Operative management of 194 cases of combined maxillofacial-frontobasal fractures: principles and surgical modifications.

The treatment modalities and results of 346 cases of frontobasal and midface fractures, 194 of which had to be managed by surgery, are reported, and the operative results are evaluated. The importance of an early one-stage operative repair of combined frontobasal-maxillofacial fractures is stressed. The indications for the pure transethmoidal, combined transethmoidal-transfrontal, and single neurosurgical handling of these patients are analyzed. Important surgical modifications that are decisive in the operative management of these fractures are described.

Use of the titanium-coated hollow screw and reconstruction plate system in bridging of lower jaw defects.

Clinical results obtained with hollow titanium screws and reconstruction plates in the bridging of mandibular bone defects of 17 patients are described. Following implantation the newly forming bone establishes direct contact with the sprayed titanium surface of the screw and the underside of the plate and permeates the screw. The stability of anchorage therefore increases with the duration of implantation. The reconstruction system also allows salvaging of small fragments such as the condyloid process. The adjustable, individually shaped, mandibular condylar prosthesis permits precise restoration of articular guidance on the reconstructed side while preventing incorrect loading of the opposite joint. The clinical results confirm the recently published findings from animal studies.

[Operative procedures in extensive upper and lower jaw cysts].

In this paper we describe the various surgical techniques in the treatment of cysts of the mandible and maxilla. The importance of primary reconstruction of extensive defects is pointed out. We developed a special press for autologous cancellous bone to achieve better osteoplastic results. We describe the advantages of this technique in comparison to the conventional methods.

[1-stage procedure in the reconstruction of frontobasal middle face fractures. Modifications and treatment modalities].

This paper deals with the management of 269 patients with frontobasal fractures of which 143 were treated operatively. The close interrelation between midface and base of skull and our operative modifications are discussed. The advantages and importance of the transethmoidal approach to the base of the skull is most adventageous for reconstruction of the midface pillars. Specially for comminuted fractures, avoiding complications such as pseudohypertelorism, telecanthus and frontobasal disturbances. This also enables an accurate reposition to be made with minimal use of foreign materials such as wire ligatures and miniplates. The operation should be performed by the same surgeon in a single sitting thus combining the transethmoidal approach with the midface reconstruction. The reduction of the rate of CSF recurrent leaking to only 3,5% is ascribed to these methods.

[Current possibilities of reconstruction of the lower jaw in bony defects following tumor resection. Animal experiments and clinical results].

The animal experimental methods as well as the clinical application in patients and histological results of a new reconstruction plate for use in bridging defects in the lower jaw are discussed. The principles of fixation of the plate with hollow titanium powder sprayed screws and a titanium mesh for stabilising the autologous cancellous bone are also given. Resection was carried out in irradiated and non-irradiated mandibles of dogs. The direct contact between the surface of the screw and the bone as well as proliferation of bone into the hollow screw are demonstrated by light and scanning electron microscope studies. New methods for the evaluation of the interface between implant and bone are described.

[Results of animal experiments following the use of a new material--biocement--for osteoplasty and fixation of alloimplants].

We report on results obtained with biocement (BC), a new material recently developed in our laboratory, in animal experiments dealing with osseous repair in the facial area. The organic matrix of BC corresponds to the Bowen formula. Anorganic filler substances comprise Ca5 (PO4)3OH, Bioceramic A2 (Battelle) and Bioglass Hench. We applied the new material to the following surgical interventions in monkeys or dogs: reconstruction of the anterior wall of the frontal sinus; covering of skull cap defect with or without a lesion of the dura mater; fixation of autologous bone transplants; bridging of defect in the mandible and the fixation of alloimplants under stress. The histological preparations of undecalcified bone were examined by light microscopy, in polarized light, with the help of scanning electron-microscope and the interface evaluation between implant and bone with energy dispersive X-ray analysis. Direct contact between BC and bone, and interdigitations between the two, as well as new bone formation were the rule. In addition, very few reactive changes were observed in cerebro-meningeal tissues neighboring BC. The latter did also not disturb the regeneration of ciliated epithelium in its vicinity.

Cement forming tumours of the lower jaw (case report).

This publication concerns two cement forming tumours - the centrally cementifying fibroma and the true cementoma. The centrally cementifying fibroma is a rare tumour and has been considered an entity for some time. Our case presents a lesion of a size and a homogeneous histological structure rarely encountered. The operative procedure is also discussed. The progression from the radiolucent to the radio-opaque stage could be observed radiologically in the case of a cementoma.
