[Indications for tonsillectomy and adenoidectomy: consensus document by the Spanish Society of ORL and the Spanish Society of Pediatrics].

Tonsillectomy and adenoidectomy are probably the commonest surgeries performed in the ENT field as well as the most controversial ones. There are very few consensus documents available for these two surgeries. In 1997 a document written by the two mentioned Societies was published, in order to update such document regarding tonsillectomy and adenoidectomy procedures we have met this year representatives from both scientific societies and a new document has been elaborated. We describe the diagnostic criteria of pharyngo-tonsillitis and adenoiditis as well as of obstructive sleep apnoea syndrome, with the aim of a better comprehension of these processes when a decision needs to be made regarding surgery. Indications and contraindications of tonsillectomy and adenoidectomy are here described.

[Bilateral aneurysm of the internal carotid artery and thrombosis of the jugular vein. Case report].

Extracranial carotid artery aneurysms is a very odd pathology which first sign usually is a neck mass. At present the arteriosclerosis is considered its main source and surgery is promoted as its regular treatment, but each case must be managed according the distinct peculiarities showed. This paper reports the clinic and radiological features of a woman diagnosed of bilateral carotid aneurysm with the addition of a thrombosis check afterwards of the left jugular vein. Because of the rarity of this case think justified the record. A bibliographical perusal of this singular entity accompanies the article.

[Otitic hydrocephalus. A report of two cases].

Otitic hydrocephalus is one the less common intracranial complications of infectious ear disease, consisting of elevated intracranial pressure without focal neurologic abnormalities other than those due to the elevated pressure. Two representatives cases is discussed in this report, followed by a review of the literature. The clinical course, diagnosis, treatment, and prognosis are outlined. The etiology and the association with lateral sinus thrombosis remains obscure.

[Neck phlebectasia].

Neck phlebectasia is an infrequent anomaly of the venous system, possibly a congenital condition, becoming apparent as a neck mass triggered by the Valsalva maneuver or any other exploratory afford with closed nose. Commonly localized in the jugular venous system, although it can turn-up, the ectasia, to other cervicofacial veins. In this paper are reported 3 neck phlebectasis including one of them affecting the anterior facial vein, occurrence exceedingly uncommon, of which case we don't know even a single reference in the literature. We make a perusal of the subject's bibliography and also discuss the clinic, diagnostic and therapeutical most important features of this entity.

[Twenty years of retrospective study of malignant paranasal sinus tumors].

Carcinomas of the nasal cavity and paranasal sinuses represent 3% of tumors of the head and neck. Their histology and location vary widely. We reviewed the case histories of patients with malignant tumors of the nasal sinuses seen by us between 1975 and 1996, a total of 37 suitable cases. The site of origin of the tumor was the maxillary sinus in 16 cases, ethmoid sinus in 15, nasal septum in 3, nasal roof in 2, and nasal floor in 1 case. Fifty percent of the tumors were squamous cell carcinomas, 11% undifferentiated carcinomas, 13% non-squamous carcinomas, 5% melanomas, 5% olfactory neuroblastomas, 3% chondrosarcomas and 8% non-Hodgkin lymphomas. When diagnosed, one patient had carcinoma in situ, 3 stage I, 5 stage II, 12 stage III and 16 stage i.v. The cumulative survival was 63% after 3 years and 47% after 5 years.

[Trans-septo-sphenoidal transnasal approach for microsurgery of hypophyseal tumors].

The surgical treatment of hypophyseal tumors has improved from external approach (transcranial-subtemporal) to the trans-septo-transphenoidal approach. Since the last thirty years this way to access has been increasingly used, because it provides an excellent exposure, little bleeding, is rapidly and easily performed with less morbi-morality and has smoother postoperative period. A retrospective study of 16 patients whose hypophyseal tumors were treated surgically using the trans-septo-sphenoidal transnasal (maxillary-premaxillary) approach is presented. All these patients were seen in the E.N.T. Department of Alicante's General University Hospital, between January 1990 and June 1993. The trans-septo-sphenoidal transnasal via avoids some of the problems of the sublabial trans-sphenoidal procedure; namely longer operating time, oral contamination of the surgical field, subsequence difficulties due to the lack of sensibility and discomfort of the upper jax area and postoperative alterations in the projection of the septal-columelar tip.

[Botryoid rhabdomyosarcoma of the nasopharynx treated with chemotherapy only. Survival lasting more than ten years].

The progress of Rahbdomyosarcoma (RMS) of the nasopharynx succeed frequently as infiltrative and destructive form to the base of the skull, in which case its prognostic is worse as when localized somewhere else. We report the case of a RMS in a 4-year-old child. The tumour infiltrating the base of the skull (stage III), was treated with chemotherapy only because the neighbourhood of the sella turcica and the pituitary gland strongly forbids the appeal to radiotherapy, in order to avoid untoward side-effects on this gland and also upon the growth of the child. At present, 10 years after, he remains asymptomatic and free of the tumour. Computered tomography has probe to be the best way to study the site and the extent of this growth and even the control of course of treatment.

[Intracranial complications of otitis].

Since the advent of antibiotics, endocranial complications due to infectious ear disease are actually unfrequent. They occur predominantly in young adults with a long term chronic otitis media and cholesteatoma. The mortality for endocraneal complications still remains significant. We report six cases of endocraneal complications due to infectious ear disease, that have been seen in our service over the last eight years; remarking that half of the studied patients had an acute ear disease. A review of the literature helps to understand the actual standard of diagnosis and treatment.

[Congenital lesions of the naso-frontal midline].

Dermoids cysts, gliomas and encephaloceles are uncommon congenital lesions that result from an aberrant embryologic development. Six cases of these nasal lesions have been treated. These may present an intracranial extension as a consequence of an unusual closure of the anterior neural tube and it requires an early diagnosis to avoid local and intracranial infectious complications. Therefore, an early surgical correction is thus imperative. Neuroimaging studies may help to predict intracranial involvement.

[Solitary plasmacytoma of the head and neck. Report of 3 cases and review of the literature].

Solitary plasmocytoma is a rare plasmactic cell tumor occurring in the head and neck. These constitute, less than 1% of all head and neck malignancies. On initial presentation they must be differentiated from multiple myeloma. This may prove to be difficult because a varying percentage may be associated at a later date with the development of multiple myeloma. We describe three cases of plasmocytomas, one occurred in the larynx, in the cavity nasal and other in the skull base. The clinical characteristics, diagnosis criteria and therapeutic problems (radiation, surgery or a combination of booth) are discussed and reviewed in the literature.

[Descriptive study of infectious ear disease in relation to summer].

A descriptive study was made of infectious ear disease (including diffuse otitis externa, otomycosis, acute-on-chronic otitis media, and superinfection of a radical mastoidectomy cavity) in relation to changes of weather and habits in summer. During the months of June, July, and August 1996, 179 patients were evaluated in the emergency room of the Alicante General University Hospital, Spain. Average patient age at presentation was 30.52 (+/- 20.08) years and 56% were men. The most frequent disease was diffuse otitis externa (78%) followed by acute-on-chronic otitis media (12%), otomycosis (8%), and superinfection of a radical mastoidectomy cavity (2%). The most frequently involved microorganisms were Pseudomonas aeruginosa in diffuse otitis externa, Aspergillus niger and Candida in otomycosis, Escherichia coli, Haemophilus influenzae, Proteus mirabilis, and Staphylococcus aureus in acute-on-chronic otitis media. Patients were treated by cleaning detritus and secretions, usually followed by topical antibiotics for a maximum period of one week.

[Open rhinoplasty: outcomes of 100 cases].

Open rhinoplasty is a technique that allows a sharper evaluation of nasal deformities since a visual diagnosis may be done. We present a review of the cases in which an open rhinoplasty was performed in our department in the last five years. Preoperative and postoperative data are recorded in a standardized form in order to facilitate their analysis. The techniques used for each type of nasal deformity (dorsal and basal) are described and the results discussed.

[Our experience with caustic substance ingestion in children].

We discuss the protocol followed in our Service for the diagnose, treatment and follow-up of patients younger than seven years old, diagnosed as "caustic substance ingestion". We report on 157 cases seen from 1987 to 1991, from which 14 showed severe oesophageal burnings which required further oesophageal dilatation with general or local anesthesia. The usefulness of pH-metry in this patients is also analyzed. The relatively mild degree of abrasiveness produced by bleach is also described, despite being the most frequent caustic agent in our group of patients. The usefulness of the different techniques applied for the treatment and follow-up of this patients is also discussed.

[Neurofibromatosis with laryngeal involvement].

The clinical features of a new case of Von Recklinghausen's neurofibromatosis affecting the larynx, are described. The patient was a three year old boy presenting with hoarseness. The diagnostic and therapeutic features, and the clinical course are described. This is another patient with neurofibromatosis, including laryngeal involvement, to be added to the 26 such cases already described in the world literature. It is the eleventh laryngeal plexiform neurofibroma described. The relevant literature is reviewed.

[Protocol for sino-nasal tumors in our service].

Tumors of the nasal fossa and paranasal region are uncommon and have non-specific initial clinical features. This complicates the diagnosis and delays treatment. We reviewed all our cases of nasal sinus tumors (84 benign, 50 malignant and 15 moderately malignant) and the relevant literature. This data was use to develop a protocol for classifying the symptoms, clinical and radiological features, coded diagnosis, and the most suitable treatment and follow-up.
