Immunoreconstitution after human bone marrow transplantation.

Immunologic reconstitution was studied in 24 patients who underwent bone marrow transplantation, 17 allogenic and 7 autologous. The GVHD prophylaxis consisted of methotrexate and prednisone. The complete immune evaluation was to be carried out prior to transplantation at 1, 2, 3, 6, 9, 12 months after BMT and subsequently every 6 months up to 4 years. The investigated immunological parameters included total lymphocyte count, B-lymphocytes, T3-, T4-, T8-lymphocytes, T4/T8 ratio, natural killer cell activity, ADCC, lymphocyte blastogenic response and serum-IgG, -IgA, -IgM. Absolute lymphocyte count, B-lymphocytes, T3-lymphocytes recovered to normal levels after 6 months. T4-lymphocytes decreased significantly during the first 180 days posttransplant. T8-lymphocytes increased after 6 months to values higher than normal and the T4/T8 ratio decreased significantly and continued below 0.8 for 48 months. Patients without and with GVHD had low lymphocyte response to PHA and Con A for the first 6 months.

CMV infections in bone marrow transplanted patients--evaluation of prophylaxis with Cytotect (CMV hyperimmuneglobulin).

Cytomegalovirus (CMV) infection is a frequent and clinically important infection following bone marrow transplantation. Candidates for this study were patients admitted for transplantation: 22 patients received bone marrow from a HLA-identical, MCR-nonreactive sibling, in 9 patients an autologous BMT was performed. The anti-CMV IgG (Cytotect) was administered at a dosage of 1 ml/kg on days -7, 13, 33, 53, 73 and 93 after BMT. 5 patients in the very beginning of our BMT program did not receive Cytotect. Patients were given random blood products from the bloodbank not tested for CMV positivity. Active CMV infection or seroconversion in our patients was defined as a rise in IgG titer against the late antigen of fourfold or more or an IgM increase. In the allogeneic BMT group the pretransplant serological status was in 6 cases negative in recipients and donor, in 7 patients positive in recipients and negative in donors, and in 4 patients positive in recipients and donors. Of the 6 patients seronegative in recipients and donors, 3 developed active infection and of the 7 patients pretransplant positive with seronegative donors 3 developed active infection and 4 latent infections during the period from 2 to 100 days following grafting. 1 patient out of the group transplanted in third partial remission of AML developed interstitial pneumonia and died on day +30.4 of the 4 cases with seropositivity of recipients and donors developed active CMV infection. Of 9 patients with autologous transplantation 6 patients were pretransplant seropositive. 3 of these 6 developed active infection and 2 latent infection 30 to 180 days after grafting.(ABSTRACT TRUNCATED AT 250 WORDS)

Cryopreservation of marrow, purging and autologous bone marrow transplantation in childhood.

Since 1984 bone marrow from 42 children with acute lymphoblastic leukaemia, non Hodgkin's lymphoma and neuroblastoma was cryopreserved. In 5 cases (c-ALL, NHL and B type) the marrow was purged by using a cocktail of three monoclonal antibodies (VIL A1, VIB C5, VIB-E3). Up to now 13 children (ALL/10, neuroblastoma/3) were autografted (one of them after purging) after supralethal chemoradiotherapy. Except one child with early death all patients had engraftment: a level of 1.0.10(9)/l leukocytes was reached at days 10-33 (median, 19); platelet level over 60.10(9)/l at days 32-60 (median, 41). 2 children died on treatment related complications, one on infection after full haematological restitution, 2 patients alive with relapse, 8/13 alive in CCR and well.

[Cryopreservation of bone marrow for the autologous transplantation in children].

In preparing the autologous transplantation of children a method for cryoconservation of bone-marrow was developed by means of investigating the donor's bone-marrow. This method is adapted to our conditions, can easily be practised and is cell-preserving. Quantity and quality of the stored bone-marrow cells were evaluated concerning their proliferation capability by means of CFU-c assays. The highest recovery in CFU-c (78%) and cells (98%) was observed if isolated mononuclear cells with cryoprotective addition of 5% DMSO, 20% of human albumin, and 20% of serum were slowly frozen at a controllable rate, stored in liquid oxygen and thawed very quickly. According to the elaborated method the remission marrow was taken from 15 children affected with malignant diseases for autologous reinfusion. The data gained here confirm the experimental experiences.

Diet and headache. Is there a link?

Diet has been implicated as a source of migraine since antiquity. Now, diet is thought of as one of many factors that can trigger migraine. Improved understanding of the migraine process and an attempt to delineate the most common causes of diet-precipitated migraine have resulted in a more rational treatment strategy. We recognize that a small group of patients will have migraine after ingesting certain foods with vasoactive properties or by missing a meal; thus, we suggest a nutritional course to limit this influence. The role of allergy in migraine is not yet determined and remains controversial.

[NK and K cell activity in children with leukemias and aplastic anemias before and after allogeneic bone marrow transplantation].

Mononuclear leukocytes from the peripheral blood and bone-marrow of children affected with aplastic anemia and leukemia were investigated for K-cell activity (antibody-dependent cellular cytotoxicity) and NK-cell activity before and after allogenous bone-marrow transplantation. 51Cr liberation test against murine Graffi erythroblast leukemic cells covered with xenoantibodies and K-562 cells were used for identification. Strongly lowered NK- and K-cell activities could be found in aplastic anemia prior to bone-marrow transplantation. However, NK-cell activity was only lowered significantly in leukemic patients with indication of bone-marrow transplantation. K-cell and NK-cell activities normalised after bone-marrow transplantation. K-cell and NK-cell activities could be observed to be reconstituted very early after bone-marrow transplantation.

Hyperkinetic heart syndrome: the role of the sympathetic nervous system.

The role of the sympathetic nervous system in the hyperkinetic heart syndrome (HHS) was studied in 5 patients. Neither increased basal noradrenaline and adrenaline levels in plasma nor an increased responsiveness of the sympathetic nervous system to various stimuli could be detected in patients with HHS as compared to normal volunteers. An increased sensitivity of adrenergic receptors to catecholamines or an impaired function of the baroreceptor could not be demonstrated. A prominent role of the sympathetic nervous system cannot be postulated.

[(Influence of somatostatin (SS-14) on early dumping reaction in patients after partial gastrectomy) ].

In seven patients suffering from early dumping syndrome after partial gastrectomy (Billroth II operation) an early dumping reaction was provoked by 100 g dextrose per os and the influence of somatostatin-14 (SS-14) infusion on clinical appearance of dumping syndrome was evaluated. SS-14 in contrast to the control trial resulted in total suppression of insulin release and slightly delayed and lowered increase of blood glucose which was not statistically significant, but with SS-14 there was no change in clinical symptoms of dumping reaction and compared to the control test no significant difference was found between the decrease of blood pressure, the increase of pulse frequency, the small increase of hematocrit and plasma free fatty acids.

[Diagnosis of pancreatic function. Simplified screening with fluorescein dilaurate using serum concentration determinations].

Fluorescein-dilaurate has been recommended as a screening test for the assessment of exocrine pancreatic function. In this test fluorescein excretion in the urine is measured for an 8--10 hour period. To avoid urine collection the present study reports on serum fluorescein determinations following oral administration of fluorescein-dilaurate-ester. According to the results of a standard secretin-pancreozymin-test 24 subjects were classified as 10 persons with normal pancreatic function, 7 patients with mild and 7 patients with severe exocrine pancreatic insufficiency. Fluorescein concentrations were measured photometrically after TCA-precipitation. In the normal persons maximal fluorescein levels were observed 4--6 hours after oral load of the test substance. Patients with mild pancreatic insufficiency showed the same pattern and only in patients with severe pancreatic insufficiency much lower serum fluorescein concentrations were found. They were statistically significantly different from normal subjects 3--6 hours after administration of fluorescein-dilaurate.
