From 1964 to 1985, 120 patients with primary liver carcinoma were treated by operation in our hospital. Regular hepatectomy was done in 7 patients, palliative irregular hepatectomy in 28 and radical irregular in 85. The operation mortality was 4.2% in irregular hepatectomy group (113 cases) but 14.3% in regular hepatectomy group (7 cases) (P greater than 0.05). The 1, 3 and 5 year survival rates were 68.8%, 48.1% and 20.0% in radical irregular hepatectomy group but 83.3%, 33.3% and 16.7% in regular hepatectomy group. 10 of 28 patients treated by palliative hepatectomy were added with radiation. Majority of these patients died in 1 year after operation but 2 patients survived for more than 2 years and 1 for more than 7 years. The data show that in Asia, the incidence of primary liver carcinoma concurrent with liver cirrhosis is high and irregular hepatectomy is a suitable treatment. There is no difference between irregular and regular hepatectomy groups in the prognosis. But the former could reduce the operative time, mortality and the possibility of bleeding and complications.

Clinical and experimental evidence shows that hepatic artery ligation (HAL) is an effective method for treatment of nonresectable primary liver carcinoma (PLC). Improvement of subjective symptoms in 50-70% patients has reported, although effective duration is short and 30-50% of the enlarged liver could be decreased. Usually, arterial collateral of the tumors after HAL may be evident again in 4-6 weeks. There is arteriographic evidence of the appearance of collateral flow as early as the fourth days after HAL. HAL combined with irradiation was carried out in 17 cases between October 1978 to October 1982 in order to improve the prognosis of nonresectable PLC.

From 1964 to 1978, 313 patients with soft tissue sarcoma were treated in our hospital by resection and followed over 5 years. 192 patients were males and 121 were females. The sex ratio was 1.5:1. In the 190 cases who developed postoperative recurrence, 55 were in the head and neck, 136 in the trunk and 122 in the limbs and 23 (7.4%) as metastasis in the lymph nodes. By pathology, the common types were fibrosarcoma (86 cases), neurofibrosarcoma (66 cases) and rhabdomyosarcoma (32 cases). All the 313 patients were treated by resection, the overall 5 and 10 year survival rates were 47.6% and 35.8%. The 5 year survival rate was 47.9% in the 117 patients treated by local resection, 62.7% in the 75 by extensive resection, 18.6% in the 43 by amputation and 48.7% in the 78 by combination of resection and irradiation. According to the UICC TNM staging system, the 5 year survival was 60% in stage I, 46.9% in stage II, 40% in stage III and 22.2% in stage IV. In this series, discussion of the treatment modalities and prognosis factors in emphasized.
