Fractures of the radial head and associated elbow injuries in children.

Fractures of the radial head are frequently associated with elbow injuries in children. Thirty-seven patients with this association treated at the Nancy Children's Hospital were analyzed to ascertain the mechanism and the unfavorable factors of prognosis. The mechanism responsible was a valgus strain on the elbow at the moment of impact, associated with forces of compression and distraction. At an average follow-up of 2 years, 76% excellent and good results were obtained. Conservative treatment was always effective in minimally displaced fractures. With surgical reduction by closed intramedullary pinning (CIMP), there were only three failures in severely displaced fractures. With open reduction, results were always poor; 88% patients with fair and poor results had an associated fracture of the olecranon. The unfavorable prognostic factors were the severity of the initial tilting and displacement, the presence of associated elbow injuries, the inadequacy of reduction, and the open reduction procedure.

Detection of naturally occurring enteroviruses in waters by reverse transcription, polymerase chain reaction, and hybridization.

Comparison in virus-seeded mineral water of three detection methods for enteroviruses, direct hybridization, cell culture, and reverse transcription into cDNA followed by polymerase chain reaction and hybridization, showed that the last procedure was 10 to 1,000 times more sensitive than detection by cell culture and 10(5) to 10(7) times more sensitive than direct hybridization. The presence of naturally occurring enteroviruses was also demonstrated in activated sludge and in concentrated and non-concentrated surface water samples by reverse transcription-polymerase chain reaction-hybridization. However, in activated sludge and in concentrated surface waters, enzymatic amplification was sometimes inhibited by contaminants.

Treatment of congenital pseudoarthrosis of the tibia using the Ilizarov technique.

The principle of treatment of congenital pseudoarthrosis of the tibia (CPT) with the Ilizarov method corrects all angular deformity and maximizes the cross-sectional area of union of the pseudoarthrosis. Fifteen patients with a total of 16 CPT were treated using the Ilizarov apparatus. Various forces were used to treat the pseudoarthrosis site including compression, distraction, open reduction, resection and shortening, resection and bone transport, and invagination of one end in the other. Lengthening was performed in 12 of the 16. Deformity was corrected in all cases. The union rate was 94% with one treatment and 100% with two treatments. There were five refractures, three early and two late. Previous pin sites, residual angular deformity, and natural history were considered predisposing factors for refractures. One patient refractured twice but remained ununited. Fifteen remained united, with a mean follow-up period of four years (range, two to seven). There were two residual deformities, one in the regenerate and one at the level of the CPT.

Systemic artery-pulmonary vein fistula without sequestration.

Fistulae between an artery arising from the aorta and a pulmonary vein associated with absence of a branch of the pulmonary artery, but without anomalies of the bronchial tree, are exceedingly rare. We report a case in a child who presented with a continuous vascular murmur and developed heart failure due to the left-left shunt. Left lobectomy was performed and showed that the entire left lower lobe was occupied by a huge arteriovenous fistula between an artery stemming from the aorta and the pulmonary veins.

[Conservative surgery for osteosarcoma in children under 8 years of age].

Recent progress is chemotherapy for treatment of malignant tumors of the skeleton permit conservative treatment in many cases by resection-reconstruction rather than amputation. The problem remains acute and divided in young children because of considerable intramedullary tumoral extension and other requirements due to growth. The authors comment their experience of femoral osteosarcoma in 3 different methods of conservative treatment: 1 Creation of a progressive regenerate by Ilizarov's lift technique previously experienced on animal. 2 Total replacement of the femur with hip and knee arthroplasty using Dr Delepine massive growth prothesis. 3 Subtotal femoral replacement, sparing the superior and inferior growth plates, by a massive homograft fixed by a locked nail. The authors discuss the advantages and difficulties of each method with regards to literature and with their own experience.

[Ilizarov's method. Histological and radiological aspects].

The Ilizarov method is an original technique of surgical lengthening of long bones. The constraints in compression and distraction obtained with this method favor the bony healing. The straining of extension induces stimulating consequences on the bone. The healing process is different from that of the fractures because the bony regeneration is accomplished by a membranous bony mechanism without intermediate fibrous or cartilaginous medium. The maturation occurs when the lamellated bone is present, at the end of the 4th month.

[A mucoid pseudo-cyst of the peripheral nerve. Apropos of a surgical case].

The authors report a case of a twelve old young man, operated on for a ganglion cyst of the common peroneal nerve. The lesion is rare, and is characterized by an accumulation of a mucoïd substance in the sheath, compressing progressively the fibres of the nerve. The case was revealed with pain, paralysis and tumefaction of the nerve. The pathogenic is uncertain. An early treatment, consisting of evacuation of the cyst and interfascicular neurolysis through the dissecting microscope, obtains usually a good functional result.

[Arm lengthening in children. Report of 4 cases].

The principle of osteogenesis under stretch elaborated by Pr. G.A. Ilizarov around 1970 comfort our decision for limb lengthening for it does not need in most cases hemorrhagic operations nor use of bone grafts. Pathological shortening of the humerus may be due to various causes: monomelic dystrophy due to epiphysiodesis of humeral superior growing plate with or without humerus varus, sequelae of neonatal septic osteoarthritis of the shoulder joint are the more frequent etiologies. On the other hand, the fibrous dysplasia, osteochondromatosis (Ollier's disease), sequelae to trauma or iatrogenic etiologies are infrequent. Resulting length discrepancy are very important from 11 to 15 cm and more when growth is achieved. Functional disorders are delayed as compared to the lower limb for each upper limb acts independently to each other. However above 6 cm discrepancy coordinated movements become more difficult or impossible especially above 8 cm discrepancy. The authors reported 4 cases of arm lengthening concerning teenagers by Ilizarov's technique with a discrepancy between 10 to 14 cm. One of the humerus was previously treated by Wagner's technique which allow us to make a comparative analysis of both methods. Technical instructions, incidents and accidents occurring during lengthening, healing delays and functional and esthetic results are discussed in recent publications on this subject.

[Axial correction of the legs of children using the Ilizarov method. Apropos fo 22 cases].

22 axial corrections of limbs in children were performed using the Ilizarov technique between 1984 and 1986. This prospective study has been performed under the auspices of the Association pour l'Etude Systémique et l'Application de la Méthode d'Ilizarov en France (ASAMIF). Total correction of deformity was achieved in 20 cases. A minimal angulation (less than 5 degrees) was observed in two cases. In 9 cases tibia or femoral lengthening was performed simultaneously. After corticotomy and placement of the stimulant effect of limb distraction. The mean lengthenings achieved was 30 mm. Serious complications were are: there was one case of fracture of the regenerating bone. We deplore only two superficial pin track infections, two cases of rarefaction of bone. The bone consolidation was obtained on an average of two months. If inequality was associated (more than 40 mm) technique of double corticotomy was performed. The best and more results were certainly performed in great deformities. On the other hand the Ilizarov method addresses all inequalities of leg length, whatever the aetiology, who are associated with common deformity.

[Centro-medullary nailing in fractures of the upper end of the humerus in children and adolescents].

The authors report a series of 55 fractures of the upper extremity of the humerus in children and adolescents surgically treated by elastic intramedullary nailing. The place of this surgical technique is evaluated in the general treatment of these fractures, which have the prognostic always favorable. The results, with a 1 year follow-up, are all excellent or good on the functional and anatomical plan. The cases of mal union have been rare and the correction is obtained with growth. This way be explained by the capacity of remodeling of the upper humerus. The treatment of fractures of the upper extremity of the humerus is first orthopaedic in children and adolescents, but the osteosynthesis by elastic intramedullary nailing can represent a surgical method of choice in the fractures which are displaced, unstable or which need a thoraco-branchial plaster.

[Stable elastic nailing].

The authors illustrate an original method of osteosynthesis concerning shaft fractures in children: Stable Elastic Nailing. They elaborate the physiopathological basis and specify all details of this technique. The results of this treatment as well as its limits are considered through personal experience of 217 femurs, 42 tibias, 36 humerus, 84 fore-arms and 96 supracondylar fractures of the humerus. We essentially proceeded for a comparative analysis with other methods of osteosynthesis or conservative treatment, illustrating the net superiority of stable elastic nailing in their indications according to the recommended technique.

[Surgical treatment of spinal deformity due to cerebral motor disorders using a Luque type appliance].

The authors report a series of 20 cerebral palsy patients with spine deformities who need a surgical treatment. The procedure is a posterior approach and a segmental spine instrumentation: Luque-Galvestone or Cotrel-Dubousset-Luque type. Vertebral bone-graft is always performed and sacral bone is fused 16 times. Sitting position is authorized 4 to 7 days after the operation. Improvement of frontal and sagital curves as well as pelvic obliquity is of 50% and remains unchanged with a two years follow-up. Among the complications, most of them are pulmonary infections with 2 severe cases on 9 patients. Even functional status is also improved, post-operative pains and social difficulties are frequent during almost 6 months.

[Congenital pseudarthrosis of the legs and the Ilizarov method. Apropos of 4 cases].

Four cases of congenital pseudarthrosis were treated by the Ilizarov method and consolidation obtained by 3.5 months in three patients who had all undergone several previous surgical procedures. Although the length of follow up is short (maximum 3.5 years) consolidation occurred quicker than by traditional methods. This technique also permitted both simultaneous correction of axial malalignment and leg lengthening. We think that this method has a place as a salvage procedure except where the distal fragment is very thin or dystrophic.

[Fractures of the pelvis and associated bladder-urethral rupture].

We report 82 cases of pelvic fractures associated with vesico-urethral ruptures. Pelvic fractures were 23 simple fractures, 5 bilateral obturator foramen's margin fractures, 30 pubic dislocations sometimes associated with other fractures and 24 complex fractures. Urological injuries were 64 urethral ruptures, 13 bladder ruptures and 5 vesicourethral injuries. These well documented cases allow us to study relations between bone and visceral injuries as well as result's quality in relation to treatment. From this study a treatment schedule has been deduced insisting upon the fact these bone and visceral injuries must be treated immediately and if possible in a one time procedure. The repairs time schedule depends upon the fracture's type from which benefit can be taken out for the urethral access.

[Early excision and grafting in the burned child. Apropos of 49 cases studied retrospectively 1986-1987].

Early excision and grafting (EEG) is a commonly-used surgical technique which limits the risk of infection and improves healing. However, when used on children, their particular anatomic and physiological specificities must be taken into account. The authors present a retrospective investigation concerning 49 EEG performed on 45 patients from five months to nineteen years old, presenting a burnt skin surface of between 1% and 70%. This short operation (1 h 30) gives rise to an important and early haemorrhage induced by blood losses which are impossible both to stop and to evaluate. This explains the frequency and the severity of the peroperative complications (one cardiocirculatory deficiency, eleven low blood pressure/tachycardias) related to an underestimated blood loss. The post-operative period is characterized by hemodynamic, infectious and hypothermic complications. As regards operative results, they can be considered to be good or excellent in 80% of the cases both from a functional and an anaesthetic point of view. In order to decrease the important operative morbidity related to EEG in the case of children, the authors stress the necessity or taking certain precautions: During the operative period --Antibiotic prophylaxis against cutaneous bacteria when the excision is important and BSS greater than 20%; --ordering sufficient quantities of blood and plasma taking into account the estimated amount of blood losses during the per and post-operative period, and also with regard to the patient's weight and the excised (AE): During the operation --prevention of hypothermia by the permanent use of an infrared radiator, warming up the solutions before perfusion, using of a heating mattress...); --provision of two large diameter venous passages; --initiation of the transfusion before the first incision.(ABSTRACT TRUNCATED AT 250 WORDS)
