Dynamic bending tolerance and elastic-plastic material properties of the human femur.

The objective of this study was to provide data on the structural tolerance and material properties of the human femur in dynamic bending. Fifteen (15) isolated femurs from eight (8) males were tested in either posterior-to-anterior or lateral-to-medial three-point bending. The failure moment was 458 +/- 95 Nm and did not differ significantly with loading direction. A method was developed to estimate the elastic-plastic material properties of the bone using both force-deflection data and strain gauge measurements. The bone material appeared to yield at about one third of the ultimate strain level prior to fracture. It is hoped that these data will aid in the development of injury criteria and finite element models for predicting injuries to pedestrians and vehicle occupants.

Retrospective analysis of malleolar fractures in an impact environment.

To protect against malleolar fractures in frontal crashes it is important to understand the mechanisms of injury. We have investigated the accuracy of Orthopaedic Specialists in deducing the injury mechanisms of experimentally generated malleolar fractures from radiographs; and the applicability of classic descriptors of injury mechanisms, such as the Lauge-Hansen classification system, in analysing impact induced trauma. Orthopaedic Specialists did not consistently deduce the mechanism of ankle injuries suggesting there may not be a unique fracture pattern for every injury mechanism and that the Lauge-Hansen classification system does not reliably describe ankle fractures created in the impact environment.

Biomechanical evaluation of early fracture healing in normal and diabetic rats.

Diabetes mellitus has been shown to alter the properties of bone and impair fracture healing in both humans and animals. The objective of this study was to document changes in the structural and material properties of intact bone and bone with healed fractures in diabetic rats compared with nondiabetic controls after 3 and 4 weeks of healing. Rods were inserted in the right femurs of control rats and rats with streptozotocin-induced diabetes, and the femurs were fractured in a standardized procedure and then allowed to heal for 3 and 4 weeks. After death, all femurs were mechanically tested to failure in torsion. The degree of healing was quantified for each animal by normalizing mechanical parameters for the femur with a healed fracture with those for the intact contralateral femur. At both time points of healing, diabetic rats exhibited inferior healing compared with that of control animals in terms of failure torque, failure stress, structural stiffness, and material stiffness of the femur with the healed fracture relative to the intact contralateral femur (p < 0.05). Our results demonstrate that the recovery of structural and material strength in femurs with healed fractures in diabetic rats is delayed by at least 1 week compared with that in controls.

Linear and quasi-linear viscoelastic characterization of ankle ligaments.

The objective of this study was to produce linear and nonlinear viscoelastic models of eight major ligaments in the human ankle/foot complex for use in computer models of the lower extremity. The ligaments included in this study were the anterior talofibular (ATaF), anterior tibiofibular (ATiF), anterior tibiotalar (ATT), calcaneofibular (CF), posterior talofibular (PTaF), posterior tibiofibular (PTiF), posterior tibiotalar (PTT), and tibiocalcaneal (TiC) ligaments. Step relaxation and ramp tests were performed. Back-extrapolation was used to correct for vibration effects and the error introduced by the finite rise time in step relaxation tests. Ligament behavior was found to be nonlinear viscoelastic, but could be adequately modeled up to 15 percent strain using Fung's quasilinear viscoelastic (QLV) model. Failure properties and the effects of preconditioning were also examined.

Tibial osteotomy.

Tibial osteotomy is the procedure of choice for young adults with disabling unicompartmental osteoarthritis of the knee. It is a conservative precursor to total knee arthroplasty. This article presents an overview of the indications for the surgery and the biomechanical rationale for the procedure. The immediate postsurgical and subsequent rehabilitative care of the patient is discussed, as well as implications for discharge planning.
