[Endoscopic drainage of the bile ducts].

Insertion of biliary endoprosthesis was attempted in 151 patients with obstructive jaundice and was successful in 124 patients (82%). The indications for the procedure were: 1) biliary and pancreatic neoplasms (79 patients) and 2) non-neoplastic diseases such as common bile duct stones or postoperative biliary strictures (45 patients). Successful drainage of bile was obtained in 95% of patients from the first group and in 93% of those from the second group. The mean survival time of patients with neoplasms was 4.5 months and the majority of them were free of jaundice and itching. In 32 patients with non-neoplastic diseases there was no recurrence of jaundice after a mean time of 20 months. Serious complications occurred in 5 patients (4%), 4 of them died (3.2%). The most frequent late complication was clogging of the endoprosthesis (19% of patients from the first group after a mean time of 5 months and in 28% of those from the second group after a mean time of 16 months). Replacement of the endoprosthesis was carried out mainly on the ambulatory basis. Endoscopic biliary stenting is an effective method of palliative biliary drainage in patients with non-operable biliary and pancreatic tumours. In high risk patients with non-neoplastic obstructive jaundice the technique may be an alternative for surgical treatment.

[Evaluation of total energy balance and food habits of obese children].

Total daily energy expenditure (TDEE) and time spent for activities of different intensities (heart rate monitoring method), as well as dietary and food habits (24-hour dietary recall) in two groups of 12-14 year-old boys, including 35 obese (weight 67.0 kg +/- 9.3, height 162 cm +/- 7.6) and 35 control group boys (weight 46.0 kg +/- 6.5, height 160 cm +/- 7.5) were estimated. Obese boys spent more time at rest and less time on physical activities than nonobese (p < 0.001). That resulted in differences of TDEE expressed per unit of body mass (p < 0.001). Time for sleeping and time in school was not statistically different in the two groups. The evaluation of nutritional habits indicated that the obese boys, when compared with controls, had higher energy intake and significantly higher percentage of fat energy in total energy intake, low intake of fruits and vegetables and irregular frequency of meals during the day. Diets of boys were not balanced with respect to calcium and phosphorus.

[QT/QS2 index in patients with arterial hypertension, mitral valve prolapse and hyperthyroidism].

QT/QS2 ratio has been assessed in 26 patients with both borderline and mild hypertension and mitral valve prolapse syndrome (19 patients), and hyperthyroidism (16 patients) in comparison with method control groups. The following polycardiographic parameters have been analyzed: QT, QTp, QS2, QT/QS2, and QTp/QS2. Higher values of QT/QS2 ratio have been noted in patients with mitral valve prolapse syndrome and hyperthyroidism than that in the control group. There has been no difference in patients with mild hypertension while the values of the analyzed parameter have been significantly lower in patients with borderline hypertension. QT has been longer than QS2 (QT)QS2 1/in 9 (56%) patients with hyperthyroidism. A positive correlation between QT/QS2 ratio and ++thyroxine levels have been noted in these patients. QT values have been higher than QS2 values only in 1 patient with mild hypertension. It seems that QT/QS2 value has limited value as an indirect index of the adrenergic activity in the dysfunction of the autonomic nervous system.

[Role of coloscopy in diagnosis and treatment of large bowel diseases].

Coloscopy is the most accurate method of evaluation of the whole large bowel. The main indications for this examination are doubtful radiological changes in the colon and unexplained intestinal symptoms, bleeding in the first place. Coloscopy plays also an important role in the differential diagnosis of colitis and in the oncological follow-up of patients after polypectomy or with long-standing ulcerative colitis. The therapeutic use of coloscopy includes removal of polyps and foreign bodies, control of bleeding, and laser photocoagulation of neoplasmatic lesions. Since 1973 in the Department of Gastroenterology of the Postgraduate Medical Center 2360 diagnostic coloscopies were carried out in 1838 patients. The most frequent diagnoses were polyps (34%) and large bowel cancer (12%). Various forms of colitis were diagnosed in 7% of patients, and all other diseases jointly in 11% of cases. In 36% of patients the large bowel showed no changes, in spite of undoubtful indications for coloscopy. Many of these patients were spared explorative laparotomy. By means of diathermic loop 808 colorectal polyps were removed in 421 patients. Rare complications of this procedure were bleedings (1.5%) and peritoneal irritation symptoms without perforation (0.25%). Adeneomas prevailed among polyps. Seventeen patients with invasive cancer in adenoma were treated successfully only endoscopically.

[Practical aspects of intracolonoscopic polypectomy].

Modern views are presented on endoscopic treatment of colonic polyps stressing the superiority of the method over surgical treatment used previously. The factors limiting the possibility of use of the endoscopic method are: the size, the configuration and the number of polyps. However, the extent of these limitations is small. The importance of histological examination of the removed polyp for the tactics of postoperative management is emphasized and a programme is proposed for oncological supervision after coloscopic polypectomy.
