The electrophysiological effects of lithium in the rat.

The effects of lithium on the tail nerve conduction velocity of the rat were studied. No effect of lithium on the conduction velocity was found, even at toxic serum lithium levels. No effect of lithium on repetitive stimulation of the distal forelimb flexor muscles was seen. The results suggest that electrophysiological measurements do not provide an effective means of titrating serum lithium levels in the rat. The results may have clinical signficance since they provide evidence against neuropathic or neuromuscular transmission defects as the cause of the transient weakness of some patients taking lithium.

Use of gas permeable contact lenses following trabeculectomy.

Some patients require a contact lens after glaucoma filtering surgery. The visual rehabilitation of such eyes can be challenging. We report on a total of eight eyes (seven patients) fit with lenticular-design, rigid gas permeable (RGP) contact lenses after the successful establishment of a filtering bleb. In follow-up (mean: 40 months), all patients have successfully worn lenses without adverse effects. We believe that, with careful fitting, close monitoring, and appropriate patient selection, the presence of a filtering bleb need not be a contraindication to contact lens wear, particularly when a smooth-edged, rigid gas permeable (RGP) daily wear lens is used.

Infectious crystalline keratopathy: an electron microscope analysis.

A 73-year-old man developed a crystalline stromal keratopathy in one eye following his second corneal transplant. Despite aggressive treatment with antibiotics and antifungal agents, the lesion increased in size. A corneal transplant was performed and histopathologic studies were performed using both light and transmission electron microscopy. The characteristic trilamellar structure of a bacterial cell wall was found. The organisms were present only anteriorly. Small electron-dense bodies with needle-like projections were found. These bodies may be responsible for the crystalline appearance of this keratopathy. The appearance of a crystalline keratopathy should alert clinicians to the possibility of a low-grade bacterial infection. After appropriate cultures, we advise early and aggressive therapy with appropriate antibiotics.
