Evaluation of growth and hormonal status in patients referred to the International Fanconi Anemia Registry.

OBJECTIVES: 1) To determine the extent of short stature in patients with Fanconi anemia (FA); 2) to determine the extent and nature of endocrinopathy in FA; 3) to assess the impact on height of any endocrinopathies in these patients; and 4) to study the correlation, if any, between height, endocrinopathy, and FA complementation group. STUDY DESIGN: Fifty-four patients with FA, 30 males and 24 females from 47 unrelated families, were prospectively evaluated in a Pediatric Clinical Research Center. The patients ranged in age from 0.1-31.9 years, with the mean age at assessment 8.6 years. RESULTS: Endocrine abnormalities were found in 44 of the 54 FA patients tested (81%), including short stature, growth hormone (GH) insufficiency, hypothyroidism, glucose intolerance, hyperinsulinism, and/or overt diabetes mellitus. Twenty-one of 48 (44%) participants had a subnormal response to GH stimulation; 19 of 53 (36%) had overt or compensated hypothyroidism, while 8 of 40 participants had reduced thyroid-hormone binding. Two patients were diabetic at the time of study; impaired glucose tolerance was found in 8 of 40 patients (25%), but most surprisingly, hyperinsulinemia was present in 28 of 39 (72%) participants tested. Significantly, spontaneous overnight GH secretion was abnormal in all patients tested (n = 13). In addition, participants demonstrated a tendency toward primary hypothyroidism with serum tetraiodothyronine levels at the lower range of normal, while also having thyrotropin (thyroid-stimulating hormone) levels at the high end of normal. Sixteen patients were assigned to FA complementation group A, (FA-A), 12 to FA-C, and 5 to FA-G; 10 of the 12 participants in FA-C were homozygous for a mutation in the intron-4 donor splice site of the FANCC gene. Patients in groups FA-A and FA-G were relatively taller than the group as a whole (but still below the mean for the general population), whereas those in FA-C had a significantly reduced height for age. GH response to stimulation testing was most consistently normal in participants from FA-G, but this did not reach statistical significance. The tendency toward hypothyroidism was more pronounced in participants belonging to complementation groups FA-C and FA-G, whereas insulin resistance was most evident in patients in FA-G, and least evident in those in FA-C. Short stature was a very common finding among the patients with a mean height >2 standard deviations below the reference mean (standard deviation score: -2.35 +/- 0.28). Patients with subnormal GH response and those with overt or compensated hypothyroidism were shorter than the group with no endocrinopathies. The heights of those participants with glucose or insulin abnormalities were less severely affected than those of normoglycemic, normoinsulinemic participants, although all were significantly below the normal mean. The mean height standard deviation score of patients with entirely normal endocrine function was also >2 standard deviations below the normal mean, demonstrating that short stature is an inherent feature of FA. CONCLUSION: Endocrinopathies are a common feature of FA, primarily manifesting as glucose/insulin abnormalities, GH insufficiency, and hypothyroidism. Although short stature is a well-recognized feature of FA, 23 patients (43%) were within 2 standard deviations, and 5 of these (9% of the total) were actually above the mean for height for the general population. Those patients with endocrine dysfunction are more likely to have short stature. These data indicate that short stature is an integral feature of FA, but that superimposed endocrinopathies further impact on growth. The demonstration of abnormal endogenous GH secretion may demonstrate an underlying hypothalamic-pituitary dysfunction that results in poor growth.

[Injury of the femoral vein--a complication in inguinal hernia surgery].

In a 21-year-old patient with recurrence of the right-sided inguinal hernia the right femoral vein was injured during the operative repair. The vein had to be reconstructed by the use of the right saphena vein and anticoagulative therapy was begun. Since the coagulation parameters were satisfactory the haematoma was produced in the operative wound and had to be evacuated through the second operation. The next operative course was without complications and the patient with healed operative wound was discharged from the hospital at the 13th hospitalization day.

[Evaluation of the clinical status of patients with gastrointestinal hemorrhage and acute pancreatitis using the APACHE II score].

In the group of 35 patients with gastrointestinal bleeding and in the group of 15 patients with acute edematous pancreatitis before beginning of treatment Apache II score was calculated. The obtained score values ranging from 2 to 18 were subsequently compared with the duration of the interval from appearing of the clinical signs to the beginning of treatment, the way of treatment, duration of hospitalization and recurrence of the diseases during hospitalization. The relations were evaluated through correlation analysis and T-test. Although the studied relations were in our conditions statistically insignificant we suppose, that Apache II score contributes to detachment of clinical assessment of patient in correlation to its severity and prognosis.

[Right-sided pneumothorax--a complication of laparoscopic cholecystectomy].

In a 49-years old woman cholecystolithiasis was confirmed by sonography and she was admitted for laparoscopic cholecystectomy. During the operation right-sided pneumothorax developed as a result of the injury of diaphragma by monopolar electrocautery. Operation was converted and the injury was treated by continuous suture. Carbon dioxide was spontaneously resorbed from thoracic cavity during the 5 days following the operation without tube thoracostomy. The operating wound was healed per primam and the patient was discharged on the 14th postoperative day.

[Obstructive ileus--a complication of laparoscopic cholecystectomy].

A 45-year old patient admitted with confirmed calculous pancreatitis underwent the laparoscopic cholecystectomy at the 14th day of the hospitalization. 4 days after the operation he had to be reoperated due to small-bowel-obstruction caused by the herniation of the loop through the supraumbilical port. The small bowel was reduced and plasty of the operative wound was done. Postoperative course was uncomplicated and the patient was discharged from the hospital 6 days after the operation.

Cholecystectomy, Laparoscopic↗

[Personal experience with laparoscopic cholecystectomy].

During the period of 29 months the first 100 laparoscopic cholecystectomies were performed at the Department of Surgery of the Ruzinov Hospital in Bratislava. There were 79 women and 21 men with mean age of 41.5 years (16-67). All operations were modified by the use of only 3 incisions. In 2 patients were the operations converted to open colecystectomy because of perforation of gallbladder in 1 patient because of bleeding. The average operating time was 99.1 minutes (35-225). Intraoperative cholangiography was not carried out in any patient. 2 patients were reoperated because of slipping of the clips from ductus cysticus and 1 patient because of hernia in a scar. All laparoscopic cholecystectomies were planned. The average hospital stay was 5.8 days (3-12). There was no death in our series.

Cholesterol crystals in synovial and bursal fluid.

Cholesterol crystals were found in two patients with classic rheumatoid arthritis (RA). In one patient, cholesterol crystals were found in synovial fluid from both shoulder joints, and in the second they were in an olecranon bursa. To examine the possible systemic etiology of cholesterol crystals in synovial and bursal fluid, lipid concentrations and the presence of serum antilipoprotein antibodies were measured. Antilipoprotein antibodies were not found. The concentration of lipid and lipoproteins, as well as the normal pattern of lipoprotein on agarose gel, eliminates the possibility of hyperlipoproteinemia. Results seemed to exclude a systemic etiology for the formation of cholesterol crystals in synovial and bursal fluid in the RA patients. It appears that several local factors such as defective drainage, local destruction, increased permeability of synovial membrane, and intraarticular (bursal) bleeding are possible etiologies.

[Use of the Apache II scoring system for clinical evaluation of the status of patients with gastrointestinal hemorrhage].

The Apache II scoring system has been used to identify a clinical status of 10 patients, admitted to the intensive therapy unit with bleeding to gastrointestinal tract during the period 1. January - 30. Juni 1992. There were 9 men and 1 woman with mean age of 61.5 years (37-82). Apache II score was calculated before beginning of treatment and 4 times during following 3 days every 12 or 24 hours. The mean score before beginning of treatment was 9.7 (5-17). All the patients were treated exclusively by the conservative therapy with mean length of hospitalization 8, 1 days (4-12). The score during the therapy has had a decreasing tendency and statistically significant decrease came after 72 hours of treatment. Mortality rate at mentioned values was zero. The use of the Apache II scoring system contributes to improvement of clinical judgment of patients with gastrointestinal bleeding.

[Morphologic findings in models of acute experimental pancreatitis].

Morphological changes found in two models of acute experimental pancreatitis are presented. Acute pancreatitis was induced in rats by the method of ligation of the lateral pancreatic ducts and by the method of the blind duodenal loop. After four hours the animals were sacrificed and processed for light and electron microscopic examination. Changes indicative of edematous and necrotizing pancreatitis were recorded. In the electron microscopic picture the predominant findings were disintegration of organelles of acinar cells and the appearance of numerous vacuoles and autophagosomes. The presented models of pancreatitis are considered to be suitable for further research into the pathogenesis and therapeutic intervention of acute pancreatitis in rats. (Fig. 15, Ref. 8.)
