[The effect of progressive incremental exercise on some parameters of hemostasis].

In 18 sportsmen the platelet count, thrombin time, prothrombin time, activated partial thromboplastin time (APTT), cloting factors (F):I, VII, VIII and fibrin/fibrinogen degradation products (FDP) were measured before, immediately after progressive incremental exercise--and 30 min later. All post-exercise changes of the values were corrected for the plasma volume changes, which were calculated from hematocrit values. Platelet count, thrombin time, prothrombin time FI and FVII did not change significantly after exercise. Immediately post-exercise APTT was significantly shortened, but FVIII and FDP were significantly elevated. 30 min later FDP level normalized but FVIII and APTT changes persisted. The obtained data show that the applied physical exercise caused shift of the existing balance between coagulation and fibrinolysis towards hypercoagulability. They also confirm the necessity of taking into account post-exercise hemostasis changes in cardiological rehabilitation and endurance training in adults.

[Serum concentration of prolactin, gastrin and glycocholic acid in patient with peptic ulcer treated with ranitidine].

In 66 patients with peptic ulcer (11 with gastric ulcer, 55 with duodenal ulcer, 19 women, 47 men) the serum concentrations of prolactin, dehydrocholic acid and gastrin were determined. The studies were repeated after treatment with ranitidine: in 50 patients after three weeks and in 40 patients after another 30 days. During the first period ranitidine 2 x 150 mg was administered, while during the second period the dose was 1 x 150 mg. The results were compared with those obtained from 120 healthy subjects. Before starting the treatment prolactin levels were significantly higher than those in the control group. During the treatment a significant decrease of the levels was observed. Similar changes of prolactin concentrations were found in the group of 39 men with duodenal ulcer isolated from the studied patients, who were compared with a group of 50 healthy men. It was not found that the development of peptic ulcer and the treatment with ranitidine exerted and effect on the changes of gastrin and dehydrocholic acid concentrations.

[Difficulties in diagnosis of cytomegalovirus mononucleosis syndrome].

A case of a healthy 23-year-old woman is reported with cytomegalovirus mononucleosis as a result of infection of cytomegalovirus probably primary. The patient presented with symptoms of generalized adenopathy, migratory arthralgias and arthritis, hepatosplenomegaly, long lasting rash as well as complications of pneumonia and myocarditis. Because on histopathological examination of lymph node the Hodgkin-Reed-Sternberg-like cells were found a misdiagnosis of Hodgkin's disease was initially made. After about 8 weeks period there was a complete recovery. The current problems related to cytomegalovirus infection are presented.

[Changes in levels of glucose, insulin, c-peptide, glucagon and potassium in serum of patients with bronchial asthma during stimulation with b2-adrenergic fenoterol].

The study was carried out on 40 patients with bronchial asthma (20 with atopic and 20 with non-atopic) and 20 healthy subjects as controls. After fenoterol stimulation in asthmatic and control groups an increase of glucose level and decrease of potassium concentration were documented. Moreover, a slight increase of insulin, C-peptide and glucogene++ release was suggested.

[Monoclonal IgG lambda protein in a patient with hereditary spherocytosis].

Three cases are presented of hereditary spherocytosis that occurred in one family. In one of those cases the presence of monoclonal IgG lambda protein was stated. This protein's level has not increased for 8 year of observation. The latest pathogenetic theories of hereditary spherocytosis are presented as well as the possibility of its coexistence with other diseases.

[Glucose-6-phosphate dehydrogenase deficiency in erythrocytes observed in two cases].

In two male patients the deficiency of G-6-PD in erythrocytes became manifested in 27th year of age with severe hemolytic crisis. In the first case hemolysis appeared after three days of heavy alcohol drinking. There was no evident relation between alcohol and hemolysis. In the second patient (an Iraqi Student) suffering from diabetes since adolescence, hemolysis was provoked by respiratory tract infection, antiinflammatory drugs and concomitant ketosis. Pneumonia, renal insufficiency and acute pancreatitis complicated the course of illness. The patient required a consecutive erythrocyte concentrate transfusions. Aplastic crisis was treated with folic acid.

[A case of functional hyperprolactinemia].

A case is presented of a 27-year-old woman in whom galactorrhoea and amenorrhoea occurred simultaneously with the signs of acute neurosis. The determination of prolactin concentration after the administration of metoclopramide made possible the diagnosis of functional hyperprolactinaemia. Other causes of hyperprolactinaemia were excluded. The signs and symptoms regressed completely after several months of treatment with parlodel. Control investigation done after 18 months showed that the concentration of prolactin after administration of metoclopramide was slightly lower than before, but was still high.

[Results of epidemiologic studies performed after the disaster in Czernobyl among the adult part of the population in the region of Krakow].

Epidemiologic studies following the Czernobyl accident were performed in region Kraków, including Kraków, Nowy Sacz and Kielce district. 1426 males and 2495 females were selected according to the random sample on the whole population of Kraków and Nowy Sacz, as well as in some selected areas in Swietokrzyski Mountains, and in Kielcecity. The aim of the study was to assess the results of the prophylaxis with Kalium iodine after the radiation and the incidence of the goiter in the population. It was stated, that 19.2% of the population in Kraków district, 16.9% in Nowy Sacz and 20% in Kielce received the prophylactic dosis of K.J. 80% took mainly the Lugol solution, between May, the 1st and 5th, 1986. Among 18 of person showing side effects like gastrointestinal disturbances, 16 were of female sex. Goiter incidence according to WHO classification was 50.7%, 67.3% and 49.9% in Kraków, Nowy Sacz and Kielce respectively. The difference between the incidence of goiter in males and females was 1:3. In women it was rather Ist and IInd degree of goiter, in men OB and Ist. Nodules of thyroid gland in the rural region of Kraków, Nowy Sacz and Kielce were seen in women in 10.8%, 1.7%, add 12.3% consecutively. Hormonal studies i.e T3, T4, TSH serum concentration showed normal results in all groups studied. TSH concentration was the highest in the group OB. The microsomal and antithyroglobulin antibodies level was the same independently on the prophylactic dosis of Lugol solution. The high incidence of thyroid diseases not related to the accident was observed.(ABSTRACT TRUNCATED AT 250 WORDS)

[Behavior of thyrotropic, triiodothyronine, thyroxine and cortisol hormone levels in blood serum of patients with duodenal ulcer treated with ranitidine].

Blood serum concentrations of thyrotropic hormone (TSH), triiodothyronine (T3), thyroxine (T4) and cortisol have been measured in 50 patients with duodenal ulcer. The determinations were repeated after a specific time of treatment with ranitidine: in 45 patients after 3 weeks and in 37 after additional 30 days of treatment. During the first period of treatment ranitidine was administered at a dose of 300 mg divided into two daily doses and during the second period of treatment at a single daily dose of 150 mg. A small but statistically significant changes, an increase in T3 and a decrease in TSH concentration, were observed before the treatment. No changes concerning these two parameters were found as an effect of the treatment. A decrease in the concentration of T4 was found, on the other hand, after three weeks of ranitidine administration. This decrease persisted after further 30 days of chronic treatment with ranitidine. No significant changes concerning blood serum cortisol concentration were found in any of the studied groups.

[Triiodothyronine (T3) and thyroxine (T4) levels in patients with bronchial asthma].

In 212 patients with bronchial asthma and 115 healthy controls the serum levels of T4 and T3 were determined. In patients not treated with corticosteroids a statistically significant rise of T3 and fall of T4 were noted. These changes were non-significantly smaller in the patients with pronounced dyspnea . During long-term corticoid++ therapy a tendency was observed for normalization of these changes.
