Gastrointestinal hemorrhage caused by secondary aorto-duodenal fistula: report of 5 cases.

Aorto-intestinal fistula is a late complication of reconstructive vascular operation, which is difficult to indentify. After the cessation of a massive, life-threatening bleeding, the best result can be expected from extraanatomical, axillobifemoral bypass. The authors treated 5 patients for fistula, developed following an aorto-bifemoral bypass. Diagnosis could be made before operation only in two of the cases. Axillo-bifemoral bypass was performed in 3 patients following the treatment of the fistula and the removal of the graft; 2 of their patients recovered.

[Experience with Tenckhoff catheter implantation in chronic peritoneal dialysis].

The authors performed 310 Tenckhoff catheter implantations during the last 17 years. They had 64 complications, and 29 cases the catheter had to be changed. The incidence of peritonitis was 0.22 patients/year. They present the technic of implantation, the complications, and the possible abdominal surgeries with the patients under dialysis treatment.

[Acute kidney failure caused by rhabdomyolysis].

Rhabdomyolysis developed in the muscles of the left lower extremity of a 43 year old man leading to myoglobinuria and acute renal failure. The diagnosis of rhabdomyolysis was strengthened by muscle and renal biopsy. The renal functions were restituted after peritoneal dialysis performed four times (12 hours each) within two weeks. The possible role of alcohol or Dalgol intoxication as well the disturbed circulation in the development of rhabdomyolysis is discussed. Authors point out the benefit of peritoneal dialysis in acute renal failure caused by rhabdomyolysis.

Local recurrences following colorectal operations.

In about 15-20% of patients operated for colorectal tumours local recurrences develop mainly in the first postoperative year. This large number can only be reduced by adequately radical operations taking into account the patient's age, tumour site and the tumour-biological factors. Indispensable factors are the organized care and regular control of the operated patients with an emphasis, beside CEA test on US and CT studies. In local recurrences attempt should be made at removal of the tumour by an additional operation which is implementable in 20-30% of cases. For palliative treatment first of all radiotherapy can be applied for pain relief.

Antimicrobial spectrum of triclosan, a broad-spectrum antimicrobial agent for topical application. II. Comparison with some other antimicrobial agents.

The antimicrobial activity of 5-chloro-2(2,4-dichlorophenoxy) phenol (triclosan, one of the active ingredients of Logamel, Ciba-Geigy) was compared in vitro with that of other antimicrobials exclusively or occasionally used as topical agents in dermatology: hexachlorophene, clioquinol, chlorquinaldol. gentamicin, neomycin, nystatin, econazole, clotrimazole and salicylic acid. Upon determination of the MICs for 53 strains of aerobic and anaerobic bacteria, yeasts and fungi, triclosan was found to display a high degree of activity against most of the test organisms and to have the broadest spectrum of chemotherapeutically significant antimicrobial activity of the substances tested.

[Experience in the treatment of peripheral vascular injuries].

The authors report the results of the treatment of injuries to the peripheral blood vessels that were observed at the Semmelweis OTE in 16 years. As a result of primary operative treatment the peripheral pulse returned to the injured limb in 10 cases. A quick diagnosis and instant reconstructive surgery is absolutely necessary. Mistakes and complications in the treatment are listed in this paper, based on the own experiences of the authors. The authors stress that these complications should be recognized in time and be treated properly.

[Role of bacterial endotoxins in the tourniquet shock in rats].

By means of the lead acetate endotoxin sensitising method it was possible to demonstrate the development of endotoxaemia in tourniquet ischaemia of rats. The lethality of shock was greatly reduced by retreatment with radio-detoxified endotoxin (Tolerin). Prelinary liquidation of the Gram-negative enceroflora caused a similar reduction of lethality. On the basis of the results endotoxaemia of intestinal origin triggered by the vasoactive substances liberated during ischaemia and reaching the circulation are considered of importance in the pathogenesis of tourniquet shock.

CGP 9000: a new orally active, broad-spectrum cephalosporin.

CGP 9000, (7-[D-2-amino-2-(1,4-cyclohexadienyl)-acetamido]-3-methoxy-3-cephem-4-carboxylic acid), is a new broad-spectrum cephalosporin antibiotic. Its antibacterial activity in vitro (MIC) is similar, but its bactericidal efficacy superior to that of cephalexin and cephradine. Upon oral administration to mice infected with various bacteria, CGP 9000 is, in general, 2 to 7 times more effective than either cephalexin or cephradine.
