[An instrument for projecting visual perception by monofocal and bifocal intraocular lenses].

With the aid of a simple optical system similar to an astronomical telescope, images of spectacle lenses, contact lenses and intraocular lenses can be projected in front of, onto or into the eye. These optical images function like real lenses and enable their performance to be evaluated. In the case of intraocular lenses, visual perception through monofocal or bifocal lenses and teledioptric systems can be reproduced.

[Evaluation of the image quality of bi- and multifocal intraocular lenses with a new optical system].

By means of a new optical device developed by Reiner, an "optical implantation" of 6 different bi- and multifocal intraocular lenses (MIOLs) was performed in 20 young healthy subjects, and results were compared with those of a monofocal reference IOL. The following parameters were investigated: distance and near visual acuity, depth of focus and contrast sensitivity. The median of distance visual acuity was 1.0 in all IOLs with the exception of a 7-zone refractive MIOL (0.75). Near acuity without near addition was 1.0 in all MIOL models; through focus curves of all MIOLs showed an increase in depth of focus with a typical two peak course. Contrast sensitivity at high contrasts was reduced for the 7-zone-model, but contrast sensitivity at low contrasts was significantly reduced for all MIOLs, except the 2- and 3-zone refractive models.

[Eliminating reflection of intraocular lenses].

Reflections on the refractive surfaces of intraocular lenses can be a nuisance when performing retinoscopy or refractometry. Some patients with IOLs also see reflections which disturb their vision. This article examines the conditions for coating IOLs. It also explains why conventional coating methods cannot be used for IOLs.

Spectacles belong to the most ancient inventions in cultural history. Their development, however, cannot be regarded as final. New impetus has resulted in the creation of non-spherical lenses and, through the development of refractive lenses, in a more progressive optic effort.

[Applications of novel stereomicroscopic spectacles in ophthalmic surgery (author's transl)].

The previous magnifying spectacles and telescopic spectacles designed on the principle of the Galilean telescope have a magnification of only 2.5 x to 3 x for good image quality. New magnifying systems from Zeiss based on the Kepler telescope principle offer 3 x to 8 x magnification and thus cover the range between magnifying spectacles and operation microscopes and even come into the latter range. The following versions are available for ocular surgery: 1. Mounted on headband. 2. Fastened to the frame in front of the lenses. 3. Cemented into drilled holes in the lenses with additional horizontal connection of the magnifying systems. 4. Cemented into drilled holes in the lenses without horizontal connection. These versions offer different advantages and are selected according to the particular field of application and the refraction of the surgeon. With these compact and easily transportable stereomicroscopic systems the surgeon has complete freedom of movement being able to select the most convenient position at any stage of the examination or operation. Further applications of such stereomicroscopic spectacles beyond the field of ophthalmic surgery are mentioned.

Steroid profiles of women suffering from idiopathic hirsutism show in more than 50% of the cases of 10--100 fold increase in the excretion of dehydroepiandrosterone (DHEA) compared with normal values. The excretion of DHEA was reduced much more than that of other 17-ketosteroids if the adrenals (NNR) were suppressed by dexamethasone (DXM). Within one week they reached values at the compound noise level of the gas chromatograms. If the ovaries were stimulated with human chorionic gonadotropin during continued suppression of the NNR with DXM no increase of DHEA could be detected.

Spectacles worn by vehicle drivers should fulfil the demands set down by DIN 58 216 standards and should furthermore be constructed in such a manner that the binocular field of vision is impaired as little as possible. In this connection, the nasal parts of the frame are particularly important. The external and internal rear-view mirrors of automobiles should be positioned within the binocular field of vision.

Spectacle lenses with variable optic power are indeed no replacement but are an interesting addition to the group of visual aids for correcting presbyopic patients. Since the first commericial lenses of this kind appeared in the year 1959, further sorts of glass with variable properties have been developed. These were evaluated and compared with each other.

[A new apparatus for testing binocular vision (author's transl)].

The Optometer can be used in a binocular manner if the loupe-lens is replaced by a concave mirror of appropriate size and form. The concave mirror or the plane mirror connected to it can be partially transparent. With an appropriate arrangement of these elements the test field can be seen in free space. The image can be adjusted from infinity to 35 cm by displacement of the test object. Choice of the test object makes possible testing the stereoscopic vision under natural conditions. Other test objects can be used to test further functions of the eyes-monocular, binocular or separately.

A astigmatic eye corrected with a spectacle lens results in an anamorphotic optical system which gives a distorted image of the environment on the retina. Fusion disturbances were ppresent especially in oblique astigmatism. The explanation for this can come either from a displacement of the image points on the retina or from the prismatic side effects of the spectacle lenses.

[A new magnifying visual instrument for ophthalmic surgery (author's transl)].

A binocular loupe on the principle of the prismatic telescope, small in size and light in weight, and which can be firmly by straps to the head of the doctor, has been developed for use in operative ophthalmology, for the clinical visit, consultation and in the office. Its optic properties greatest possible distance from the work which can be varied with additional lenses, variable magnification even into the region of the smallest biomicroscopes with a relatively large angle of vision, good mobility with regard to the object of vision with unchanged adjustment, simultaneous possibility of using semispectacles for presbyopia and easy swinging of the instrument out of and into the line of vision with one hand movement make this instrument very appropriate for the reasons for which it was developed and makes possible also - especially in the surgery of injured eyes, exact observation and assessment of the eye and performance of operations where no operating microscope is present with much greater magnification as that obtained with a customary loupe. This instrument occupies the spare between the possibilities of use of the loupe and the biomicroscope and can in addition be used in the boundary areas of use of the other instruments with great advantage.
