[Prevalence of menstrual and eating disorders among infertile women].

Menstrual disorders are among the most frequent somatic complications and symptoms of eating disorders. Since 1972 amenorrhea has been registered among the diagnostic criteria of anorexia nervosa (AN). Although such a criterion is not essential for establishing a case of bulimia nervosa (BN), international research has, on several occasions, described menstruational disorders (in 95% of the cases) and amenorrhea (in 20% of the cases). This study was designed to assess the prevalence of eating disorders connected to menstruational disturbances having no organic causes. Our method was the following: the test was carried out via questionnaires (ANIS, BCDS, BITE, EDI, BDI) completed by patients of endocrine surgery of two gynecological out-patient wards. We have found the following (n = 75) prevalence figures: AN: 4%, BN: 12% while the total prevalence of clinical and subclinical eating disorder syndromes was at 44%. Our findings, are in line with other international findings.

Increased yield of F VIII from pooled plasma vs. single-donor plasma in the production of cryoprecipitates.

Coagulation factor VIII:C yield was studied in two types of cryoprecipitates. The first group contained products from single-donor plasma units. The other group contained cryoprecipitates which were produced from pooled plasma. The volume of plasma/bag was not different between the two groups, but both the yield and the total content of F VIII:C in cryoprecipitates were significantly different. The yield of F VIII:C was higher (+20% in relative terms) in cryoprecipitates produced from pooled plasma, resulting in higher potency of such products. The positive effect of plasma pooling on the recovery of F VIII:C might be a result of reassembly of factor VIII subunits of different individuals in the plasma pools. The findings may have a role also in large-scale production of F VIII concentrates.

[Blood products stored in plastic bags: release of plasticizers from the bag material].

The plasticizer di-(2-ethyl-hexyl)phthalate (DEHP) is slowly leached from the wall of the PVC blood bags into the blood products. It will be partly incorporated into the membranes of the blood cells, and finally will be transfused together with the blood products into the recipient, where DEHP might exert toxic effects. DEHP content was determined by HPLC in different blood products stored in plastic bags manufactured by different firms (Medicor--Hungary, MacoPharma--France, Terumo--Japan, Baxter--USA). In whole blood DEHP is present mainly in the plasma and blood bags of different firms show similar results. 1 unit of whole blood contains 20-50 mg DEHP on day 14 of storage, and 30-55 mg on day 28, while red blood cells resuspended in plasma contained only 2-15 mg on day 14. 15-20 mg DEHP was found in 1 unit of plasma stored for 4 days at +4 degrees C. Only 1-4 mg DEHP was detected in 1 unit of erythrocytes (cca. 200 ml) even on day 14 of storage. The supernatant of platelet concentrates contained the relatively highest amount of DEHP (even 50-65 mg was found in 1 unit of platelet concentrates on day 5 of storage). When platelet concentrates were studied, essential differences were recorded between the bags manufactured by different firms. Platelet reactivity is better maintained in storage bags manufactured without DEHP.

[Correlation between elevated maternal serum alpha-fetoprotein, certain pregnancy complications and fetal death].

In a prospective epidemiological study of 23,792 singleton pregnancies the authors investigated the relationship between elevated maternal serum alfa-fetoprotein level and the risk of pathological pregnancy outcome. It has been found that elevated level of maternal alfa-fetoprotein (above 2.5 MoM) is associated with a higher risk of pathological pregnancies and an increased risk of subsequent fetal death. Physicians should be aware of the benefits of the maternal alpha-fetoprotein screening not only in the context of prenatal detection of congenital defects, but also in early identification of increased risk of certain obstetric complications.

[Relationship between congenital heart disease and various factors affecting pregnancy].

In a prospective epidemiological study data of 24,732 pregnancies were analysed by computer programmes for the incidence of congenital heart disease (CHD) and its relationship to various factors affecting pregnancy (age and weight of mother, smoking, alcohol-, and coffee-consumption, usage of oral contraception). Frequency of CHD was 0.9%. Data showed that CHD could be associated with alcohol consumption, smoking, maternal age. Other factors like coffee consumption or taking oral contraceptive tablets did not influence the frequency of CHD. Authors conclude that in the prevention of CHD, reduction or even omission of both alcohol consumption and smoking are highly justified.

[Composition of blood preparations. Quantitative and qualitative properties].

Some aspects of the composition of the main blood bank products are reported and discussed. Appearance of the desired clinical effect or undesired side-effect is a consequence of qualitative and quantitative characteristics of blood derivatives. Red cell products are of different purity (leukocyte contamination and plasma content) therefore clinical effects and side-effects, including long-term consequences, are also different. Actual volume of single-donor products are in a wide range, thus the effect of quantity should be taken also into consideration. Actual practical problems will be answered adequately only in the case, if the transfusiologist knows the exact fine details of the composition and quantitative aspects of blood products.

Blood Banks

[In utero creation of a reno-amniotic shunt for the preservation of a hydronephrotic fetal kidney].

The authors wish to discuss the successful decompression treatment of a serious, fast progressing one-sided hydronephrotic fetal kidney (that also caused deformation of abdominal and thoracic organs) in a 29th week pregnancy. Under the guidance of ultrasonography they created a permanent link between the amniotic cavity in the uterus and the pelvis of the kidney. As a final solution a pyelo-ureteral anastomosis operation was performed on the newborn that was delivered on the 37th gestational week. The baby is 11 months old at the moment and both of her two kidneys are functioning perfectly.

Adult

[Intraoperative sonography in organ preserving operations of kidney tumors].

The use of intraoperative ultrasonography in urology was exclusive in open stone surgery to search for residual stones. However, this procedure may applied successfully to renal cancer, if parenchyma-sparing resection or enucleation should performed. Our experience in 6 cases show the helpful use of intraoperative ultrasonography in the judgement of normal parenchyma.

Adenocarcinoma

[Bladder repair using an ileocecal segment].

Subtotal cystectomy and bladder replacement from shorted-out ileocecal segment as well as uretero-ileostomy was carried out in a 25-year-old woman because of a contracted urinary bladder, consequence of a developmental disorder, i.e. colonic-type mucous membrane of the bladder. The patient is continent passes urine every 3-4 hours without residuum.

Adult

[Combined therapy of malignant testicular tumors].

The 5-year survival of patients with malignant testicular tumors rose during the past 25 years from 10-25% depending on stage to 55-100% after radical castration performed from high inguinal opening, retroperitoneal lymphadenectomy, ultra-tension irradiation and adjuvant chemotherapy. It is stressed that patients diagnosed at stage T1-2, N0-2, M0 may even recover perfectly upon complex therapy. Three to five-year long or even longer survival may be reached with 50% of the patients at the stage T1-3, N1-3, M1. Patients at stage T4, N3-4, M1 die because of the recurrence of the tumor and/or the complication of the adjuvant treatment within 1-3 years and even the interdisciplinary work which is very effective in other stages fails to change this at the present time.

Combined Modality Therapy

Transvesical and transabdominal closure of vesicovaginal fistulas.

Between 1973 and 1985, a total of 46 patients were operated for vesicovaginal fistula. Depending on the extension and localization of the fistula, transvesical or transperitoneal closure was performed. In 12 patients ureteral neoimplantation was made. Fistula recurrence was observed in 6 cases. Based on their favourable experience, authors recommend this surgical solution for the closure of fistulas adjacent to the ureteral orifices and/or for that of larger fistulas.

Adult

Removal of renal tumours from thoracoabdominal incision.

It is stressed that the 5-year survival in the T3 stage renal tumour patients is decisively influenced by the radical removal of the tumorous kidney. It is, however, often difficult to secure radicality. According to the authors' experience, radicality can be ensured by extrapleural throacoabdominal incision also in the case of large renal tumours. This way of exploration combines the advantages of both the abdominal and high lumbar incisions. It facilitates postoperative care by making thoracic drainage unnecessary.

Abdominal Muscles

Technical points in the ablastic radical removal of tumorous kidneys.

Experience from the removal of 250 tumorous kidneys furnished ground for the author to describe a few important technical points of surgery, which emerged in the course of radical nephrectomy, isolation of the renal artery and vein, and removal of tumour thrombus from the vena cava. Radical surgery in the T2-3 N1-2 M0 stage has been found to heighten the chances of five-year survival for the majority of patients.

Humans

Data on renal hypothermia by the venous passage.

The temperature values obtained in renal hypothermia through the venous circulation were biometrically analysed. It was found that there could be even about 4 degrees C temperature differences between the lower and upper poles of the kidney, always as large as 4 degrees C could be elicited in the first five minutes of hypothermia, the lower pole being colder. Similar but smaller differences were observed in ten minutes' hypothermia.

Animals

Pyeloplastic surgery in the presence of suppurative pyelonephritis.

Six out of 135 pyeloplastic operations were performed in the presence of an acute infection. In spite of the inflammatory process the postoperative course was uneventful and the late results were also satisfactory. With the exception of one case, the operation was invariably of the Anderson-Hynes type, with splinting of the ureter and nephrostomy. The anastomosis was prepared with interrupted cat-gut stitches. Three cases are reported in some detail.
