An epidemic of enterovirus 71 infection in Taiwan. Taiwan Enterovirus Epidemic Working Group.

BACKGROUND: Enteroviruses can cause outbreaks of hand-foot-and-mouth disease (characterized by vesicular lesions on the hands, feet, and oral mucosa) or herpangina, usually without life-threatening manifestations. In 1998 an epidemic of enterovirus 71 infection caused hand-foot-and-mouth disease and herpangina in thousands of people in Taiwan, some of whom died. METHODS: We assessed the epidemiologic aspects of this outbreak. Cases of hand-foot-and-mouth disease or herpangina in ambulatory patients were reported to the Taiwan Department of Health by a mean of 818 sentinel physicians. Severe cases in hospitalized patients were reported by 40 medical centers and regional hospitals. Viruses were isolated by 10 hospital laboratories and the department of health. RESULTS: The sentinel physicians reported 129,106 cases of hand-foot-and-mouth disease or herpangina in two waves of the epidemic, which probably represents less than 10 percent of the estimated total number of cases. There were 405 patients with severe disease, most of whom were five years old or younger; severe disease was seen in all regions of the island. Complications included encephalitis, aseptic meningitis, pulmonary edema or hemorrhage, acute flaccid paralysis, and myocarditis. Seventy-eight patients died, 71 of whom (91 percent) were five years of age or younger. Of the patients who died, 65 (83 percent) had pulmonary edema or pulmonary hemorrhage. Among patients from whom a virus was isolated, enterovirus 71 was present in 48.7 percent of outpatients with uncomplicated hand-foot-and-mouth disease or herpangina, 75 percent of hospitalized patients who survived, and 92 percent of patients who died. CONCLUSIONS: Although several enteroviruses were circulating in Taiwan during the 1998 epidemic, enterovirus 71 infection was associated with most of the serious clinical manifestations and with nearly all the deaths. Most of those who died were young, and the majority died of pulmonary edema and pulmonary hemorrhage.

Clinical spectrum of enterovirus 71 infection in children in southern Taiwan, with an emphasis on neurological complications.

An outbreak of enterovirus 71 (EV71) infection occurred in Taiwan in 1998. The clinical spectrums and laboratory findings for 97 patients with virus culture-proven EV71 infections were analyzed. Eighty-seven percent of the patients were younger than age 5 years. Hand-foot-and-mouth syndrome occurred in 79% of the children and central nervous system (CNS) involvement in 35%, including nine fatal cases. The predominant neurological presentations were myoclonus (68%), vomiting (53%), and ataxia (35%). Brain stem encephalitis was the cardinal feature of EV71 CNS involvement during this outbreak. Magnetic resonance imaging and pathological findings illustrated that the midbrain, pons, and medulla were the target areas. EV71 brain stem encephalitis can present either with cerebellar signs and an initially mild, reversible course or with overwhelming neurogenic shock and neurogenic pulmonary edema (NPE) resulting in a fatal outcome. Brain stem encephalitis that progressed abruptly to neurogenic shock and NPE was indicative of poor prognosis in this epidemic. Early aggressive treatment and close monitoring of the neurological signs are mandatory to improve the chance of survival.

Fatal coxsackievirus B infection in early infancy characterized by fulminant hepatitis.

OBJECTIVES: to clarify the major features of fatal coxsackievirus B infection characterized by fulminant hepatitis in early infancy. METHODS: clinical manifestations and laboratory investigations concerning five consecutive young infants with overwhelming coxsackievirus B fulminant hepatitis between 1994 and 1997 were retrospectively reviewed. Aetiological diagnosis was made by viral cultures and confirmed by a neutralization test with a type-specific antiserum. RESULTS: all five had a deteriorating clinical course of severe hepatitis complicated by disseminated intravascular coagulopathy (DIC). Coxsackievirus B1 infection was established in four patients and coxsackievirus B3 in one. The pathological findings of the two cases illustrated extensive hepatocellular necrosis. Fulminant hepatitis can occur as a leading presentation of disseminated coxsackievirus B infections and dominant the clinical features in neonates and young infants. CONCLUSIONS: the liver was the target organ of fatal coxsackievirus B infection in our patients. Hepatic involvement progressed rapidly to jaundice and coagulopathy, and was considered to be indicative of poor prognosis. Coxsackievirus B hepatitis may be serious in early infancy.

Effect of group A streptococcal cysteine protease on invasion of epithelial cells.

Cysteine protease of group A streptococci (GAS) is considered an important virulence factor. However, its role in invasiveness of GAS has not been investigated. We demonstrated in this study that two strains of protease-producing GAS had the ability to invade A-549 human respiratory epithelial cells. Isogenic protease mutants were constructed by using integrational plasmids to disrupt the speB gene and confirmed by Southern hybridization and Western immunoblot analyses. No extracellular protease activity was produced by the mutants. The mutants had growth rates similar to those of the wild-type strains and produced normal levels of other extracellular proteins. When invading A-549 cells, the mutants had a two- to threefold decrease in activity compared to that of the wild-type strains. The invasion activity increased when the A-549 cells were incubated with purified cysteine protease and the mutant. However, blockage of the cysteine protease with a specific cysteine protease inhibitor, E-64, decreased the invasion activity of GAS. Intracellular growth of GAS was not found in A-549 cells. The presence or absence of protease activity did not affect the adhesive ability of GAS. These results suggested that streptococcal cysteine protease can enhance the invasion ability of GAS in human respiratory epithelial cells.

[A model of osteoporosis induced by retinoic acid in male Wistar rats].

An animal model of osteoporosis induced by retinoic acid was successfully established in 3-month-old male Wistar rats. The animals were given the drug 70 mg.kg-1.d-1 for 14 d intragastrically and sacrificed on day 29. The proximal tibia and middle tibia of the rats were processed undecalcifiedly for quantitative bone histomorphometry. Compared with the control rats, the cancellous and compact bone volume of the model rats were reduced markedly. The bone tissue microstructure showed some obvious pathological changes that the trabecular number were decreased, the separation of trabecular and medulla ossium cavity became large, the thickness of trabecular and cortex of bone were decreased. The mechanism of bone loss in the model rats was that the osteoclast was activated by retinoic acid which promoted bone resorption. Other changes in the model rats were also observed such as: the body weight, and the weights of seminal vesicle and prostate were decreased, the adrenal glands and spleen showed hyperplasia and hypertrophy. No change of the blood concentration of calcium, phosphorus, alkaline phosphatase, alanine transaminase, estradiol and testosterone in the model rats was observed.

Evaluation of Doppler echocardiographic atrial pacing stress test in the detection of coronary artery disease.

Doppler echocardiographic atrial pacing stress tests were performed in 23 patients with coronary artery disease (CAD) and 35 normal subjects. It was found that the peak flow velocity (PFV) through the mitral valve decreased at 160 beats per minute (BPM) during pacing in the CAD group (P less than 0.01), but no significant variation was seen in the normal group. No differences between the two groups were observed in 1/3 filling fraction (1/3 FF), total diastolic filling period (DFP) and the ratio of early diastolic filling period (EDFP) to DFP before pacing. However, 1/3 FF and DFP decreased and EDFP/DFP increased immediately following rapid pacing in the CAD group but did not change in the normal group (P less than 0.05, 0.01, and 0.001 respectively). The peak flow velocity during atrial contraction (APFV), time velocity integral during atrial contraction (ATVI) and the ratio of ATVI to total TVI (ATVI/TTVI) increased immediately following pacing in the CAD group. Using two of three indices--ATVI/TTVI more than 0.35, ATVI increase and 1/3 FF decrease immediately following pacing--as diagnostic criteria for CAD, the sensitivity was 86% and the specificity was 77%. It may be concluded that the Doppler echocardiographic atrial pacing stress test is a feasible and reliable method for detecting CAD.

M protein mediates streptococcal adhesion to HEp-2 cells.

Streptococcus pyogenes adheres to human epithelial cells in vitro and in vivo. To identify adhesins, cell wall components were extracted from S. pyogenes M6 with alkali or by treatment with mutanolysin and lysozyme. HEp-2 cells were incubated with extracts of S. pyogenes M6 and then analyzed by Western blot (immunoblot) assays, using antibodies to S. pyogenes. Only one streptococcal component (62 kDa) was bound to HEp-2 cells and was identified serologically as M6 protein. Experiments with pepsin-cleaved fragments of M protein indicated that the binding site was located at the N-terminal half of the molecule. M protein was bound selectively to two trypsin-sensitive surface components, 97 and 205 kDa, of HEp-2 cells on nitrocellulose blots of sodium dodecyl sulfate-polyacrylamide gels. Tritium-labeled lipoteichoic acid bound to different HEp-2 cell components, 34 and 35 kDa, in a parallel experiment, indicating that lipoteichoic acid was not complexed with M protein and does not mediate M-protein binding. The four HEp-2 components were unrelated to fibronectin since they did not react with specific antibodies. An M-protein-deficient (M-) strain of streptococcus (JRS75), grown in chemically defined medium, showed 73% less adhesion activity to HEp-2 monolayers than an M+ strain (JRS4). Streptococcal adhesion was insensitive to competitive inhibition by selected monosaccharides. These results indicate that M protein binds directly to certain HEp-2 cell membrane components and mediates streptococcal adhesion.

Streptococcal M6 protein binds to fucose-containing glycoproteins on cultured human epithelial cells.

M6 protein of Streptococcus pyogenes binds directly to HEp-2 cell surfaces and helps to mediate bacterial adhesion. Two epithelial cell receptors for M protein were identified as 97- and 205-kDa glycoproteins. Purified recombinant M6 protein (rM6) showed a dose-dependent and saturable binding to isolated HEp-2 membranes in an enzyme immunoassay. The HEp-2 cell receptors were selectively denatured by pretreatment of isolated membranes at 80 degrees C or with chymotrypsin; binding activity for rM6 was reduced 83 and 80%, respectively. Pretreatment of the HEp-2 membranes with neuraminidase-N-glycosidase, neuraminidase-O-glycosidase, alpha-L-fucosidase, or Ulex lectin caused 33, 42, 73, and 80% reduction of rM6 binding, respectively. Quantitative analysis of HEp-2 cells pretreated with alpha-L-fucosidase showed that the 97- and 205-kDa glycoproteins lost 70 and 62% of their abilities to bind M6 protein and that 33% of the HEp-2 cell's ability to bind whole streptococci was also lost. These results indicated that binding of M6 protein to HEp-2 cell surfaces is highly selective for certain fucose-containing oligosaccharides on these glycoproteins.

[Influence of cardiac structure, blood flow velocity and heart function on circulating atrial natriuretic peptide and renin-angiotension system].

Heart function and plasma atrial natriuretic peptide (ANP), plasma renin activity (PRA) and angiotension II (Ang II) were examined with echocardiography and radioimmunoassay in 9 patients with dilated cardiomyopathy (DCM), 41 with rheumatic heart disease (RHD), 29 with hyperthyroidism (Ht) and 24 normal subjects. ANP level was significantly increased as heart failure progressed (P < 0.01 and 0.001). There was negative correlation between ANP and left ventricular fractional shortening, and between ANP and ejection fraction in DCM and Ht groups. There was positive correlation between ANP and maximal left atrial diameter, right atrial area, and diastolic diameter or volume of left ventricle in DCM and RHD groups, and negative correlation between ANP and peak flow velocity in aorta or through mitral valve in DCM group (r = -0.608, P < 0.05 and r = 0.710, P < 0.05). These findings suggest that the stronger the myocardial contractility and the faster the blood flow, the lower the plasma ANP level.

[Study on the expression of oncogenes in hepatocytes with hepatitis B virus infection].

The expression of oncogene N-ras mRNA and c-myc mRNA in paraffin embedded liver tissues from patients with hepatitis B virus (HBV) infection had been studied by in situ hybridization with biotin labeled probes. The results showed that the detection rates of N-ras mRNA and c-myc mRNA were 37.5% (24/64) and 29.7% (19/64) respectively in the liver tissues of 64 hepatitis B patients, 44.4% (16/36) and 47.2% (17/36) respectively in the liver tissues of 36 hepatocellular carcinoma (HCC) patients with HBV infection, and they were detected each in one of 6 normal liver tissue samples. No significant differences were observed among the three groups (P > 0.05). However, in HCC group, 11 out of 36 patients (30.5%) were positive for N-ras mRNA and c-myc mRNA simultaneously, which was higher than that in hepatitis B group (14.1%) (P < 0.05). None of the normal liver samples were N-ras mRNA and c-myc mRNA positive simultaneously. Further more, in the hepatitis group it was noticed that the detection rate of c-myc mRNA in HBV DNA positive cases (by in situ hybridization) was significantly higher than that in HBV DNA negative cases (P < 0.025).

[Determination of blood hormone levels in patients with "spleen deficiency" syndrome after major operations].

UNLABELLED: The character of "Spleen Deficiency" (SD) Syndrome was evaluated by determination of the blood levels of various gastroenteropancreatic hormones and thyroid hormone. Patients of SD group were diagnosed clinically by Syndrome Differentiation, and those of the normal group were volunteer blood donors. RIA was used to determine all the hormones except T3RUR. RESULTS: TT3, FT3 of the SD group were very significantly lower than that of normal group (P less than 0.001). The levels of rT3 in SD group were significantly higher than normal control (P less than 0.05). For TT4, T3RUR and FT4, there were no significant differences between the two groups. Among the gastroenteropancreatic hormones, gastrin in the SD group was significantly lower than that of the normal group (P less than 0.01), motilin was very significantly higher (P less than 0.002). The value of pp was similar in the two groups, under physiological conditions, however, in adults the hormone tended to increase with age. Since the average age of patients in the SD group was 54.9, and that of the normal group was 38.5, remarkably lower values were seen in the SD group. Other hormones such as VIP, SST, glucagon and insulin were of no significant difference between the two groups. The result showed that thyroid hormone and some of gastroenteric hormones in the blood of patients with SD after major operations were quite different from normal, and was common to patients with general SD Syndrome, with low T3 syndrome.(ABSTRACT TRUNCATED AT 250 WORDS)

Correlation of mutations of oncogene C-Ha-ras at codon 12 with metastasis and survival of gastric cancer patients.

Mutations of c-Ha-ras at codon 12 were detected from 11 out of 27 fresh tissues and cell-lines of human gastric cancer patients using PCR-restriction analysis. Further statistical investigation showed that the presence of point mutations was related to the distal metastases and the survival time of gastric cancer patients after surgical operations.

[Dynamical comparison between Drummond's and Luque's operations. An experimental study].

Eleven specimens of spinal segments from cadavers were divided into four groups for the study of the tension endured by the implant applied at the base of the spinous process in different ways till the breaking off of the specimen. The results indicated that the endurance of tension varied in each group and was related directly to the direction of the pulling force. In group 1, in which the force had been perpendicular to the spinous process, the specimen failed at an average force of 513.25 N.; while in group 3, in which parallel force had been applied, it ended at 477.3 N., although the anchoring of the implant, a home made cement-button-wire, had been the same. In group 2, the implant had been laid on both sides and it did not give way until the force up to 764.5 N., a little less than that in group 4, a force of 887.3 N., in which the wire had passed under lamina like that in Luque's operation. We conclude that double fixation would work more effectively on correcting spinal deformity, and that modified Luque operation, instead of sublaminar wiring by anchoring on both sides, cement-button-wire at the base of the spinous process is biomechanically efficient and safe.

[Effects of ursodeoxycholic acid on mechanical and electric activities in isolated guinea pig heart muscles].

Using electromechanical transducer and conventional microelectrodes in isolated guinea pig heart muscles, the effects of ursodeoxycholic acid (UDCA) on the mechanical and electric activities were firstly observed. The results showed that, UDCA in dose of 0.05 mg/ml decreased contractile force by 36% (P less than 0.001) and shortened action potential duration (n = 3). It is inferred that UDCA may block inward calcium current. The results suggest that the action of UDCA is similar to that of bear bile and may be an effective composition of bear bile which acts on heart muscle. Our findings provide the experimental basis for the clinical application of UDCA.

[Clinical effects of nifedipine on blood pressure and left ventricular performance in patients with isolated hypertension].

17 patients with isolated systolic hypertension (ISH) and 8 with diastolic hypertension (DH) were treated with 10 mg of nifedipine sublingually. In ISH group the blood pressure decreased on the average from 180.2/87.4 to 166.2/84.2, 161.9/82.6, 163.4/83.7 mmHg after 5.20 and 60 minutes, while in DH group the blood pressure decreased on the average from 139.2/102.0 to 138.0/100.0, 137.6/97.6, 134.4/97.2 mmHg. With pulsed Doppler echocardiography, we found that early diastolic time-velocity integral increased (P less than 0.001) and E time prolonged (P less than 0.01) after 20 and 60 minutes in ISH group; and 1/3 filling fraction increased (P less than 0.05) after 20 minutes in DH group. With M-mode echocardiography we found that cardiac index and ejection fraction were improved in both groups. No obvious side effects were noted. From above-mentioned findings we are of the opinion that sublingual nifedipine is effective in reducing blood pressure and improving left ventricular performance especially in ISH patients.

Uneven myocardial hypothermia among cardiac chambers during hypothermic myocardial preservation.

During the development of methods to protect the heart from ischaemic injury, attention has been focused on protection of the left ventricle. In an attempt to assess right heart preservation. 55 consecutive patients undergoing open heart surgery were studied. Mean aortic cross-clamp time was 59.3 +/- 29.4 min. Temperature probes were inserted into the right atrium (RA), right ventricle (RV), and left ventricle (LV). During cardioplegia, the mean myocardial temperatures of RA, RV and LV were 19.1 degrees +/- 4.1 degrees C, 12.7 degrees +/- 4.8 degrees C and 7.3 degrees +/- 3.4 degrees C, respectively. Of the LV temperature measurements, 67.2% were 10 degrees C or lower. By contrast, 94.1% of RA measurements and 58.5% of RV measurements were above 10 degrees C. The inhomogeneity of chamber temperatures was observed irrespective of the patient's disease or age and whether the atrium or right ventricle were open or not. Hearts with mitral regurgitation (MR), in contrast to mitral stenosis and stenoinsufficiency, had higher LV temperatures, similar to those in the RV. We conclude that there is uneven hypothermia among the three cardiac chambers during hypothermic cardioplegic arrest, regardless of disease states except MR and regardless of age and procedure performed.

Pharmacokinetics of sulbactam and ampicillin in mice and in dogs.

Sulbactam is a new beta-lactamase inhibitor. The pharmacokinetic characteristics of sulbactam was similar to ampicillin after a single intravenous injection of 200 mg/kg in mice with a half-life of approximately 50 min. The two drugs appear to equilibrate rapidly between central and peripheral compartments. The Vc and Vd suggest that they are widely distributed in the extracellular fluid and into tissues. Co-administration of sulbactam and ampicillin in mice and in dogs showed essentially no change on the kinetics of either ampicillin or sulbactam.
