removed by a right-sided thoracotomy. The clinical course, diagnostic investigations, and the therapeutical approach of this rare lesion are discussed.

Spontaneous pneumothorax in chronic obstructive pulmonary disease: complications, treatment and recurrences.

Data from 303 patients with 389 admissions for spontaneous pneumothorax from 1970 to 1980 at Ullevaal Hospital, Oslo, Norway, were reviewed. Spontaneous pneumothorax carried a significantly higher complication and mortality rate in patients suffering chronic obstructive pulmonary disease (COPD). Their higher median age compared to non-COPD patients contributed to this, but did not account for the increased mortality. The risk of developing wound infection and/or pneumonia was significantly higher after 7 days of chest tube treatment in both patient groups, independent of age. There was no association between recurrence rate and COPD/non-COPD, age or duration of chest tube treatment (1-7 days, 8 days or more). Complications were not more frequent after thoracotomies performed in COPD patients. Therefore operative treatment for both primary and COPD-related spontaneous pneumothorax should be considered if tube treatment is not successful after 1 week and there are no contraindications.
