[Intrathyroidal metastases of a nephroepithelioma. Reflections on 5 operated cases].

Out of 3,340 thyroidectomies (237 for cancer), the authors report 5 cases of thyroid metastases due to clear cell renal cancer. They emphasize the slow development of these metastases. Two cases were operated on 11 years after nephrectomy and one after 27 years. In one case, an intrathyroid metastasis led to the discovery of renal carcinoma. In 3 cases, evolution was rapidly fatal (within the first year). One patient is still alive 6 years after thyroidectomy. The authors emphasize clinical grounds for diagnosis and expose pathology findings that favor longer survival when there is early removal of intrathyroid metastases.

Proteinases and sialyltransferase in human breast tumors.

Proteolytic and sialyltransferase activities were determined in extracts of 65 human primary breast tumors, 6 lymph node metastases, 6 fibroadenomas and 27 normal tissues. Using proteins and synthetic selective substrates, we observed the presence of collagen-peptidases, plasminogen activator, cathepsin-B and cathepsin-D-like enzymes, and sialyltransferase. No active or trypsin-activatable type-IV collagenase activity was detected. Although individual variations between tumors were large, proteinase and sialyltransferase contents were significantly elevated in malignant breast tissues. Enzyme activities were found to be related to the epithelial volume of the tumor. No significant correlation was found between the proteinase or sialyltransferase activities and the degree of differentiation of the tumor cells, or the degree to which tumors had metastasized to regional lymph nodes. Since large variations of enzyme levels apparently reflect the heterogeneity of epithelial cell densities in tumor samples, proteolytic or sialyltransferase activities cannot therefore be used as a measure of quantitative evaluation of invasive properties in breast cancer.

[Chondrosarcoma of soft tissue. Apropos of a case].

This article reports a case of soft-tissue chondrosarcoma in the subcutis of the right supraspinal fossa of an 18-year-old woman who had undergone irradiation in infancy. A review of 123 published cases of extraskeletal chondrosarcoma raises the question of whether or not these tumors are really malignant in patients other than children. Their relationship with previous irradiation is also examined.

[Multinucleated giant cell tumor of the thyroid: an unusual anaplastic carcinoma (author's transl)].

Neoplasms of the thyroid with multinucleated, osteoclast-like giant cells, are rare. The numerous giant cells observed in these undifferentiated neoplasm have extensive eosinophilic cytoplasm and contain two to several hundred round or oval, uniform small nuclei. The clinical and morphologic data obtained from the two cases we describe and seven others collected from the literature are as follows: a) peak incidence in late adulthood; b) frequent association with a preexisting goiter (6 cases); c) rapid growth and fatal outcome within one year following diagnosis (5 cases); d) histologic pleomorphism with a background of uniform and spindle- or bizarre-shaped mononuclear cells; e) occasional coexistence of multinucleated giant cell tumor with well differentiated follicular carcinoma (1 case). Indirect evidence suggests that the multinucleated giant cells which characterize these unusual anaplastic carcinomas are epithelial in origin.

[Sarcoma of the breast].

Reports of 4 personal cases of breast sarcomas (3 cases of cystosarcoma phyllodes and 1 angiosarcoma), the authors review the literature concerning these rare tumors which account for less than 1% of malignant tumors of the breast. Whilst the usual clinical picture is that of a large tumor deforming the breast with pseudo-inflammatory signs, sarcomas 1 to 3 cm in diameter were discovered and treated with good chances of recovery. The natural history of breast sarcomas is marked above all by the risk of hematogenic metastatic spread (30% of cases) whilst very few (5%) local recurrences are seen following surgical treatment by mastectomy. Emphasis is placed upon problems of histological diagnosis of the variety of sarcoma and the demonstration of histoprognostic factors which can be used to predict the outcome (mitotic activity, grading) showing evidence of good correlation in the series analyzed. The various characteristics of cystosarcoma phyllodes are described emphasizing the conditions surrounding its development, relations with benign phyllodes tumors. The particular severity of this sarcomatous form in the very young woman is mentioned. Prevention lies in the sufficiently extensive excision of the phyllodes tumor. Mesenchymatous sarcomas of the breast are also accompanied by a fairly favorable prognosis (5 year actuarial survival of 50 to 80% according to series). By contrast, two special and rare histological varieties, angiosarcoma and lymphosarcoma, are characterized by a much more gloomy prognosis and require different treatment including radiotherapy and chemotherapy.
