[The orthopedics-traumatology department].

The Department is involved in clinical and research activities. We pioneered the clinical application of "smart of Orthopaedic implants". The monitoring concept, bringing a high added value, helps in assessing fracture healing or implant fixation. The technique, featuring the 21st century, also requires advanced knowledge in biomaterials. Such implants have been used during the ESA Parabolic Flight Campaigns. These measurements are a prerequisite to establish prevention programs of the bone demineralisation during space flights but also in disuse osteoporosis. The Department is deeply involved in external fixation. In vitro studies of the mechanical insertion and holding parameters of external fixation pins have been performed to produce new implants, which efficiency has been proved by a clinical study. We defined original clinical principles of external fixation as its use in upper limb indications, to maintain microsurgery transplant, preserving devascularization and providing adequate fixation. Our biomechanical approach of the upper limb, now confirmed by others research groups, questioned old clinical concepts. The isokinetic evaluation assesses the dynamic function of articulations, i.e. after arthroplasty, and is also used for medico-legal evaluation. The clinical research touches orthopaedic (or systemic) diseases as algodystrophy, Kienböck disease, Kashing Beck. We conducted a multidisciplinary research on the effect of electromagnetic fields (EMF) on cells differentiation. In addition to possible healing properties, it helps as an objective support to analyse the exposure to the environmental EMF.

[Study of cotyloid anteversion by coxometric measurement].

A coxometric evaluation is helpful for the diagnosis and the prognosis of hip dysplasia. These measurements also given an intrinsic guide to the surgeon for total hip arthroplasty. A coxometric protractor is drawn on the majority of the goniometers and allows the measurement of the angle of internal and external roof, the angle of the acetabular roof obliquity and the femoral neck-shaft angle on a hip AP X-ray. The purpose of this report is to demonstrate that on the same X-ray, the adequate placement of the coxometric protractor allows to calculate the inclination angle, the acetabular anteversion angles and the anterior roof angle.

Radiological and histological improvement of oxalate osteopathy after combined liver-kidney transplantation in primary hyperoxaluria type 1.

A 15-year-old patient with severe bone disease (with bilateral fractures of hips and shoulders) due to primary hyperoxaluria type 1 (PH1) was treated with combined liver-kidney transplantation after a 4-year hemodialysis period. Normalization of excessive oxalate synthesis brought in by the liver graft combined with the slow excretion of skeletal oxalate stores by the renal graft led to progressive improvement of clinical, radiological, and histological evidence of oxalate osteopathy. This allowed bilateral hip replacement 3 years after transplantation, which led to complete physical rehabilitation of the crippled patient. Combined liver-kidney transplantation constitutes the treatment of choice for end-stage renal failure due to PH1, even in the face of severe oxalate bone disease.

Single stage reconstruction of a large tibial defect using a free vascularised osteomyocutaneous ulnar transfer.

We present the case of a young man with 13 cm of bone and soft-tissue loss in the tibia and a severe traction injury of the brachial plexus. A free vascularised composite transfer of the bone and soft tissues of the ulnar side of the forearm was undertaken to reconstruct the defect. Bony union was achieved after a year and was followed by complete functional recovery of the lower limb.

Osteoarticular amyloidosis associated with haemodialysis: an immunoultrastructural study.

Osteoarticular amyloidosis occurred in a patient receiving long term haemodialysis. Histological examination showed that the amyloid deposit was surrounded by inflammatory cells and macrophages filled with haemosiderin. Electron microscopy showed that the amyloid fibrils were in close contact with cytoplasmic expansions, or located in intracytoplasmic pockets of the infiltrating cells. Immunohistological and immunoultrastructural observations confirmed that beta 2-microglobulin was a major constituent of amyloidosis associated with dialysis. Amyloid P component was also detected within the amyloid deposits. These findings suggest that amyloid P component, iron overload, or macrophage derived factors could have a role in the polymerisation of beta 2-microglobulin into amyloid deposit.

Skeletal involvement in sarcoidosis: a case report.

A patient is described in whom sarcoidosis caused a pathological fracture of the middle phalanx of the little finger. A bone scan showed increased uptake. She was treated by resection of the diseased area and bone grafting, which led to bone healing. The bony manifestations of sarcoidosis are reviewed.

[The hip in sickle-cell anemia and its surgical treatment. Apropos of 4 cases].

The authors report 4 cass of hip involvement in sickle-cell anaemia. The patients were aged, 3, 12, 18 and 24 years and presented with the appearances of avascular necrosis of the femoral head. Three had a femoral osteotomy with a good result. A review of the literature was made to determine the pathogenesis, epidemiology and classification into 3 groups. The authors consider that osteotomy acts by diminishing pressure in the area of bone necrosis. The eventual need for total hip replacement is discussed.

[Parenteral feeding after total gastrectomy (author's transl)].

The authors report on 2 cases of radical gastrectomy for cancer. Parenteral feeding composed of amino acids, solutions of lipids and carbon hydrats insures an intake of 2,500 to 4,000 calories per day in the immediate postoperative phase. Daily nitrogen and measurement of the different blood and urine parameters make it possible to estimate the daily needs of these patients and to provide for a smooth recovery.

[Chinese flaps].

The chinese forearm flap is vascularized by the radial artery. It can be used as an island transplant or as a free flap. The medial plantar island flap has also been called chinese flap. Advantages and disadvantages of these flaps are discussed in an attempt to summarize their indications and contraindications.
