Neoplasia in the vagina following hysterectomy for dysplasia or carcinoma in situ of the uterine cervix.

With a view to establishing some of the factors responsible for an increased risk of neoplasia in the vagina following primary hysterectomy for dysplasia or carcinoma in situ of the cervix, 63 surgical specimens from patients with subsequent neoplasia in the vagina have been compared with specimens from 40 patients without subsequent neoplasia. We found the frequency of neoplasia in the vagina to be greater when dysplasia or carcinoma in situ could be demonstrated in the vaginal margin. Neither the degree of endocervical glandular involvement, the number of mitoses, nor the inflammatory response of the connective tissue appeared to be significant in this respect. No significant connection was found between the severity of the preinvasive lesion of the cervix and the subsequent neoplasia in the vagina.

Structural features of cytochalasins responsible for gram-positive bacterial inhibitions.

A study of the relative effectiveness of some eighteen natural and synthetically modified cytochalasins on the uptake of glucose by the Gram-positive bacterium Arthrobacter sialophilus showed that cytochalasins B, C or D and aspochalasins A, C or D were inactive natural congeners. The presence of an alpha,beta-unsaturated carbonyl group in the macrolide moiety of these compounds with appropriate bioisosteric placement, as exemplified by cytochalasin A and aspochalasin B, are requisite molecular features. The transmembrane inhibitory index of active compounds was enhanced by increasing their lipophilicity. Thiol adducts of CA were around 20% as active in solute uptake inhibition as was the free drug. Radioactive 7-O-acetyl CA and its thiol adduct were each rapidly taken up by A. sialophilus and remained firmly bound to cellular components even after denaturant manipulations. These findings provide strong evidence for a stable association between CA and presumptive macromolecular receptors in transport and related processes.

Toxic epidermal necrolysis.

Toxic epidermal necrolysis is one of the rarest events -- a dermatologic emergency. The clinical presentation is characteristic -- disseminate erythema and widespread bullous necrosis of the epidermis and mucous membranes. Toxicity is universal and mortality is substantial even with prompt and intensive therapy. Drugs are the most commonly incriminated agents and therapy remains controversial, the majority favoring high-dose systemic corticosteroids.

Hormone treatment of stromal endometriosis.

A report of two cases of recurrent stromal endometriosis, both the patients having positive estrogen receptors in the tumor tissue. They were first treated with tamoxifen, which failed to halt growth of the tumor. After a month's subsequent treatment with progesterone the tumor regressed and the patients' condition improved.

Studies on the required analytical quality of TSH measurements in screening for congenital hypothyroidism.

In the present study analytical reliability has been evaluated from the point of view of medical practice. The data were obtained from a screening program for congenital hypothyroidism in newborns using measurements of TSH. The aim of the investigation has been to evaluate the optimal analytical quality of the TSH determinations in the classification of children into two groups, with and without hypothyroidism. Evaluation of analytical quality was attempted at four different levels: (i) the numbers of misclassifications at a certain fixed value of the cutoff point, (ii) the numbers of false positives when the percentage of false negatives is fixed, (iii) the total costs of the screening program, and (iv) the social costs. When the evaluations are performed at level (i) and (ii) the results indicate a need for a high analytical quality. At level (iii) a lower analytical quality is acceptable from consideration of costs only. When social costs (iv) are considered, the acceptable analytical quality will depend on the weights given to the different factors. Finally the results point to a close relationship between the imprecision and the cutoff point chosen.

Liposarcoma of the breast. Case report and review of the literature.

A case of primary liposarcoma of the breast in a 65-years-old woman is described by light- and electron microscopy. The patient had previously had a cystosarcoma phyllodes removed from the same area. A review of the literature showed 34 previously published cases of liposarcoma of the breast, which regarding age distribution, histological type and pattern of metastasis corresponded to liposarcomas of the lower limbs.

Reliability of histo-pathological diagnosis of squamous epithelial changes of the uterine cervix.

The reliability of histological diagnosis of squamous epithelial changes was tested by letting 13 pathologists read 1,001 consecutive cervical biopsies twice. Intra-observer and inter-observer agreement, variance, and deviation of diagnosis were determined. The diagnostic ability showed great individual variation and no significant correlation to experience in pathology. The diagnosis of invasive cancer had a high diagnostic specificity, and the diagnostic sensitivity of the diagnosis of no significant epithelial changes was high too. The reliability of the diagnosis of dysplasia and carcinoma in situ proved unsatisfactory.
